OBJETIVO: Avaliar prospectivamente o tratamento cirúrgico de pacientes portadores de obesidade mórbida, realizadas no Serviço de Cirurgia Geral do HC-UFPE (SCG/HC-UFPE). MÉTODO: No período de novembro de 1997 a fevereiro de 2001 foram operados 228 pacientes oriundos do Ambulatório de Cirurgia Bariátrica do SCG/ HC-UFPE e da clínica privada dos dois primeiros autores. A idade variou de 20 a 59 anos de idade (média de 34 anos). O índice de massa corporal médio (IMC) foi de 46 Kg/m², variando entre 35 e 98 Kg/m². O sexo feminino foi predominante, constituindo 58% dos casos. RESULTADOS: A gastroplastia com bypass jejunal em Y de Roux (operação de Fobi/Capella) foi realizada em 207 pacientes (47 com colecistectomia), gastroplastia vertical (Operação de Mason) em três casos, sete casos de operações descritas por Scopinaro, cinco casos de utilização de banda gástrica por via laparoscópica, cinco casos gastroplastias verticais em Y de Roux (operação de Fobi/Capella) videolaparoscópica e um caso de " Switch duodenal". As comorbidades mais freqüentemente encontradas foram: hipertensão arterial sistêmica (68%), refluxo gastroesofágico (34%), varizes de membros inferiores (36%), artropatia degenerativa (31%), dislipidemia (21%), e diabetes (19%). A presença de colelitíase, com indicação de colecistectomia, ocorreu em 21% dos pacientes. A ferida operatória foi o principal sítio das complicações pós-operatórias: formação de seroma e infecção ocorrendo em 33% e 8,1% respectivamente. Complicações graves ocorreram em 18 pacientes (7,8%), com quatro óbitos (1,8%). O período médio de internamento foi de 4,3 dias. O acompanhamento ambulatorial demonstrou que a perda ponderal média em 12 meses atingiu 41% do peso pré-operatório. CONCLUSÃO: O tratamento cirúrgico da obesidade mórbida é uma alternativa eficaz e eficiente no controle do excesso de peso.
RESUMO: Objetivo: O objetivo do presente estudo foi avaliar o quadro clínico, a incidência, os fatores predisponentes e a evolução de pacientes com fitobezoar após Gastroplastia Vertical e Y de Roux. Método: No período de Novembro de 1997 à Janeiro de 2004, foram realizadas 512 operações para o tratamento da Obesidade mórbida, seguindo a técnica proposta por Fobi/Capella (Septação gástrica com reconstituição em Y de Roux Proximal). Em dez pacientes foram identificados fitobezoar. Resultados: A incidência de fitobezoar, causando algum tipo de obstrução gastrointestinal, foi de 1,95% (10/512). Todos os casos de fitobezoar estavam relacionados ao fio de sutura inabsorvível do tipo prolene. Sete pacientes apresentaram quadro semioclusivo e de obstrução intestinal e foram tratados com laparotomia exploradora, ressecção da anastomose enteroenteral e confecção de nova enteroenteroanastomose. Nos três pacientes com bezoar situado na anastomose gastrojejunal, a secção do fio e a retirada do corpo estranho foram realizadas por endoscopia digestiva alta. Conclusão: A confecção de anastomose intestinal com fio inabsorvível predispõe a formação de bezoar em pós-operatório de gastroplastia (Rev. Col. Bras. Cir. 2006; 33(1): 35-38). ABSTRACT Background:The aim of this study was to evaluate the relation, incidence, clinical features, predisposing factors and phytobezoar in patients submitted to bariatric surgeries. Methods: During November 1997 and January 2004, 512 bariatric surgeries (Fobi/Capella bypass) were performed to treat morbid obesity. In 10 patients phytobezoar was identified. Results: The incidence of phytobezoar resulting in gastrointestinal obstruction was 1,95% (10/512). All cases of phytobezoar were related to unabsorbed suture stitch (prolene). In seven patients an intestinal semi-occlusive or total intestinal obstruction was treated by laparotomy, resection of the anastomosis and confection of a new enteroanastomosis. In three patients the bezoars were located in the gastrojejunal anastomosis and was successfully treated by upper endoscopic procedure. Conclusion: The use of unabsorbed suture stitch predicted the formation of phytobezoar in the late postoperative period of gastroplasty patients.
Nonalcoholic steatohepatitis is highly prevalent in morbidly obese patients, but there was no positive correlation between aminotransferases and degree of obesity and liver histopathology. Hypertriglyceridemia and waist-hip ratio were positively correlated with the intensity of disease.
The authors report five cases of cystic dilatation of the common bile duct Type I (Todani’s classification) in adults patients, in Division of General Surgery of a University Hospital, treated over a- 25-year- period from 1974 to 1999, among 16.057 operations, and not previously published. Diagnosis was obtained by operative cholangiogram (OC) in the first case, percutaneous transhepatic cholangiogram on the second one (PTHC) and by ultrasonography (US), endoscopic retrograde cholangiopancreatography (ERCP), and operative cholangiogram (OC), respectively, on the last three cases. The second patient had an adenocarcinoma arising in the cystic wall associated with peritoneal metastasis. The first two cases were treated by internal drainage and the last three by excision of the cysts and bilioenteric anastomoses. Classification, incidence, etiology, diagnosis, malignization and surgical treatment of biliary cystic disease (BCD) were revised, with the conclusion that resection must be the preferable method of treatment, when possible, especially due to the concern of malignization.
