Introduction We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

ResumoA larva migrans cutânea, também conhecida como bicho geográfico, é uma infecção cutânea causada por nematódeos. A resolutividade do tratamento de larva migrans é otimizada quando o indivíduo é cuidado por equipes que compreendem a relevância da interface do meio ambiente ao processo saúde-doença. O objetivo é relatar neste trabalho o caso de uma paciente portadora de larva migranscutânea, cuidada pela equipe de uma Unidade Estratégia Saúde da Família no município de Vassouras/RJ. A visão ampliada de saúde, embasada nos princípios da Ecologia Médica, foi um diferencial para a resolutividade do caso. AbstractCutaneous larva migrans, also known as geographic critter, is a cutaneous infection caused by nematodes. The resoluteness of larva migrans treatment is optimized when the individual is cared for by teams who understand the relevance of the interface of the environment to the health-disease process. The objective is to report in this work the case of a patient with cutaneous larva migrans, cared for by the staff of a Family Health Unit in the city of Vassouras / RJ. The broad vision of health, based on the principles of Medical Ecology, was a differential for the resolution of the case.

O forame oval representa uma comunicação entre as câmaras cardíacas atriais do feto e este se fecha ao nascimento. Em algumas pessoas, no entanto, o mesmo permanece pérvio e favorece o desenvolvimento de acidente vascular isquêmico (AVE) criptogênico. Cabe salientar que a isquemia cerebral de causa desconhecida pode ocorrer em pacientes jovens e que não possuem fatores de risco, por isso é de extrema relevância que o forame oval patente seja incluído como hipótese etiológica, a fim de que o tratamento adequado seja instituído. O objetivo do artigo é relatar o caso de um paciente portador de forame oval patente que evoluiu com AVE isquêmico, correlacionando achados clínicos, métodos diagnósticos e conduta com dados da literatura. Paciente C.F.S., sexo masculino, 53 anos, sem comorbidades, apresentou déficit na acuidade visual associado à escotomas cintilantes bilateralmente e cefaleia bitemporal em aperto enquanto realizava atividades domésticas de baixo esforço. Foi admitido em setor de emergência e após a avaliação inicial, realizou-se ressonância magnética de crânio que comprovou AVE isquêmico. A ecocardiografia transesofágica confirmou o diagnóstico de forame oval patente. A conduta estabelecida foi o fechamento percutâneo transcateter eletivo com dispositivo de oclusão do forame oval. É de suma importância o avanço dos estudos do AVE criptogênico, uma vez que o manejo adequado pode interferir na sobrevida e qualidade de vida do paciente, ao evitar a progressão de sequelas ou um novo evento.

Resumo Introdução: Os receptores de transplante renal (TR) apresentam um alto risco para desfechos adversos de infecções, tais como a COVID-19. Métodos: Revisamos retrospectivamente todos os receptores de TR com COVID-19 documentada entre 1º de Março de 2020 e 15 de Março de 2021, e analisamos as características, curso clínico, tratamento e desfechos dos pacientes. Resultados: Identificamos 123 pacientes, 72% do sexo masculino, com uma média de idade de 54,5±13,0 anos. Vinte por cento eram assintomáticos, 7% apresentaram transmissão nosocomial, e 36% do restante necessitaram de internação. Quase todos os pacientes internados receberam oxigênio, 30% necessitaram de ventilação mecânica invasiva (VMI), mais da metade apresentou lesão renal aguda, com 10% necessitando de diálise, e 20% foram a óbito. A incidência foi comparável à da população portuguesa, mas a taxa de mortalidade foi quase quatro vezes superior (TMP de 3,768 (IC 95%: 1,723-7,154). Maior índice de massa corporal (OR 1,275; P=0,001), menor função do enxerto basal (OR 0,968; P=0,015), e transmissão nosocomial (OR 13,836; P=0,019) foram associados à demanda de oxigênio, enquanto sexo feminino (OR 3,801; P=0,031) e menor função do enxerto renal basal (OR 0,955; P=0,005), mas não índice de massa corporal, foram associados à VMI e/ou óbito. Conclusão: A taxa de mortalidade em pacientes com TR foi mais elevada do que na população em geral e a função renal basal mais baixa foi o marcador mais consistente para desfechos adversos.

Introduction: Kidney transplant (KT) recipients have a high risk for adverse outcomes from infections, such as COVID-19. Methods: We have retrospectively reviewed all KT recipients with documented COVID-19 between March 1, 2020, and March 15, 2021, and analyzed patients’ characteristics, clinical course, treatment, and outcomes. Results: We identified 123 patients, 72% were male, with a mean age of 54.5±13.0 years. Twenty percent were asymptomatic, 7% had a nosocomial transmission, and 36% of the remainder required hospitalization. Almost all admitted patients received oxygen, 30% required invasive mechanical ventilation (IMV), more than a half had acute kidney injury, with 10% requiring dialysis, and 20% died. Incidence was comparable to that of the Portuguese population, but the mortality rate was almost four times higher (SMR of 3.768 (95% CI:1.723-7.154). Higher body mass index (OR 1.275, P=0.001), lower baseline graft function (OR 0.968, P=0.015), and nosocomial transmission (OR 13.836, P=0.019) were associated with oxygen demand, whereas female gender (OR 3.801, P=0.031) and lower baseline kidney graft function (OR 0.955, P=0.005), but not body mass index, were associated with IMV and/or death. Conclusion: Mortality rate in KT patients was higher than in the general population and lower baseline kidney function was the most consistent marker for adverse outcomes.
