Introduction. Patients with somatic disorders often have sings of concomitant mental distress such as depressive mood, anxiety, apathy, asthenia, which can have a negative impact on the course of the underlying disease and recovery processes. One of the non-medicinal approaches that have been increasingly introduced into medical practice in recent years is the use of techniques based on virtual reality.The paper presents the results of an open comparative study of the effectiveness of a new scenario of virtual reality platform named Flow. Flow was developed as an innovative tool to reduce the severity of negative psychological states.The aim of the study. To evaluate the effectiveness of the virtual reality application Flow in relation to symptoms of anxiety and asthenia in somatic inpatients.Material and methods. 60 hospitalized patients with different diagnoses and comorbid complaints of mental disadaptation were included. 40 patients underwent a course of five daily relaxation sessions in virtual reality in addition to their basic treatment. 20 patients of the control group were treated in accordance with the hospital routine practice. Psychometric scales and questionnaires were used to assess patients condition; indicators of blood pressure and pulse were also measured.Results. The addition of Flow to the course of standard treatment was associated with a significant advantage in reducing affective complaints in patients, both after one session and as a result of the course, and a few days after its completion. Patients of the experimental group also demonstrated a significant decrease in blood pressure after the sessions, and it was most pronounced in individuals with initially high blood pressure. Conclusion. The course of relaxation in virtual reality Flow is an effective and promising method of non-pharmacological care for patients of a somatic hospital in relation to symptoms of anxiety and depressive mood, as well as of hypertension.
Musculoskeletal pain syndromes are one of the most common causes of temporary disability, they are often associated with a significant decrease in the quality of life of patients. Due to the peculiarities of biomechanics (significant physical exertion experienced throughout life, a large volume of movements in various directions), the lumbar spine is especially vulnerable, the lesion of which is often associated with the development of lumbar pain (PB). The mechanisms of formation of PB are diverse, however, as a rule, inflammation is the basis of the pain syndrome. The results of modern studies have convincingly demonstrated the presence of imaging and biochemical markers of the inflammatory process in the area of altered spinal structures, in particular, in intervertebral discs and arch-process joints. There are reasons to believe that it is the focus of inflammation that can be the source of pain, although in the future the role of the active inflammatory process may become less important, and other mechanisms are involved in maintaining pain and other clinical manifestations. In this regard, drugs should be chosen for the treatment of patients with PB, depending on the predominant action - analgesic or anti-inflammatory. In the article, along with the main mechanisms of the occurrence and persistence of PB, modern approaches to the treatment of such patients are considered. The undoubted validity of the use of nonsteroidal anti-inflammatory drugs (NSAIDs) for the treatment of patients with PB is noted. Their most significant clinical effects are noted, which may be useful in the treatment of patients with PB. The features of side effects associated with the use of NSAIDs, including those from the gastrointestinal tract and the cardiovascular system, are considered. Information is provided on the results of studies devoted to the study of the efficacy and safety of dexketoprofen (Dexonal®, Binnopharm Group) in the treatment of patients with PB. The undoubted positive properties of the drug are noted (rapid development of action with a powerful analgesic effect and a favorable safety profile) Dexonal®.
The challenge of postcovid syndrome (PCS) is of great interest due to its wide distribution and variety of clinical signs. The main neurological signs of PCS are discussed. Data on the presumptive mechanisms forming PCS are presented. The potential for using the drug Mexidol to treat patients with PCS is addressed.
Cerebrovascular disorders are one of the main causes of cognitive impairment. Understanding the cause of vascular brain damage, establishing the existing risk factors allow choosing the optimal treatment strategy and slowing the progression of cognitive impairment. The article discusses information about risk factors for cognitive impairment: age, level of education, social isolation and lack of physical activity, excessive alcohol consumption, smoking, exposure to polluted air, hypertension, hearing loss, traumatic brain injury, diabetes mellitus, depression, cerebrovascular diseases, Alzheimer’s disease, genetic predisposition. Information is provided on the possible impact of COVID-19 on the development and progression of cognitive impairment. The possibilities of neurometabolic drugs in the treatment of cognitive disorders are considered. The results of the analysis of the experience of using the drug citicoline (Noocil) in the treatment of patients with cognitive impairment are presented, the possible advantages of prescribing Noocil in the form of a solution for oral administration are noted.
