Zusammenfassung Hintergrund Mangelernährung stellt eine große Herausforderung im klinischen Alltag dar und ist mit einer erhöhten Mortalität assoziiert. Ziel der Arbeit Im vom Bundesministerium für Bildung und Forschung (BMBF) geförderten Forschungsprojekt Prävention und Behandlung von Mangelernährung bei geriatrischen Patienten im Krankenhaus werden Routinedaten ausgewertet. Ziel ist es, die Ursachen eines in der Klinik erworbenen Mangelernährungsrisikos aufzudecken. Material und Methoden Ausgewertet werden anonymisierte Daten von insgesamt 2058 PflegeheimbewohnerInnen aus 19 Pflegeeinrichtungen mit mindestens 3‑tägigem Klinikaufenthalt. Mangelernährungsrisiko wird mithilfe der kombinierten Screenings MUST/PEMU (Malnutrition Universal Screnning Tool/Pflegerische Erfassung von Mangelernährung und deren Ursachen) , die manifeste Mangelernährung entsprechend den ESPEN-Kriterien (European Society for Clinical Nutrition and Metabolism) bestimmt. Ergebnisse Ein initialer Anteil von 36,2 % (n = 744) weist ein Mangelernährungsrisiko und 12,7 % (n = 262) eine manifeste Mangelernährung auf; die Anteile steigen nach Klinikaufenthalt auf 48,6 % (n = 881) bzw. 14,3 % (n = 259). Die logistische Regressionsanalyse zeigt eine signifikant steigende Wahrscheinlichkeit, ein Mangelernährungsrisiko während des Klinikaufenthaltes zu entwickeln, wenn Einweisungsdiagnosen der ICD-10-Gruppen (International Classification of Diseases and Related Health Problems) Krankheiten des Atmungssystems (OR 2,686; 95 %-KI 1,111–4,575) und Nebendiagnosen der Gruppe Osteopathien und Chondropathien (OR 1,892; 95 %-KI 1,149–3,115) vorliegen sowie bei einem höheren BMI (OR 1,108; 95 %-KI 1,038–1,181), positiveren Gewichtsveränderungen 6 Monate vor Krankenhaus (OR 1,055; 95 %-KI 1,017–1,094) und einer steigenden Anzahl an Behandlungstagen (OR 1,048; 95 %-KI 1,029–1,067). Diskussion Sowohl die Identifikation einer initialen Mangelernährung als auch die Prävention eines innerklinischen Mangelernährungsrisikos stellt eine große Herausforderung für die Kliniken dar. Beides ist aber gleichermaßen erforderlich.
Zusammenfassung Hintergrund Polypharmazie und arzneimittelbezogene Probleme (AbP) stellen eine große Herausforderung bei der Betreuung und Behandlung von Pflegeheimbewohner*innen dar. Viele Interventionsstudien zeigen enttäuschende Ergebnisse, wodurch sich die Frage stellt, ob dies auch an der Auswahl der Zielparameter liegen könnte. Material und Methoden Mithilfe eines Routinedatensatzes aus 6 Langzeitpflegeeinrichtungen soll retrospektiv geprüft werden, ob der kürzlich validierte Medication Risk Score (MERIS) geeignet ist, in einer Population von Pflegeheimbewohner*innen eine Risikoeinschätzung vorzunehmen. Geprüft wurden Assoziationen zwischen MERIS und den abhängigen Variablen Klinikeinweisungen und Stürze über 12 Monate sowie ein Gewichtsverlust ≥ 5 % pro 3 Monate. Ergebnisse Von 495 Bewohner*innen weisen gemäß MERIS 38,6 % (n = 191) eine hohes Risiko für AbP auf. Eine univariate Regressionsanalyse erbrachte bei hohem MERIS ein signifikant erhöhtes Risiko für Krankenhauseinweisungen (OR 2,2; p < 0,001) und einen Gewichtsverlust ≥ 5 % pro 3 Monate (OR 1,95; p = 0,041), jedoch keine signifikante Assoziation mit Stürzen. In der multivariaten Regression steigt das Risiko für eine Krankenhauseinweisung mit einem Diabetes mellitus (OR 1,88; p = 0,004), erfolgtem Sturz im selben Zeitraum (OR 1,91; p = 0,001), positivem MERIS (OR 1,75; p = 0,006) und sinkt bei stabileren Gewichtsverläufen (OR 0,88; p = 0,004). Diskussion Die Ergebnisse deuten das Potenzial des Scores für zukünftige Forschungsprojekte und die individuelle Risikoeinschätzung an. Aufgrund der Einschränkungen bei retrospektiven Sekundäranalysen bedarf es aber weiterer Studien.
Zusammenfassung Zielsetzung Ziel der Arbeit ist es, den Einfluss soziodemografischer und pflegespezifischer Variablen auf die gesundheitsbezogene Lebensqualität (HRQoL) älterer Bewohner*innen in Langzeit-Pflegeeinrichtungen zu untersuchen. Methodik Ein Datenset von 288 Bewohner*innen aus Pflegeeinrichtungen in Deutschland wurde zu Alter, Geschlecht, Ernährungs-, Kontinenzstatus und Sturzereignissen in Zusammenhang mit HRQoL (EQ-5D-5L) analysiert. Ergebnisse Eine altersadjustierte multivariate lineare Regressionsanalyse ergibt signifikante Assoziationen der HRQoL mit Harninkontinenz (überwiegend inkontinent β = –0,200; vollständig inkontinent β = –0,330; p < 0,001) und vorhandenem Blasendauerkatheter (β = –0,236; p < 0,05), einem BMI < 25 kg/m² (β = –0,117; p < 0,001) und weiblichem Geschlecht (β = 0,079; p < 0,05). Schlussfolgerung Die Ergebnisse unterstreichen die Bedeutung von Kontinenzförderung in Langzeit-Pflegeeinrichtungen, um die gesundheitsbezogene Lebensqualität zu steigern
