Research DigestPoor-quality studies suggest that vitamin B 6 use is beneficial in premenstrual syndrome QUESTION Is the use of vitamin B 6 (pyridoxine hydrochloride) effective for relieving overall premenstrual symptoms and depressive premenstrual syndrome? DATA SOURCESStudies were identified by searching MEDLINE , EMBASE (1988EMBASE ( -1996, PsychLIT (1974PsychLIT ( -1997, CINAHL (Cumulative Index to Nursing and Allied Health) (1982)(1983)(1984)(1985)(1986)(1987)(1988)(1989)(1990)(1991)(1992)(1993)(1994)(1995)(1996)(1997), and the Cochrane Controlled Trials Register with the terms premenstrual syndrome, pyridoxine, premenstrual tension, late luteal phase dysphoric disorder, and premenstrual dysphoric disorder. Bibliographies of relevant articles were scanned, and pharmaceutical companies were contacted. STUDY SELECTIONPublished and unpublished studies in any language were selected if they were randomized, double-blind, placebocontrolled studies for which data could be acquired. Studies involving multivitamin supplements were included if the supplement contained Ն50 mg of vitamin B 6 . Authors were contacted if additional information was required. DATA EXTRACTIONData were extracted on participants, dose and preparation of vitamin B 6 , outcome measures, results, withdrawals, side effects, and quality scores. Main outcome was subjective lessening of overall premenstrual symptoms, and a secondary outcome was a decrease in depressive premenstrual symptoms. High-quality studies had scores of 3 on the Jadad scale (maximum: 5 points) and of 6 on the authors' quality rating scale (maximum: 8 points).

Van de vrouwen in de vruchtbare leeftijd heeft volgens de literatuur 3-5% ernstige premenstruele klachten. Deze klachten worden in de DSM-IV gerangschikt onder het syndroom Premenstrual dysphoric disorder (PMDD). SSRI's worden steeds meer voorgesteld als behandeling van eerste keus bij PMS. Doel Vaststellen van de werkzaamheid van SSRI's bij het verminderen van de symptomen bij een ernstig premenstrueel syndroom.Zoekstrategie en selectie Met als zoektermen PMS en de synoniemen daarvan in combinatie met serotonine of SSRI werd gezocht in het register van de Cochrane Menstrual Disorders and Subfertility Group en in de databanken.Referenties zijn nagegaan en indien nodig werden auteurs aangeschreven voor details of ontbrekende gegevens. Ook is gezocht naar ongepubliceerde trials. Die trials werden geselecteerd waarin vrouwen met een tevoren ('prospectief') vastgesteld premenstrueel syndroom, na registratie van klachten en stellen van de diagnose een SSRI of placebo kregen.Van de 31 gevonden onderzoeken werden er 15 ingesloten; 16 werden uitgesloten op methodologische gronden. De gegevens werden volgens de criteria van Jadad (1996) en eigen criteria onafhankelijk beoordeeld. Bij 13 van de 15 onderzoeken was voor de diagnose een DSMclassificatie gehanteerd. Niet alle onderzoeken presenteerden de gegevens van de afzonderlijke symptomen, zodat de gegevens van 10 van de 15 onderzoeken (n=844) konden worden gebruikt voor de subanalyses. De gebruikte meetinstrumenten varieerden nogal. In verband met de heterogeniteit van de trials zijn de gegevens gestandaardiseerd; als effectmaat werd de gemiddelde afname van symptomen gekozen (SMD).Resultaten De symptomen van het premenstrueel syndroom verminderden significant door het gebruik van een SSRI (SMD -0,75, (95%-BI -0,98 --0,51). Dit geldt voor alle klachten tezamen, maar ook voor de lichamelijke en psychische klachten afzonderlijk (4 van de 16 onderzoeken: SMD -0,63 versus -0,51). Prikkelbaarheid (3 onderzoeken) verminderde eveneens, maar niet significant. De werking van SSRI's trad in tegenstelling tot bij depressies, kort na het begin van de behandeling op.Omdat premenstruele klachten nogal eens goed lijken te reageren op placebo werd bij 7 onderzoeken een inloopperiode met placebo gebruikt; de patie¨nten die goed reageerden op placebo werden uitgesloten van het verdere onderzoek. Deze opzet gaf een iets minder groot, maar nog steeds significant effect (SMD -0,71 versus -0,91). Ook bij intermitterend gebruik (slechts twee onderzoeken) van een SSRI was het middel effectief. Fluoxetine werkte iets beter dan sertraline (SMD -0,91 versus -0,71).Er was geen significant verschil tussen onafhankelijke en door de industrie betaalde trials.
