Objective: To characterize patients with syndromic craniosynostosis with respect to their neuropsycholinguistic abilities and to present these findings together with the brain abnormalities. Methods: Eighteen patients with a diagnosis of syndromic craniosynostosis were studied. Eight patients had Apert syndrome and 10 had Crouzon syndrome. They were submitted to phonological evaluation, neuropsychological evaluation and magnetic resonance imaging of the brain. The phonological evaluation was done by behavioral observation of the language, the Peabody test, Token test and a school achievement test. The neuropsychological evaluation included the WISC III and WAIS tests. Results: Abnormalities in language abilities were observed and the school achievement test showed abnormalities in 66.67% of the patients. A normal intelligence quotient was observed in 39.3% of the patients, and congenital abnormalities of the central nervous system were observed in 46.4% of the patients. Conclusion: Abnormalities of language abilities were observed in the majority of patients with syndromic craniosynostosis, and low cognitive performance was also observed.
O objetivo deste trabalho foi descrever os aspectos fonoaudiológicos de processamento auditivo, leitura e escrita de um paciente do gênero masculino com diagnóstico de síndrome de Silver-Russell. Aos dois meses de idade o paciente apresentava déficit pôndero-estatural; frontal amplo; orelhas pequenas, proeminentes e com baixa implantação; palato ogival; discreta micrognatia; esclera azulada; manchas café-com-leite; sobreposição do primeiro e segundo artelhos à direita; refluxo gastroesofágico; voz e choro agudos; atraso leve no desenvolvimento neuropsicomotor; e dificuldade de ganhar peso, recebendo o diagnóstico da síndrome. Na avaliação psicológica, realizada aos 8 anos de idade, o paciente apresentou nível intelectual normal, com dificuldades cognitivas envolvendo atenção sustentada, concentração, memória verbal imediata e processos emocionais e comportamentais. Para avaliação da leitura e escrita e de seus processos subjacentes, realizada aos 9 anos de idade foram utilizados os testes de Compreensão Leitora de Textos Expositivos, Perfil das Habilidades Fonológicas, Teste de Discriminação Auditiva, escrita espontânea, Teste de Desempenho Escolar (TDE), teste de Nomeação Automática Rápida e prova de memória de trabalho fonológica. Apresentou dificuldades em todos os testes, estando as pontuações abaixo do esperado para sua idade. Na avaliação do processamento auditivo foram realizados testes monóticos, dióticos e dicóticos. Foram encontradas alterações nas habilidades de atenção auditiva sustentada e seletiva, memória sequencial para sons verbais e não-verbais, e resolução temporal. Conclui-se que o paciente apresenta alterações na aprendizagem da leitura e escrita que podem ser secundários a síndrome de Silver-Russell, porém tais dificuldades também podem ser decorrentes das alterações em habilidades do processamento auditivo.

The Apert Syndrome (AS) is considered a syndromic craniosynostosis that corresponds to 4,5% of all of the craniosynostosis. The different aspect of AS can be relevant in the process of language development because this syndrome can present abnormalities in the orofacial structures and hearing system. Difficult with child's social adaptation and neuropsychological development are also related to language disorders in these individuals. The speech and language evaluation used in this study will allow us to characterize the profile language of the patients with AS. The objective of the present study is to investigate the components of the language considering reception and expression in different modalities. Related aspects of speech and psycholinguistics abilities were also evaluated. The sample was composed by 9 patients with AS, 3 were of the masculine gender and 6 of the feminine gender. The age in the moment of the evaluation varied from 9 to 26 years old. The speech and language evaluation consisted of a clinical evaluation and application of standardized tests, being these Illinois Test of Abilities Psycholinguistics (ITPA), the Test of receptive vocabulary for images Peabody (TVIP), the Token Test and the Test of School Performance. During the clinical evaluation 100% of the patients presented expression phonology and appropriate reception, 33,3% demonstrated reception difficulty and expression difficulty in the components syntax, semantics and pragmatic and 11,1% presented exclusively expression difficulty in the components syntax, semantics and pragmatic. It was observed that 55,5% of the patients didn't present difficulty in any component of the language. Using the TVIP 66,6% patients demonstrated a low performance considering receptive vocabulary. Three patients presented difficulty at the Token Test. The Test of School Performance presented inferior score in 87,5% (8 patients were evaluated). Three subjects had difficulty in the subtestes of ITPA. Related to speech aspect, 44,4% of the patients presented hypernasality, 11,1% presented hyponasality, 33,3% had distortions in the speech, none of them manifested ruptures in the fluency neither vocal alterations.
