Background: The treatment of choice for patients with cirrhosis and HPS is LT. The clinical manifestations associated with hypoxemia result in limitations and a poor health-related quality of life of affected patients. The present report aims to study the differences in outcomes between patients with PaO 2 < 50 mm Hg and those with PaO 2 ≥ 50 mm Hg. Methods: This was a retrospective study of 21 patients under 18 years of age conducted from 2001 to 2018; the patients were divided into 2 groups: G1-PaO 2 ≥ 50 mm Hg, 11 patients, and G2-PaO 2 < 50 mm Hg, 10 patients. Demographic, clinical, laboratory, and perioperative data; outcome variables; and post-transplant survival were compared between the groups.Results: In total, 2/11 (18.2%) patients in G1 and 8/10 (80%) patients in G2 required supplemental oxygen therapy at home (P = .005). Patients in G2 required prolonged MV (median 8.5 days in G2 vs 1 day in G1, P = .015) and prolonged ICU and hospital stays (P = .002 and P = .001, respectively). Oxygen weaning time was longer in G2 (median 127.5 days) than in G1 (median 3 days; P = .004). One (9.1%) patient in G1 and three (30%) patients in G2 died (P = .22). The survival at 90 months was 90.9% in G1 and 70% in G2 (P = .22).
Conclusion:The survival between groups was similar. Patients with very severe HPS required a longer MV time, longer ICU and hospital stays, and a longer O 2 weaning time than those with mild, moderate, or severe HPS.
normal. Ultrasound abdomen showed multiple gall stones. Patient underwent laparoscopic cholecystectomy and had uneventful recovery. On 10th post-operative day, patient presented with complaints of dull aching pain in the right hypochondrium on inspiration and vomiting. Patient had tenderness in right hypochondrium on abdomen examination. Ultrasound showed a liver abscess. Contrast enhanced computed tomogram abdomen revealed 13 Â 7 cm abscess in the right lobe of liver. Patient underwent ultrasound guided pig-tail drainage. 500 cc pus was drained. Culture showed E.coli. Patient was discharged with oral antibiotics. Pigtail catheter was removed after ultrasound abdomen showed no residual collection. Conclusion: Delayed complications after ERCP can occur in patients with choledocholithiasis with cholangitis. Careful evaluation of symptoms is required on follow-up.
Introdução: O transplante hepático é empregado no tratamento do carcinoma hepatocelular em pacientes cirróticos que se enquadram nos Critérios de Milão com resultados adequados. A quimioembolização é utilizada como padrão ouro para downstaging em pacientes com lesões que se apresentam no momento do diagnóstico fora dos critérios para transplante de fígado. Objetivo: Nosso objetivo foi avaliar a resposta tumoral em pacientes submetidos a quimioembolização no pré-transplante hepático, através da análise do fígado explantado após a realização da intervenção cirúrgica. Material e Método: Foram avaliados 17 pacientes submetidos a transplante ortotópico de fígado entre junho de 2009 e dezembro de 2012 por carcinoma hepatocelular, que apresentaram tratamento prévio através da quimioembolização com lipiodol e doxorrubicina no Hospital das Clínicas da Unicamp. Foram excluídos pacientes com lesão infiltrativa ou com trombose de porta não tumoral. A resposta ao tratamento foi avaliada por tomografia computadorizada de abdome multislice. A média de realização do downstaging ao transplante foi de 3,38 meses. Resultados: A análise do fígado explantado demonstrou necrose tumoral menor que 25% em três pacientes, de 25-50% em dois pacientes, de 50-90% em três pacientes e maior que 90% em nove pacientes. A média de sobrevida foi de 75% em um ano, 64% em três anos e 47% em cinco anos. Não houve nenhuma recidiva nesse grupo estudado. O comprometimento microvascular à avaliação do explante nas lesões residuais foi visto em 17,64% dos casos. Conclusão: A quimioembolização é um procedimento utilizado em pacientes cirróticos com carcinoma hepatocelular para fins de downstaging e inclusão do paciente nos critérios para transplante ortotópico de fígado, apresentando resposta satisfatória com indução de necrose tumoral e sem evidência de recidiva em longo prazo após a realização do transplante.
