Objetivo: Analisar o papel da Atenção Básica na detecção precoce do câncer de pele, visando mostrar as principais estratégias utilizadas. Revisão bibliográfica: O câncer de pele é a neoplasia que possui maior incidência no Brasil. A identificação do câncer em fase inicial, precoce, ou ainda de lesões pré-malignas, tais como queratoses actínicas e nevos melanocíticos displásicos, é um fator de bom prognostico devido ao tratamento precoce, com maiores chances de cura e menores sequelas cirúrgicas. Devido ao fato de a pele ser um órgão de fácil visualização, é importante orientar a população em relação aos sinais de alerta. Os médicos de família devem fazer o rastreio baseado no exame clínico visual da pele, incluindo a regra do ABCDE (assimetria da lesão, bordas irregulares, heterogeneidade de cor; diâmetro mais de 6mm e evolução ao longo do tempo), pode-se realizar também exames auxiliares, como a dermatoscopia, exame confocal e mapeamento das lesões estudos feitos pelo dermatologista. Considerações finais: É importante orientar o paciente em reação da realização do autoexame, pelo menos uma vez no mês, para estar sempre atento às possíveis alterações.
A cardiomiopatia de Takotsubo (CT) é uma forma aguda e reversível de insuficiência cardíaca e caracteriza-se por anormalidades transitórias da parede do ventrículo esquerdo. Essa síndrome, de acordo com os sintomas e as alterações nos exames, tem como diagnóstico diferencial o infarto agudo do miocárdio (IAM). METODOLOGIA: Trata-se de uma revisão narrativa de literatura com busca nas bases de dados Literature Analysis and Retrieval System Online (MEDLINE), Scientific Electronic Library Online (SciELO) e Literatura Latino-americana e do Caribe em Ciências da Saúde (LILACS). Foram utilizados os descritores "Myocardial infarction", "Differential diagnosis'', "Takotsubo cardiomyopathy", "Analogies and differences" e "Cardiac". RESULTADOS: Foram identificados cinco artigos relevantes para a realização deste estudo. Trata-se de trabalhos realizados entre 2008 e 2021, sendo o principal, o do ano de 2020. DISCUSSÃO: A CT é uma doença subdiagnosticada, o que dificulta o conhecimento a respeito de sua incidência. Os exames que permitem o diagnóstico diferencial são ventriculografia esquerda e cineangiocoronariografia. Podem ser utilizados também o eletrocardiograma e exames de biomarcadores. CONCLUSÃO: O conhecimento médico a respeito da semelhança entre IAM e CT é relevante para a boa conduta do mesmo perante esses casos, a fim de melhorar o prognóstico dos pacientes.
Objetivo: Correlacionar a infecção pela dengue e o desenvolvimento da Síndrome de Guillain-Barré (SGB). Revisão bibliográfica: Os resultados demonstram que existe uma associação da infecção pela dengue, e o desenvolvimento da SGB, principalmente no verão, período de predominância do vírus. De acordo com a Organização Mundial da Saúde (OMS), a dengue é uma patologia febril associada a dois ou mais dos sintomas: cefaléia, dor retroorbital, mialgia/ artralgia, exantema e petéquias com prova do laço positiva, de início agudo com duração de 2-7 dias. Já a SGB é uma polirradiculoneuropatia inflamatória desmielinizante aguda de caráter autoimune, ou seja, o sistema imunológico reconhece e ataca o sistema nervoso levando à lesão da bainha de mielina. Considerações finais: Portanto, este estudo revelou uma associação entre SGB com essa arbovirose, sendo que a febre é a principal sintomatologia para identificação da infecção pelo arbovírus, seguido de mialgia, artralgia e exantema, bem como paresias e/ou plegias progressivas e ascendentes são fatores determinantes para a identificação da SGB. Dessa maneira, a prevenção contra os vetores é fundamental para diminuir o índice de infecções e consequentemente diminuir os casos de possível desenvolvimento da SGB, melhorando o prognóstico dos pacientes.
Objetivo: Identificar se a origem da estenose laringotraqueal pós-intubação (PILS) é uma complicação direta da covid-19 ou se acontece em pacientes em eventos isolados/raros. Revisão bibliográfica: Os resultados apresentados na literatura demonstram que existe a associação direta da PILS em pacientes com COVID-19, principalmente naqueles que ficam um longo período em intubação orotraqueal (IOT) e nos que possuem alguma comorbidade prévia, como Hipertensão Arterial Sistêmica, Diabetes, Asma ou alguma outra síndrome respiratória. Considerações finais: Este estudo demonstrou que a estenose de traqueia pós-intubação é uma complicação menos comum, mas que ocorre, em pacientes com COVID-19, principalmente nos susceptíveis e que ficam mais de 3 semanas em ventilação mecânica. Além disso, um bom trabalho em equipe nas unidades de terapia intensiva (UTIs), entre os clínicos gerais, médicos de pronto-socorro e outros profissionais médicos é fundamental para direcionar esses pacientes aos cuidados de um centro terciário com experiência adequada no campo da cirurgia das vias aéreas, melhorando o prognóstico do paciente.
