ABSTRACT:Objective: To describe the care measurements provided to patients with self-reported diabetes mellitus in Brazil. Methods: Data from the Brazilian National Health Survey (2013) were used. This is a crosssectional population-based study in which the subjects with self-reported diabetes mellitus answered questions concerning their use of health services and access to medicine. Results: The prevalence of self-reported diabetes mellitus was 6.2%, while 11.5% of the population had never undergone a glucose testing. From the adults with diabetes mellitus, 80.2% had taken medications two weeks before the interview, 57.4% used the Popular Pharmacy Program, 73.2% received medical care, and 47.1% were cared for in the Health Basic Units. In 65.2%, the physician who cared for them in the last appointment was the same from previous ones, 95.3% of the patients were able to perform the required complementary examinations, and 83.3% could go to the appointments with a specialist. About 35.6 and 29.1% of the subjects with diabetes mellitus reported feet and eyes examination, respectively. About 13.4% declared previous hospitalization owing to diabetes or any complications, and 7.0% mentioned limitations in their daily activities owing to the disease. In general, women and the elderly people, those with higher education levels, white, and those living in the south and southeastern regions showed a higher prevalence of the disease and greater access to services, medicine, and appointments. Discussion: The care reported by patients with diabetes, which is essential to maintain their quality of life and prevent serious outcomes, seemed, in most cases, to be adequate.
Novos desafios epidemiológicos e demográficos demandam novas formas de organizar os sistemas de saúde. O presente ensaio propõe a telessaúde como ferramenta organizativa, capaz de suavizar o triângulo de ferro da atenção à saúde e de facilitar a busca pelo triple aim, pelo seu potencial de aumento do acesso e qualidade com redução de custo. A integração da telessaúde ao processo de referência e transição entre serviços assistenciais aumenta a resolutividade da Atenção Primária à Saúde (APS), favorece a coordenação do cuidado, promove adesão terapêutica, diminui reinternações e estimula a prevenção quaternária. Este ensaio propõe a telessaúde como metasserviço que confere densidade tecnológica à APS e permite que ela se torne coordenadora efetiva do cuidado, passando a organizar o fluxo de informações, pessoas e insumos. Frente às inovações propostas, é essencial avaliar o impacto de ações já existentes de telessaúde para viabilizar a sua aplicação como metasserviço de saúde.
Background Primary health care (PHC) delivery in Brazil has improved in the last decades. However, it remains unknown whether the Family Health Strategy teams are meeting the health needs of the population. Objectives To describe the reasons for encounter (RFEs) in PHC in Brazil and to examine variations in RFEs according to sex, age and geographic region. Methods This descriptive study is part of a national cross-sectional study conducted in 2016. The sample was stratified by the number of PHC physicians per geographic region. Physicians who had been working for at least 1 year in the same PHC unit were included. For every participating physician, 12 patients aged ≥18 years who had attended at least two encounters were included. Patients were asked about their RFEs, which were classified according to the International Classification of Primary Care. Results In 6160 encounters, a total of 8046 RFEs were coded. Seven reasons accounted for 50% of all RFEs. There was a high frequency of codes related to test results, medication renewal and preventive medicine. RFEs did not vary significantly by sex or geographic region, but they did by age group (P < 0.001). The rates of prescriptions, requests for investigations and referrals to specialized care were 71.1%, 42.8%, and 21.3%, respectively. Conclusion This novel study opened the ‘black box’ of RFEs in PHC in Brazil. These findings can contribute to redefining the scope of PHC services and reorienting work practices in order to improve the quality of PHC in Brazil.
Resumo Treinamento especializado, provimento e fixação de profissionais na Atenção Primária à Saúde são desafios prementes no Brasil. A recente expansão dos Programas de Residência em Medicina de Família e Comunidade coexiste com lacunas na literatura sobre o efeito deste processo. O objetivo do trabalho é explorar a perspectiva do gestor municipal de saúde acerca das estratégias para o fortalecimento das residências e do papel destas na formação profissional e na qualificação da atenção. Trata-se de um estudo de análise quantitativa e qualitativa, com aplicação de questionário a gestores de municípios cenários destes programas. As respostas de 48 sujeitos foram submetidas a estatística descritiva e análise de conteúdo. Os resultados revelam um esforço em incorporar o Médico de Família e Comunidade na rede de atenção à saúde, uma percepção do potencial das residências no incremento da qualidade da atenção e da formação profissional e fragilidades nas ações para melhoria da estrutura e organização dos serviços com residentes. Vislumbra-se assim o potencial das residências para a superação de problemas históricos da Atenção Primária à Saúde brasileira, se atrelada a ações de fortalecimento do serviço, dos recursos humanos e do próprio programa.
Evaluation of health care coordination and health care networks ordination by Primary Health Care -a proposal of items for the evaluation of these attributes Introdução: A formação de redes de atenção à saúde (RAS) tem se mostrado como uma forma de organização de sistemas de saúde eficaz para responder a alguns dos atuais desafios epidemiológicos e estruturais destes sistemas. O Brasil vem caminhando para a regionalização e a conformação de redes de atenção à saúde. A fim de avaliar o desenvolvimento das RAS, propõe-se um conjunto de itens para inferir dois dos atributos das redes: a coordenação do cuidado individual e a ordenação das RAS pela Atenção Primária à Saúde. Métodos: Utilizou-se o Método Delphi, composto pelas seguintes etapas: definição inicial dos itens a serem avaliados e composição do grupo de experts, realização de quatro rodadas do Delphi e avaliação dos resultados. Resultados: O produto final foi uma lista de 69 itens, destes, 59 questões atingiram estabilidade de opinião entre os experts e podem ser divididas em: um grupo de 44 itens que atingiram grau de consenso alto entre os experts para inclusão do item na lista final; um grupo de 12 itens que atingiram grau de consenso intermediário; e um grupo de 3 itens que atingiram grau de consenso baixo. A construção desta proposta de itens para avaliação das RAS tem por intuito central colaborar com as pesquisas relacionadas às avaliações sistêmicas dos serviços de saúde. Brazil has been moving towards the regionalization and conformation of HCN. In order to assess the development of HCNs, this research offers to define initial items that in the future will compose an evaluation tool capable of measuring two attributes of health care networks: individual care coordination and planning of HCNs by the Primary Health Care. Methods: We used the Delphi method, which involves the following steps: initial setting of items to be evaluated and composition of the experts group, conducting four rounds of Delphi and evaluation of results. Results: The final product was a list of 69 items, of these, 59 items reached stability between experts and can be divided into: a group of 44 items that achieved a high degree of consensus among the experts to include the item in the final list; a group of 12 items that reached an intermediate degree of consensus, and a group of 3 items that reached a low degree of consensus. The construction of this initial proposal of items for evaluation of HCN has the main goal of collaborating with researches related to the more systemic evaluations of health services.
Este artigo é um breve relato histórico sobre a formação do Grupo de trabalho de Mulheres na Medicina de Família e Comunidade da SBMFC (GT-MMFC), que ocorreu em 2016. Em paralelo, descreve-se as principais ações do Wonca Working Party on Women & Family Medicine e do próprio GT-MMFC até os dias atuais. Os objetivos do artigo são registrar a construção deste grupo de trabalho assim como fomentar e fortalecer o debate de todas as dimensões relacionadas às mulheres e a medicina de família e comunidade e a equidade de gênero.
