This blind and randomized-controlled trial analysed chlorhexidine dentifrices in relation to dental plaque, gingivitis, bleeding, calculus and enamel extrinsic staining development. Volunteers in fixed orthodontic therapy used the following dentifrices: 1100 ppmF, NaF (group A, n=27); experimental, 1100 ppmF, NaF and chlorhexidine 0.95% (group B, n=28); and experimental, chlorhexidine 0.95% (group C, n=28). At baseline, after 6, 12 and 24 weeks, clinical examinations were carried out. The gingivitis, bleeding and plaque data were tested by anova and Tukey's post hoc tests. Stain and calculus data were analysed by Kruskal-Wallis and Dunn's post hoc tests (p<0.05). Plaque, gingivitis and bleeding scores improved in all three groups, but up to the 6 and 12 weeks examination the products containing chlorhexidine were statistically better. The chlorhexidine dentifrices significantly increased the mean of the stain index, although most of the patients did not notice the stains. The calculus index was not significantly modified. In summary, this study suggests that the use of dentifrices containing chlorhexidine seems to be effective for the treatment of gingivitis in orthodontic patients, although the intense motivating contact that the volunteers had with the researchers may have also played a role.
This double-blind study assessed the fluoride (F) concentration in whole saliva and F bioavailability after the use of low-F dentifrices with reduced pH. Whole saliva was collected from 10 volunteers after brushing with: experimental dentifrices (pH 5.5) 275, 550 and 1,100 ppm F; commercial 500 ppm F, pH 6.9 and a ‘gold standard’ 1,100 ppm F, pH 6.5. To analyze F bioavailability, 9 volunteers ingested weights of four dentifrices equivalent to 2 mg F: 1,500 ppm F/MFP/CaCO3, pH 9.5; 1,100 ppm F/NaF/silica, pH 5.5; 1,100 ppm F/NaF/silica, pH 7.0 and 1,100 ppm F/NaF/silica, pH 6.5 (‘gold standard’). Ductal saliva and urine were collected. F was analyzed by electrode. Data were tested using ANOVA and Tukey’s post hoc test (p < 0.05). The 550 ppm F/pH 5.5 dentifrice was similar to the ‘gold standard’ in its effect on whole saliva F concentration. The area under the curve of ductal saliva F concentration × time and urinary F excretion rates did not differ among the dentifrices. The results show that acidic low-F dentifrices are effective in increasing salivary F concentration and pH reduction does not seem to affect their F bioavailability.
O tratamento ortodôntico envolve procedimentos, geralmente, realizados durante um longo período. No ato da remoção do aparelho, freqüentemente, são encontradas áreas de desmineralização sob braquetes e bandas, quando já não há cavitações e, na maioria das vezes, inflamações dos tecidos periodontais. Para evitar estas iatrogenias, o clínico geral ou o especialista e toda sua equipe devem estar realmente motivados e envolvidos na aplicação dos conceitos de Odontologia Preventiva em seus pacientes ou, pelo menos, dispostos a delegar esta tarefa a alguém capacitado para fazê-la. Existem métodos relativamente simples para se avaliar o potencial de colaboração do paciente com o tratamento proposto, antes mesmo de seu início, e de se manter este paciente em níveis aceitáveis de saúde bucal. A utilização de recursos áudio-visuais para motivar os pacientes, os reforços positivos, a realização de escovação supervisionada e da limpeza interdentária, orientação sobre higiene, manutenção de saúde bucal e dieta, utilização de diferentes tipos de acessórios ortodônticos e de agentes cimentantes, além do uso, quando necessário, de diversos métodos químicos, são importantes instrumentos a serem utilizados durante o tratamento ortodôntico. Tomando-se as devidas precauções, é possível alcançar uma oclusão funcional e esteticamente aceitável sem deixar que a doença tenha um papel coadjuvante no transcorrer e após o término do tratamento ortodôntico.
INTRODUÇãO: os cirurgiões-dentistas têm a responsabilidade de prevenir doenças, minimizar riscos e promover saúde. Os pacientes também precisam ser despertados sobre o seu papel nos cuidados com a saúde bucal. No caso de pacientes em tratamento ortodôntico, é particularmente difícil manter uma higiene bucal satisfatória devido à presença de bandas, fios e ligaduras. Torna-se, então, indispensável a instituição de métodos preventivos de motivação e orientação para o controle mecânico da placa dentária. OBJETIVO: verificar os efeitos de ações educativas, preventivas e motivacionais sobre a saúde bucal de pacientes em tratamento ortodôntico fixo. MéTODOS: os participantes receberam gratuitamente dentifrício e escova dental durante todo o estudo e instruções sobre higiene bucal foram fornecidas e reforçadas no decorrer dos 6 meses da pesquisa. Foram realizados exames clínicos baseline e após 6, 12 e 24 semanas, para verificação dos índices de Placa, Gengival e Sangramento. RESULTADOS: as condições de saúde bucal dos participantes, que inicialmente eram insatisfatórias, melhoraram significativamente no decorrer do estudo, considerando-se todos os índices. As ações preventivas, educativas e motivacionais realizadas foram estatisticamente eficazes na melhora da saúde bucal dos pacientes ortodônticos. CONCLUSõES: a promoção de saúde e a prevenção de doenças devem fazer parte do atendimento que os ortodontistas direcionam aos seus pacientes, sendo que a orientação e motivação quanto aos cuidados com a saúde bucal devem estar presentes antes e durante o tratamento.
Analisou-se a percepção de mães ou responsáveis sobre a saúde-doença bucal de seus filhos, com idade de 2 a 6 anos, matriculados em uma creche filantrópica de Bauru (SP). Foram realizadas 26 entrevistas, analisadas por metodologia qualitativa. Os entrevistados foram ouvidos sobre a presença de cárie dentária em suas crianças e como compreendiam sua causalidade. Foi realizado o exame de cárie das crianças, utilizando critérios epidemiológicos para o diagnóstico. Na maioria das entrevistas (65,4%) houve concordância de respostas com a condição bucal verificada no exame das crianças. A percepção dos entrevistados sobre a presença da cárie dentária foi relacionada à aparência do dente ("furadinho") e à dor. A compreensão quanto à causalidade da doença fez referência ao cuidado dispensado às crianças pelos entrevistados, acesso ao cirurgião-dentista, ingestão de doces e à fatalidade ("Todo mundo tem cárie, pelo menos um pouquinho tem"). A maneira pela qual as pessoas percebem o processo saúde-doença influencia as práticas e os cuidados em saúde bucal. Buscar compreender estas construções e valores é essencial para se trabalhar, de forma contextualizada, a falta de informação e os conceitos que ainda persistem, como a crença que a cárie dentária é um fenômeno "natural" e não um processo patológico.
