ObjectiveTo determine the incidence of postoperative atrial fibrillation in patients
undergoing on-pump and off-pump coronary artery bypass grafting.MethodsA retrospective study with analysis of 230 medical records between January 2011
and October 2013 was conducted.ResultsFifty-six (24.3%) out of the 230 patients were female. The average age of patients
undergoing on-pump coronary artery bypass grafting was 59.91±8.62 years
old, and off-pump was 57.16±9.01 years old (P=0.0213). The
average EuroSCORE for the on-pump group was 3.37%±3.08% and for the
off-pump group was 3.13%±3% (P=0.5468). Eighteen (13.43%)
patients who underwent off-pump coronary artery bypass grafting developed
postoperative atrial fibrillation, whereas for the onpump group, 19 (19.79%)
developed this arrhythmia, with no significant difference between the groups
(P=0.1955).ConclusionOff-pump coronary artery bypass grafting did not reduce the incidence of atrial
fibrillation in the postoperative period. Important predictors of risk for the
development of this arrhythmia were identified as: patients older than 70 years
old and presence of atrial fibrillation in perioperative period in both groups,
and non-use of beta-blockers drugs postoperatively in the on-pump group.
Resumo A obesidade grau III é definida como acúmulo excessivo de gordura no corpo, caracterizada por IMC > 40kg/m2 e está relacionada a uma série de comorbidades, sendo, por isso, de fundamental importância a adoção de um tratamento adequado para reduzir os efeitos deletérios na saúde do indivíduo. A gastrectomia vertical laparoscópica está bem estabelecida para o tratamento. Entre as complicações pós-operatórias possíveis, a trombose da veia porta destaca-se, apesar de rara. Em nosso estudo, foram analisados oito casos de pacientes submetidos à gastrectomia vertical laparoscópica que apresentaram como complicação pós-operatória o desenvolvimento de trombose da veia porta. Observamos em nossa série o aumento na incidência de trombose venosa portomesentérica, especialmente nos pacientes que não seguiram as recomendações de hidratação via oral no pós-operatório. A maioria dos pacientes com essa complicação responde de maneira positiva à anticoagulação, com recanalização completa ou parcial da porta. O tratamento com anticoagulantes mostra-se eficaz, devendo ser considerado como primeira opção. A hidratação vigorosa também tem se mostrado conduta essencial no pós-operatório desses pacientes, devendo ser sempre estimulada.
ResumoAneurismas da artéria temporal superficial são incomuns segundo a literatura. A grande maioria é secundária a traumatismos ou cirurgia na região temporal, sendo que 95% dos casos evoluem para pseudoaneurismas. Entretanto, os aneurismas verdadeiros ou espontâneos são extremamente raros e representam 8% dos casos de aneurismas da artéria temporal superficial, sendo estes, geralmente, de origem aterosclerótica. Aneurismas temporais espontâneos podem coexistir com outras lesões vasculares, incluindo aneurismas intracranianos. Nosso relato trata de um paciente com aneurisma de artéria temporal superficial esquerda, de origem aterosclerótica, no qual foi realizada a excisão cirúrgica, sob anestesia geral.
The most common cause of recurrence of varicose veins in the lower limbs is inadequate dissection of the internal saphenous arch, with ligature that is not flush to the saphenofemoral junction. Direct access to the scar tissue of previous surgery should be avoided to the maximum because of the high risk of bleeding and of lymphatic injuries. Access proximal to the scar tissue, initially at the common femoral vein, above the saphenofemoral junction, continuing caudally to the saphenous arch, proved to be an effective and relatively simple technique when compared to medial, lateral and direct approaches.Keywords: saphenous vein; recurrence; varicose veins.
ResumoA maior causa de recidiva das varizes dos membros inferiores é a inadequada dissecção da crossa da safena interna, com a ligadura não rente da junção safenofemoral. O acesso direto ao tecido cicatricial de uma cirurgia prévia deve ser evitado ao máximo pelo elevado risco de sangramento e de lesões linfáticas. O acesso proximal ao tecido cicatricial, abordando inicialmente a veia femoral comum acima da junção safenofemoral, seguindo-a em direção caudal até a crossa da safena, mostrou-se uma técnica eficiente e relativamente simples quando comparada às abordagens medial, lateral e direta.
RESUMOO acidente vascular encefálico é pouco frequente na população jovem. Entretanto, o impacto permanece devastador sobre a qualidade de vida, sendo uma importante causa de óbito e incapacitação. O objetivo do trabalho foi relatar um caso infrequente de AVE isquêmico em território da artéria basilar em uma paciente de 16 anos, que apresentava dois fatores de risco potenciais, o forame oval patente e o uso de anticoncepcional oral.
