RESUMEN:La variabilidad del arco aórtico y sus ramas emergentes presentan implicaciones en los abordajes quirúrgicos de tórax y cuello, y posiblemente en el desarrollo de procesos ateromatosos ubicados a ese nivel y accidentes cerebrovasculares. Se evaluaron 122 arcos aórticos de individuos adultos de ambos sexos obtenidos como material de autopsia. Se identificó la configuración general de la emergencia de las ramas colaterales de los arcos y se determinó la morfometría de sus componentes con medición electrónica. Se observó la presencia de la configuración usual (tipo A) en 87 arcos (71,3%); un tronco braqui-bicarotideo (tipo B) en 21 piezas anatómi-cas (17,2%) y en 10 casos (8,2%) la arteria vertebral izquierda se originó directamente del arco aórtico (tipo C); en 4 especímenes (3,3%) se presentó emergencia atípica de las ramas. El calibre de la aorta en el punto previo a la emergencia de sus ramas y justo después de emitir su última colateral fue de 20,1mm (DE 3,19) y 17,2mm (DE 2,57) respectivamente, con una disminución del 14,5%. El calibre de las arterias sublavias (7,7mm, DE 1,10) fue significativamente mayor (P=0,0001) que el de las arterias carótidas (6,4mm, DE 0,78). El diámetro de las arterias carótidas derecha e izquierda fue de 6,5mm (DE 0,81) y 6,3mm (DE 0,75) respectivamente. La arteria subclavia derecha presentó mayor calibre que la izquierda (7,9mm, DE 1,09; 7,6mm, DE 1,12) sin diferencia estadisticamente significativa (P=0,0801). La distancia entre el origen del tronco braquiocefálico y el de la subclavia izquierda fue de 32,8mm (DE 6,16) y la longitud del tronco braquiocefálico fue 30,2mm±5,27. Se destaca la alta frecuencia de arcos con emergencia de dos y cuatro ramas. INTRODUCCIÓNEl arco aórtico (AA), ubicado en el mediastino superior, usualmente emite el tronco braquiocefálico (TBC), la arteria carótida común izquierda (ACCI) y la subclavia izquierda (ASI), que participan en la irrigación de la cabeza y los miembros superiores (Moore & Dalley, 2007;Draken et al., 2010). De la cuarta a la octava semana del desarrollo embrionario los factores que actúan sobre reguladores genéticos determinan modificaciones a nivel del tercer y cuarto arcos aórticos, y la consecuente distribución final variante de las ramas que se derivan de esas estructuras (Momma et al., 1999;Sadler, 2004;Bathia et al., 2005). Background: The great variability of the extrahepatic bile duct (EBD) has clinical-surgical implications. The objective of this study was to characterize the morphological expressions of EBD. Methods and findings This descriptive study, done by injecting a semi-synthetic (Palatal GP40L 85%; styrene 15%) impregnated with mineral green dye into the gallbladder, to determine the anatomical characteristics and the biometrics of EBD in 33 blocks formed from the supra-mesocolic floor.The gallbladder presented a length of 66.9 ± 1.7 mm. The Hartmann´s Pouch was observed in 16 specimens (50%). The lengths of the cystic duct (CD), common hepatic duct (CHD) and common bile duct (choledoch duct) were 27.8 ± 1.6 mm, 28.6 ± 11.39 mm and 60.6 ± 11.6 mm respectively. The presence of accessory hepatic ducts (AHD) was found in three samples (9.1%). In 29 specimens (87.9%) the cystic duct presented medium length, while in 4 cases (12.1%) the CD was long (P < 0, 05). The trajectory of the lateral oblique of CD was present in 23 cases (69.7%), with statistically significant differences in relation to the other trajectories of the CD (P <0, 05). In 18 samples (55%) the cystic-hepatic union appeared at the level of the middle third of the EBD, while in 15 (45%) cases the union of the CD was low (P<0,05). Conclusions The mathematical distribution of the segments of the EBD, carried out in this study, provides reliability to the assessment of the cystic-hepatic junction level. The presence of CHA and the level of the cysto-hepatic junction are important anatomical references, especially in emergency room procedures.
Background: The distal medial striated artery (DMSA) is a small, constant branch that emerges from the base of the anterior cerebral artery (ACA) and is directed backwards to irrigate internal structures such as the nucleous of the base. Objective: This study proposes to evaluate the morphology of the DMSA, useful for clinical and surgical management that compromise this vascular structure. Methods: The DMSA of 71 unclaimed male bodies, who underwent necropsy at the Institute of Legal Medicine and Forensic Sciences of Bucaramanga-Colombia, were evaluated using the perfusion technique of vascular structures with polyester resin. Results: The DMSA was presented in four cases (2.8%) duplicated and with agenesis in 2.1% of the hemispheres. Its origin was 43.9% of the post-communicating segment of the ACA (A2), 10.8% of the pre-communicating segment of the ACA (A1) and 44.6% of the ACA junction site with the anterior communicating artery (AComA). The diameter of the DMSA was 0.5 ± 0.2 mm and its total length was 20.3 ± 4.1 mm. Sinuous trajectory was observed in 73 cases (51.4%). There was a superolateral relationship with the ACA in 48% of the samples. The DMSA was observed in 129 cases (90.9%) as a single trunk while in 6 cases (4.2%) it presented with a common trunk with the orbitofrontal artery (OF). The completion of the DMSA in the anterior perforated substance was given by a single trunk in 129 cases (90.9%) bifurcation in 9 cases (6.3%) and trifurcation in four cases (2.8%). The incidence of sinuous presentation and hypoplasia of the DMSA is considerably higher than previously reported. Conclussions: The importance of the DMSA lies in its involvement with the surgical procedures and interventions of the anterior segment of the arterial cerebral circle, and in the complex clinical management that determine its injury in clinical practice.
