Intrathoracic gossypiboma, a retained surgical sponge in the thoracic cavity, is a rare but serious complication of thoracic surgeries. A 70-year-old man presented with an eight-month history of cough. He had undergone coronary artery bypass surgery eight years ago. The posteroanterior chest X-ray revealed a well-marginated homogeneous opacity at the left hemithorax with striped appearance in the center. Thoracic CT revealed a pleural-based mass at the left lower lobe with a hyperdense rim. After the diagnosis of gossypiboma, it was removed surgically. Although rare after thoracic surgery, gossypibomas need to be considered in the differential diagnosis in case of respiratory symptoms.
Bu çalışmada Türkiye'de küçük hücreli dışı akciğer kanserinde mediastinal lenf nodlarının pozitron emisyon tomografi-bilgisayarlı tomografi değerlendirmesinde standardize tutulum değeri için yeni bir eşik değeri belirlendi. Ça lış ma pla nı: Kasım 2006-Şubat 2010 tarihleri arasında pozitron emisyon tomografi-bilgisayarlı tomografi uygulanan küçük hücreli dışı akciğer kanserli toplam 207 hasta prospektif olarak incelendi. Bu hastalardan pozitron emisyon tomografi sonrası invaziv evrelemesi yapılan 143 hasta (125 erkek, 18 kadın; ort. yaş 62.1 yıl; dağılım 39-85 yıl) çalışmaya alındı. Pozitron emisyon tomografinin duyarlılığı, seçiciliği, pozitif prediktif değeri, negatif prediktif değeri ve doğruluk oranları ≥2.5'lik bir maksimum standardize tutulum değeri eşik değeri ve yeni saptanan maksimum standardize tutulum değeri eşik değeri kullanılarak hesaplandı ve karşılaştırıldı. Bul gu lar: Mediastinal lenf nodlarında maksimum standardize tutulum değeri eşik değeri 2.5 olarak kabul edildiğinde; duyarlılık, seçicilik, pozitif prediktif değer, negatif prediktif değer ve doğruluk oranları sırasıyla %45.3, %78.9, %55.8, %71 ve %66.4 idi. Metastatik lenf nodlarında yeni maksimum standardize tutulum değeri eşik değeri 4.8 olarak belirlendi. Yeni maksimum standardize tutulum değeri eşik değeri 4.8'e göre bu değerler sırasıyla; %39.6, %91.1, %72.4, %71.9 ve %72 idi. İki farklı maksimum standardize tutulum değeri eşik değeri kullanıldığında, sadece seçicilik oranları arasında anlamlı farklılık vardı (p= 0.022). So nuç: Bu çalışmada mediastinal lenf nodlarının değerlendirilmesinde pozitron emisyon tomografinin duyarlılığı literatürde bildirilenlerden daha düşük idi. Bu durum ülkemizde tüberküloz gibi granülomatöz enfeksiyonların sık görülmesine bağlı olabilir. Pozitron emisyon tomografi sonuçları ülkelere göre değerlendirilmeli ve ülkemizde yapılacak çok merkezli çalışmalar ile özellikle mediastinal lenf nodu metastazı için yeni bir maksimum standardize tutulum değeri eşik değeri hesaplanmalıdır. OBJECTIVE: Pulmonary metastasectomy for the treatment of metastatic colorectal cancer is essential, but high ranked evidence of survival benefit is lacking. Here, we aimed to examine the prognostic factors after pulmonary metastasectomy in patients with colorectal cancer.METHODS: This is a single-center, retrospective hospital-based observational case series study. We reviewed data for 607 patients with metastatic colorectal cancer (mCRC) who were treated and observed from 2012 to 2019. Of the 607 patients with mCRC, 87 were with solitary lung metastases. Of the 87 patients, 39 were not appropriate for metastasectomy, while 15 patients recognized as suitable candidates by the multidisciplinary thoracic tumor board rejected metastasectomy. Consequently, only 33 patients were included in the final analysis.RESULTS: Rectum was detected as the primary site in 16 (48.5%) patients. Over 80% of patients had metachronous lung metastases, with a median of 29.0 months from initial diagnosis. Video-assisted thoracic surgery with wedge resection was performed in 20 (60.6%) patients. Over 90% of patients had solitary metastasis resected, with 97% of R0 resection. Median tumor size was 23.0 mm (min: 10; max: 90). Adjuvant treatment was given to 31 (93.9%) patients, while neoadjuvant treatment was given only to 8 (25%) patients. Of the 33 patients, there were 25 (75.7%) relapses. The most frequent site of relapse was lung in 15 (45.5%) patients. Interestingly, there were only 4 (12.2%) patients who had a relapse in the liver after lung metastasectomy. We found that median disease-free survival (DFS) and overall survival (OS) were 43.0 (13.0-73.0) and 55.0 (31.6-78.4) months, respectively. CONCLUSIONS: Pulmonary metastasectomy was associated with significantly long-time survival rates in mCRC (43 months of DFS and 55 months of OS). The second relapse occurred in 25 (75.7%) patients, with isolated lung metastases in nearly half of the patients (45.5%).Therefore, lung metastases in mCRC were unique and a multidisciplinary team including a thoracic surgeon should manage these patients.

Therefore; madness, regardless of how it is discussed, represents to the unordinary. Though madness if it is studied with Foucaultian view is an phenomenon which can reveal the paradigms of government and power that are being improved in the history. This study is examining the varieties of madness that are accepted in the society and the relation between the madness and the power implemented by the modern governments. Madness as it is considered is not only psychological problem but also a frame used in the construction of the normal to ease to practice power implemented by the modern society.
Locating and removing ectopic parathyroid tissue may result in more than 20% failure. Seven cases which were detected by single photon emission computed tomography/computed tomography (SPECT/CT) imaging with Tc-99m sestamibi and treated with minimally invasive surgical methods were presented. The cases that underwent SPECT/CT due to high parathormone (PTH) level after total/subtotal parathyroidectomy operation, and detected to have mediastinal ectopic parathyroid were included. The success of the surgery was assessed with preoperative and postoperative PTH levels. Video-assisted thoracic surgery (VATS) was performed on five of the cases and video-assisted mediastinoscopy (VAM) was performed on remaining two. The mean age was 52.4±19.0. There were six women and one man. Mediastinal adenomas ranging from 1.5 to 6 cm in size, were successfully treated after single attempt. If the mediastinal ectopic parathyroid adenomas are fully localized before surgery, it can be treated with minimally invasive surgical methods.
