ГБУЗ НО «Городская больница № 35», Нижний Новгород В основе операций Ramirez и TAR (transversus abdominis release), выполняемых пациентам с большими послеоперационными грыжами, лежат обширная диссекция слоев брюшной стенки и имплантация больших эндопротезов. Экспериментальных работ, где было бы исследовано возможное влияние указанных операций на развитие спаечного процесса в брюшной полости, до настоящего времени не представлено. В эксперименте на кроликах моделировали большие послеоперационные грыжи, затем проводили реконструкцию брюшной стенки способами Ramirez и TAR. Оба варианта операций реализованы как с протезирующей пластикой, так и без таковой. Через 30 суток оценивали спаечный процесс в брюшной полости по Вандербильтской шкале. Данные анализировали с помощью тестов Shapiro-Wilk, Kruscal-Wallis, Mann-Whitney. Отличия между вариантами операции Ramirez с протезированием и без него не были статистически значимыми (Ramirez с сеткой -1,6 балла, та же операция без сетки -0,67, p=0,236942, Mann-Whitney). Отличия между вариантами TAR с протезированием и без него были достоверными (TAR с сеткой -2,77 балла, TAR без сетки -1,2, p=0,046296, Mann-Whitney). Спаечный процесс после сепарационных пластик с протезированием брюшной стенки был более выражен (2,26 балла), чем без него (0,95), отличия значимы (p=0,025379, Mann-Whitney.) Сепарационная пластика брюшной стенки в условиях эксперимента может приводить к развитию спаечного процесса в зоне операции. Данное явление менее выражено после передней сепарации. Диссекция в глубоких слоях брюшной стенки (TAR) и размещение в этой зоне эндопротеза оказывают достоверное влияние на формирование висцеропариетальных адгезий. Objective. To study the current Russian and foreign literature dedicated to the problem of application of organometallic compounds immobilized on drugdelivery in the treatment of purulent-inflammatory disease of the skin and soft tissues. Methods. The modern Russian and foreign literature, available in the Pubmed, Medline, Springer, Scopus, e-LIBRARY databases were reviewed according to the problems of purulent-inflammatory diseases, skin and soft tissue infections, the integrated approach to the treatment of purulent-inflammatory diseases, synthesis, immobilized organometallic compounds. Results. The observational study of the specificrecent achievements in the modification of antimicrobial biomaterials is presented. Metal ions havea broad range of antimicrobial activity (especially on proliferation and remodeling), possess by bacteriostatic and bactericidal effect, demonstrate multiple inhibitory effects against bacterial strains and havebeen proveneffectivein improvingwoundhealing in all its phases. Natural products and especially biologically active metals such as silver, copper, zinc and germanium, are believed to be an alternative for the development of perspective biomaterials with antimicrobial properties. In recent years, new approach for the production and application of therapeutic and diagnostic drugs based on the immobilization or grafting of drugsubstances on polymer carriers has been developed. At present, namely the immobilized compounds that have opened the way to the creation of prolonged-action drugs with low toxicity and allergenicity. Conclusion. Template synthesis of new organometallic drug compounds is considered to be a promising direction in the wound infection treatment, which requires further experimental and clinical study.
