BackgroundCurrently a large number of atypical antipsychotics available in the market are endorsed as better option for treating schizophrenia than the typical antipsychotics. Information regarding the utilization pattern of antipsychotic drugs is lacking in Nepalese population particularly in Western Nepal. By means of this study one is expected to acquire an idea concerning clinician’s preference to the antipsychotic drugs in actual clinical setup. The main objective of the study was to find the commonest antipsychotics prescribed in a tertiary care center among hospitalized patients in Western Nepal.MethodsThis cross sectional study was carried out between 1st January 2009 and 31th December 2010 at Manipal Teaching Hospital, Nepal. The diagnosis of schizophrenia was based on ICD-10 (Tenth revision).The main outcome variables of the study was commonest antipsychotic drug prescribed. Z test, Chi square test and logistic regression were used for analytical purpose. P-value < 0.05 was considered to be statistically significant. This is the first study done on the utilization pattern of antipsychotics drugs among hospitalized patients in Nepal.ResultsOut of 210 cases of schizophrenia, most of the patients were less than 40 yrs. 78.6%, male 61.9%, unemployed 86.7% and having their monthly income less than NPR 10000 /month 80.5%. As far as religion, 78.1% patients were the Hindus and ethnically schizophrenia was common among the Dalit 26.2%. The study revealed that 46.2% of patients were students followed by 25.2% of housewives. Olanzapine was the commonest antipsychotic drug to be prescribed 34.3%. It was observed that the psychiatrists had a tendency of using antipsychotic drugs by trade names [OR 3.3 (1.407, 8.031)] in male patients as compared to female patients.ConclusionAccording to the utilization pattern of antipsychotics, it is concluded that atypical antipsychotics were used relatively more commonly than that of typical antipsychotics. Among the atypical antipsychotic drugs, there is a trend of using Olanzapine during Schizophrenia as compared to other atypical antipsychotic drugs in Western Nepal.

Context:Aging of population is currently a global phenomenon. At least one in 5 people over the age of 65 years will suffer from a mental disorder by 2030. Study of psychiatric morbidities in this age group is essential to prepare for upcoming challenges.Aims:To find out the prevalence of different psychiatric morbidities in elderly population and to find out if there are any age and gender specific differences.Settings and Design:Retrospective review; Psychiatric outpatient department of Manipal Teaching Hospital, Pokhara, Nepal.Materials and Methods:Data for patients ≥ 65 years of age attending the psychiatric outpatient department of Manipal Teaching Hospital, Pokhara, Nepal, from 1st January 2012 to 15th January 2013 were collected retrospectively in a predesigned proforma.Statistical Analysis Used:Risk of having different psychiatric disorders was estimated using odds ratio.Results:The mean age of 120 patients included in this study was 69.67 (SD = 5.94) years. Depressive disorder (26.7%) was the most common diagnosis. There was no statistically significant difference in psychiatric disorders in >75 years compared with ≤75 years except for dementia [odd ratio (OR) (≤75 years/>75 years)=0.055, 95% confidence interval (CI)=0.016; 0.194]. Alcohol dependence syndrome [OR (male/female)=7.826, 95% CI = 1.699;36.705] and dementia [OR (male/female)=3.394, 95% CI = 1.015;11.350] was more common in males.Conclusions:Depressive disorder was the most common psychiatric morbidity among the elderly patients. The odds suffering from dementia increased with increasing age. The odds of having alcohol related problems and dementia were more in males compared with females.

Background: Stroke rehabilitation is a program designed to help the stroke patients to overcome the disability. Few studies have evaluated the profile of stroke patients. To provide information about demographic data & disease pattern among the patients receiving stroke rehabilitation. Methods: A retrospective study was carried out in the Department of Physical Medicine and Rehabilitation (PMR), National Institute of Neuroscience and Hospital (NINSH) Dhaka, Bangladesh for the period of two year from 1st July 2013 to 30th June, 2015. Results: Total five thousand nine hundred thirty nine (n=5939) patients were studied, of which 62.33% were male and 37.67% were female. Maximum patients (27.93%) belong to 51-60 years of age. Major patients (52.67%) came from Dhaka city and most of the studied patients were housewife (25.43%). Largest disease group was ischaemic stroke (81.7%). Regarding service pattern, 69.59% patients received outdoor and 30.41% indoor services. Among clinical profile of stroke, 90.5% were first time onset, almost 99% suffered limb weakness, 24.03% speech problem and 32.6% face involvement. Maximum patients (58.8%) had history of multiple risk factors. Conclusion: Rehabilitation procedures in stroke patients can enable greater return of neurological functions and prevents complication, thereby improves long term outcome and quality of life. Background: Management of osteoarthritis of knee joint is very important. Objective: The purpose of the present study was to observe the efficacy of transcutaneous electrical nerve stimulation (TENS) in the treatment of osteoarthritis (OA) of the knee. Methodology: This study was designed as randomized control trial which was conducted in the Department of Physical Medicine at Dhaka Medical College Hospital, Dhaka Bangladesh from July 2007 to December 2007 for a period of six (06) months. Patients presented with osteoarthritis at the age group of more than 30 years to 60 years with both sexes who were attended in Physical Medicine and Rehabilitation department in the Dhaka Medical College Hospital, Dhaka were selected as study population. The study populations were included by purposive sampling method after fulfilling the inclusion and exclusion criteria and were divided into two groups designated as intervention group (group A) and control group (group B). Group A was treated with TENS, therapeutic exercise, NSAID and ADL. Group B was treated with therapeutic exercise, NSAID and ADL. Results: A total of 60 patients with osteoarthritis (OA) of the knee joint were included in the study group. The mean age of both sexes were 46.67±7.04 years and male (46.83±7.73) patients were higher than female (46.56±6.41). There was significant difference in improvement of pain between two groups after treatment for two week (p=0.002); however, finally it was found that there was no significant difference in improvement of pain between two groups after treatment for 6(six) weeks. There was no significant difference in improvement of range of movement (ROM) between two groups after treatment for six weeks (p=0.946). There was no significant difference in improvement of walking speed between two groups after treatment for six weeks (p=0.611). Conclusion: In conclusion significant decrease of pain with increase of ROM and walking speed is found in all patients group with OA knee regardless of the treatment program.
