Introdução: O transplante hepático é empregado no tratamento do carcinoma hepatocelular em pacientes cirróticos que se enquadram nos Critérios de Milão com resultados adequados. A quimioembolização é utilizada como padrão ouro para downstaging em pacientes com lesões que se apresentam no momento do diagnóstico fora dos critérios para transplante de fígado. Objetivo: Nosso objetivo foi avaliar a resposta tumoral em pacientes submetidos a quimioembolização no pré-transplante hepático, através da análise do fígado explantado após a realização da intervenção cirúrgica. Material e Método: Foram avaliados 17 pacientes submetidos a transplante ortotópico de fígado entre junho de 2009 e dezembro de 2012 por carcinoma hepatocelular, que apresentaram tratamento prévio através da quimioembolização com lipiodol e doxorrubicina no Hospital das Clínicas da Unicamp. Foram excluídos pacientes com lesão infiltrativa ou com trombose de porta não tumoral. A resposta ao tratamento foi avaliada por tomografia computadorizada de abdome multislice. A média de realização do downstaging ao transplante foi de 3,38 meses. Resultados: A análise do fígado explantado demonstrou necrose tumoral menor que 25% em três pacientes, de 25-50% em dois pacientes, de 50-90% em três pacientes e maior que 90% em nove pacientes. A média de sobrevida foi de 75% em um ano, 64% em três anos e 47% em cinco anos. Não houve nenhuma recidiva nesse grupo estudado. O comprometimento microvascular à avaliação do explante nas lesões residuais foi visto em 17,64% dos casos. Conclusão: A quimioembolização é um procedimento utilizado em pacientes cirróticos com carcinoma hepatocelular para fins de downstaging e inclusão do paciente nos critérios para transplante ortotópico de fígado, apresentando resposta satisfatória com indução de necrose tumoral e sem evidência de recidiva em longo prazo após a realização do transplante.
