Introducciónl hombre es infectado, con alta frecuencia, por amebas desnudas de vida estrictamente parasitaria, pertenecientes a la familia Endamoebidae, en especial en países en vías de desarrollo como el nuestro 1 .En 1965 se publicaron los primeros casos de infección humana por una ameba de vida libre 2 , aunque estudios retrospectivos han encontrado casos desde 1909 3 . Las primeras amebas de vida libre descritas pertenecen a la familia Vahlkampfiidae y género Naegleria 3 . A partir de 1977, se precisó que otras amebas de vida libre, de las familias Hartmannellidae y Acanthamoebidae, producen también infección humana 3 . Más recientemente, se ha agregado una nueva especia, Balamuthia mandrillaris, una ameba de la familia Leptomyxidae que afecta al ser humano 4,5 .Actualmente, se han sistematizado muy bien los principales cuadros anátomo-clínicos producidos por amebas de vida libre, a saber: encefalitis amebiana granulomatosa, meningoencefalitis amebiana primaria, y la queratitis acantoamebiana 3,6-8 .En Chile, se han aislado del medio ambiente varias especies de amebas de vida libre 9,10 y se ha publicado, según nuestra información, sólo un caso de queratitis Encefalitis amebiana granulomatosa por Balamuthia mandrillaris. Primer caso diagnosticado en Chile Granulomatous amebic encephalitis caused by Balamuthia mandrillaris. First case diagnosed in ChileThe case of a 7 month-old baby boy is presented, with a history of several weeks of evolution of personality changes, convulsions and fever. While he was hospitalized, the brain image study showed multifocal cavity lesions, which varied from millimeters to 4 cm of diameter, not producing mass effect. Their biopsy revealed necrotizing encephalitis. In spite of treatment, the lesions progressed and the boy died. The autopsy demonstrated an extensive necrotizing encephalitis with fibrinoid necrotic arteritis, signs of organization, and the presence of parasitic elements with characteristics of trophozoites and cysts of free -living amoebas belonging to the genus Acanthamoeba or Balamuthia. In addition, hypoplasia-dysplasia of the thymus and signs of shock were found. The morphologic elements correspond to the disease described as granulomatous amebic encephalitis and the agent found was identified as Balamuthia mandrillaris (Centers for Diseases Control and Prevention, Atlanta, USA ).

Objectives:To establish the etiology of pneumonia and to compare the yield of diagnostic techniques for diagnosis of Pneumocystis jiroveci and Mycobacterium tuberculosis infections in HIV-1-infected patients. Patients and Methods: Subjects underwent sputum induction and bronchoalveolar lavage (BAL). Gram, Ziehl-Neelsen, silver stain (SS) and immunofl uorescense staining (IF) for P. jiroveci, fl uorescent stain for mycobacteria, PCR for P. jiroveci and M. tuberculosis, aerobic, fungal and mycobacterial cultures, respiratory viruses and CMV cultures were performed on the sputum and BAL. IgM for Mycoplasma pneumoniae and Chlamydophyla pneumoniae, and Legionella pneumophila urinary antigen were also obtained. Results: Sixty patients were included. An etiologic diagnosis was made in 97%. Pneumocystis jiroveci was the most frequent etiology (58%) followed by Streptococcus pneumoniae (12%), and Mycobacterium avium complex (12%). Mycobacterium tuberculosis was found in 5%. Conclusions:The comparison of diagnostic methods for P. jiroveci showed a higher sensitivity of IF and SS in BAL than in sputum, however PCR was equally sensitive in both samples. With this approach a precise etiologic diagnosis was reached in the great majority of patients. The most common etiology was P. jiroveci. El sistema respiratorio es un sitio donde frecuentemente se producen infecciones fúngicas (Pneumocystis jiroveci, anteriormente denominado Pneumocystis carinii), bacterianas y por micobacterias. La tasa de neumonía por P. jiroveci en los pacientes con recuentos de linfocitos CD4 ≤ a 200 céls/mm 3 , sin uso de profi laxis, es de 8,08 casos por 100 personas-año 2 . La neumonía bacteriana se presenta con una incidencia entre 2,3 y 10,8 episodios por 100 personas-año en los pacientes infectados por VIH (comparado con 0,9 por 100 personasaño en individuos sanos) 3,4 . La broncoscopia con lavado bronco-alveolar (LBA) se considera el método estándar para el diagnóstico etiológico de la neumonía en pacientes infectados por VIH. No obstante, puesto que es un método invasor, el esputo inducido aparece como una estrategia diagnóstica sensible y costo-efectiva. Two cases of vitamin D toxicity in infants, caused by mistaken administration by their families aie presented: One received 9.000.000 I.U. in 15 days, the other 4.200.000 U.I. in 7 days. The clinical features were similar in both cases, consisting in nausea, vomiting, polyuria, dehydration, reduced muscular tonus, hipeicalcemia, hipercalciuria and disturbances of renal concentration ability. Treatment with low calcium diet, intravenous fluids administration and oral prednisone was done with good results. Serum calcium levels were normal at four days of the treatment but the disturbances on renal concentration ability persisted for almost one year in the first patient.El 1,25 dihidroxi-colecalciferol (1,25 (OH) 2 D 3 ) es una sustancia que se considera actualmente una hormona derivada del colecalciferol o Vitamina D 3 .Los requerimientos de Vitamina D son mayores a menor edad y a mayor velocidad de crecimiento. Dependen de una adecuada exposition a la luz solar y de la normal absorcion de grasa en el intestine 1 . La Vitamina D se absorbs en el intestine y se deposita en el higado, donde se hidroxila a 25 hidroxi Vitamina D (25-OH-D 3 ) que coristituye su principal metabolite circulante 2-3 . En el rifion la 25-OH-D 3 se transforma principalmente en 1,25 (OH) 2 D 3 proceso de hidroxilacion que es influido por variaciones en la concentraci6n de calcic y fosforo y tiene mecanismos de retroalimentacjon positiva con la paratohormona y negativa con la tirocalcitonina 3 " 4 . La Vitamina D ejerce su efecto. ftsiologico mediante la 1,25 (OH) 2 D 3 . En los cases de intoxicacion se acumula 25-OH-D 3 4 ' 5 . La Academia Americana de Pediatria 1 recomienda 400 U.I. diarias de Vitamina D, cantidad que previene deficiencias desde el nacimiento hasta la adolescencia en un nifio normal. Se puede producir sobredosis o intoxicaci6n con Vitamina D por un error en la administration producido por incorrecta prescription medica, mala interpretation de ella por parte del paciente o sus familiares, automedicaci6n o por no tener en cuenta otras fuentes de aporte de la Vitamina como es el caso de los alimentos fortiflcados y los suplementos vitaminicos.
