The clinical differentiation between epileptic seizures (ES) and non-epileptic seizures (NES) is often difficult and mostly based on the presence or absence of widely recognized features of ES such as tongue biting, falling, incontinence or concomitant epileptic abnormalities in the electroencephalogram (EEG). We retrospectively analysed the records of all patients referred to our Epilepsy Centre for refractory epilepsy and finally diagnosed with NES between 1980 and 1999 ( n= 103), half of them also exhibiting ES. The mean time-lapse between first attack and NES diagnosis was 8.7 +/- 1.3 years and 16.5 +/- 1.4 years for the NES and NES + ES groups respectively. At least one of the usual signs associated with generalized tonic-clonic seizures (tongue biting, falling or incontinence) was reported by 66% and 60% of patients with NES or NES + ES respectively. Interictal EEG abnormalities were recorded in 16% of NES patients vs. 80% of NES + ES patients. In the NES group, delay before establishing the correct diagnosis was significantly longer when the patients exhibited > or =1 symptom(s) of generalized seizures, or when patients exhibited interictal EEG abnormalities. Upon admission, 72% of NES patients and all NES + ES patients were being treated with antiepileptic drugs (AEDs).We conclude that EEG or clinical abnormalities suggestive of epileptic seizures are common in undiagnosed NES patients. Such diagnostic pitfalls, besides considerably delaying NES diagnosis, also considerably delay appropriate treatment implementation.
Dans une société où rapidité et efficacité sont des maîtres mots, à quoi ressemble un service d’urgences psychiatriques ? Comment accueille-t-on des sujets psychotiques qui se présentent encore et encore et qui sont violents envers eux-mêmes ou envers les équipes ? Quel travail peut être réalisé lorsque le sujet est pris dans un phénomène de répétition qui le ramène à la place d’objet-déchet qu’il se sent occuper ? Ophélie est une jeune femme bien connue des institutions psychiatriques. Elle se présente plusieurs fois par semaine aux urgences et elle est mise en observation à de nombreuses reprises du fait d’un comportement suicidaire. Toutefois, elle finira par être sur la liste noire de la plupart des salles fermées de Bruxelles. Après un an, pour elle, un point d’ancrage émerge de l’un des services d’urgences de la ville ; des suivis hebdomadaires se mettent en place ; ses rapports au corps et à l’Autre sont plus apaisés. Ainsi, au regard de la clinique au sein des urgences psychiatriques, répétition et passage à l’acte ne sont pas antinomiques d’une construction de points d’ancrage.
Résumé Le contexte clinique est celui d’une pratique en service de médecine (hémato-oncologie). Notre travail soutient qu’une démarche de diagnostic différentiel de logique subjective structurale (psychose, névrose, perversion), par opposition à une conception générique du « soin psychique », procure, au clinicien, les repères lui permettant d’orienter son accompagnement d’une manière qui soit opérante pour le patient, dans un lieu où les atteintes violentes du corps et l’angoisse de mort sont quotidiennes. Nous exposons ici un cas de logique névrotique obsessionnelle. Il permet d’examiner une série de questions liées à cette logique particulière, de voir comment le traitement singulier de la pulsion est intriqué au rapport du sujet au langage, et de saisir quel usage des entretiens cliniques le sujet parvient à élaborer, dans une situation qui le confronte à l’impensable d’une mort imminente.
Dans la recherche quantitative, la notion de qualité de vie incarne l’intérêt porté à la perception subjective du patient schizophrène sur ses difficultés, ses propres ressources et son bien-être. Au travers d’une revue de la littérature et de l’analyse d’un cas, l’article examine la pertinence de cette notion pour l’établissement d’un diagnostic et l’orientation d’un accompagnement. Il en ressort que, contrairement aux objectifs annoncés, le cadre théorique et méthodologique, qui soutient cette notion, engendre une minimisation de l’expression subjective. Dans le cas de M. V., des éléments essentiels pour l’accompagnement restent ignorés par cette anayse standardisée, qui n’articule pas symptomatologie et logique singulière d’un parcours de vie. Les auteurs concluent que la notion de qualité de vie se situe moins comme une alternative satisfaisante, que comme un complément aux conceptions déficitaires de la schizophrénie.
