Objective: To analyze actions promoting adherence to tuberculosis treatment that are being carried out by nurses in different countries. Method: Scoping review with selection of articles on the subject in LILACS, MEDLINE, IBECS, BDENF, SciELO, CINAHL, Embase, Web of Science, and Scopus databases. Results: Forty studies, published between 2009 and 2020, allowed the identification of nursing actions in two thematic categories. In the category “Nursing care: TB patients’ specific needs to promote adherence to treatment”, actions involving clinical aspects, professionals’ knowledge and skills, educational and relational processes were identified. In the category “The role of nursing in coping with the social determinants of health to promote adherence to treatment”, interventions related to the strengthening of family and community support, the inclusion of socioeconomic issues in care plans, and respect for cultural differences were highlighted. Conclusion: Nursing work directed to the adherence to disease treatment requires the development of technical, ethical and, above all, political skills, aiming to increase the success of the actions carried out by these professionals.
Objective Investigate which knowledge emerges in the Tuberculosis Control Manuals and how power relations are established for treatment adherence. Method Documentary and qualitative research on five Manuals published between 2002 and 2019, available on the Ministry of Health’s Website, based on Michel Foucault’s theoretical and methodological framework. Results 1. Knowledge: emerges as human resource training; health surveillance actions; medicalization; and multifunctionality of nurses in addressing the social determinants of tuberculosis. 2. Power relations: occur through articulation between different subjects and establish disciplines on the body and the behavior of the person undergoing treatment. Conclusions Certain knowledge/powers constitute the actions of treatment adherence to tuberculosis treatment and normalize the “correct/true” way of conducting disease control. Such behaviors refer to self-responsibility, autonomy and empowerment of the subject and do not consider the countless conditions of vulnerability existing in the history of people with tuberculosis.
Objetivo: identificar e analisar as práticas de Promoção da Saúde, destinadas a grupos e coletivos, realizadas por equipes de Atenção Primária em Saúde (APS) no Brasil nos últimos cinco anos. Método: Revisão integrativa feita a partir do cruzamento do descritor e/ou MeSH “Promoção da Saúde” com as palavras-chave “Atenção Primária em Saúde” e/ou “Atenção Básica em Saúde” na Lilacs,no Ibecs, na BDENF, na SciELO, na CINAHL, na Medline, Embase, Web of Science e Scopus. Foram incluídos artigos originais publicados entre janeiro de 2016 e fevereiro de 2021. Resultados: dos artigos, 37 foram incluídos na revisão e agrupados em uma síntese numérica e temática. As práticas de Promoção da Saúde descritas nesses estudos inscrevem-se majoritariamente na abordagemcomportamental, posto que alguns apresentaram práticas que remetem à abordagem reformista participativa. Considerações finais: identificamos práticas de Promoção da Saúde na APS em seis temáticas: Promoção da Saúde como sinônimo de educação em saúde para a mudança de comportamentos e adoção de hábitos saudáveis; fatores associados à adesão/participação nas práticas ofertadas; descrição e análise de um conjunto de práticas de Promoção da Saúde; Promoção da Saúde como resistência ao modelobiomédico e aumento da qualidade de vida; processos reflexivos e participação como práticas de Promoção da Saúde; e, oferta de espaços para atividade física nas comunidades. Ressalta-se que a necessidade de ampliação do escopo das práticas de Promoção da Saúde, realizadas na APS, pode possibilitar um maior impacto destas sobre os Determinantes Sociais da Saúde.
RESUMO Objetivo Investigar quais saberes emergem nos Manuais de Controle da Tuberculose e como as relações de poder são instituídas para adesão terapêutica. Método Pesquisa documental, qualitativa, que analisou cinco manuais publicados entre 2002 e 2019, disponíveis no Website do Ministério da Saúde, a partir do referencial teórico metodológico de Michel Foucault. Resultados 1. Saberes: a capacitação de recursos humanos; ações de vigilância em saúde; medicalização; e multifuncionalidade dos enfermeiros no enfrentamento dos determinantes sociais da tuberculose. 2. Relações de poder: se articulam entre diferentes sujeitos e instituem disciplinas sobre o corpo e o comportamento da pessoa em tratamento. Conclusões Determinados saberes/poderes constituem as ações de adesão terapêutica ao tratamento da tuberculose e normalizam a forma “correta/verdadeira” de se conduzir o controle da doença. Tais condutas remetem à autorresponsabilização, autonomia e empoderamento do sujeito, mas não levam em consideração as inúmeras condições de vulnerabilidade existentes no histórico das pessoas com tuberculose.
