started to complain of side effects of NCTx and could not stand NCTx furthermore. Fortunately, the tumor showed interval decrease in size and we decided to perform operation. Left trisectionectomy with caudate lobectomy and lymphadenectomy was successfully carried out despite fibrotic changes in upper abdomen.Histopathological examination revealed cholangiocarcinoma with decreased size and clear resection margins. Additionally nodal metastasis, intrahepatic metastasis, and microscopic vascular and perineural invasion were found.NCTx using gemcitabine and cisplatin could decrease tumor size and offer chance of radical surgery for locally advanced IHCC in selected cases. Further studies are warranted to evaluate the effect of NCTx on locally advanced IHCC.
