OBJECTIVE:To evaluate the quality of life for persons affected by age-related macular degeneration that results in monocular or binocular legal blindness. METHODS: An analytic transversal study using the National Eye Institute Visual Functioning Questionnaire (NEI VFQ-25) was performed. Inclusion criteria were persons of both genders, aged more than 50 years old, absence of cataracts, diagnosis of agerelated monocular degeneration in at least one eye and the absence of other macular diseases. The control group was paired by sex, age and no ocular disease. RESULTS: Group 1 (monocular legal blindness) was composed of 54 patients (72.22% females and 27.78% males, aged 51 to 87 years old, medium age 74.61 ± 7.27 years); group 2 (binocular legal blindness ) was composed of 54 patients (46.30% females and 53.70% males aged 54 to 87 years old, medium age 75.61 ± 6.34 years). The control group was composed of 40 patients (40% females and 60% males, aged 50 to 81 years old, medium age 65.65 ± 7.56 years). The majority of the scores were statistically significantly higher in group 1 and the control group in relation to group 2 and higher in the control group when compared to group 1. CONCLUSIONS: It was evident that the quality of life of persons with binocular blindness was more limited in relation to persons with monocular blindness. Relationship between diabetic retinopathy severity and the timespan between the endocrinopathy diagnosis and the first ophthalmic examination Keywords: Diabetes mellitus; Diabetic retinopathy Purpose: To investigate whether the time interval between type 2 diabetes mellitus (DM) diagnosis and the first fundoscopic examination is related with the presence and the severity of diabetic retinopathy (DR) observed. Methods: A survey of 105 type 2 DM patients referred to ophthalmologic evaluation in the "Hospital das Clinicas" (HC), University of São Paulo Medical School (USPMS). Results: Regarding classification of DR in the 105 patients, 15 (14.28%) did not show signs of DR, and 90 (85.72%) exhibited them on fundoscopy. Sixty patients underwent laser therapy, and 46.66% reported poor control of DM. Only 15.23% of DM patients were adequately screened for DR on the first year of their DM diagnosis. Among the 36 patients (34.30%) examined within five years of DM diagnosis, 58.33% did not present or demonstrate signs of mild DR and 22.20% of proliferative DR; 30 patients underwent an ophthalmologic examination after more than eleven years of DM diagnosis, 21.62% did not exhibit signs of DR and 59.46% were classified as proliferative DR. Conclusion: This study showed a statistically significant relationship between the time interval from the diagnosis of type 2 DM and the first fundoscopic examination with the severity of DR.ABSTRACT
Trabalho realizado no Hospital Universitário Professor Edgard Santos, Universidade Federal da Bahia (UFBA) -Salvador (BA), Brasil RESUMOObjetivo: Descrever oito casos de inflamação esclerosante idiopática da órbita, incluindo a análise clínica e histopatológica. Métodos: Estudo retrospectivo através do livro de registros do Laboratório de Patologia Ocular (LPO) do Hospital Universitário Professor Edgard Santos, Universidade Federal da Bahia, no período de 1974 a 2010. Além de revisão de lâminas de todos os casos e das imagens clínicas e de exames complementares recuperadas dos arquivos pessoais dos autores. Resultados: Dentre o total de espécimes submetidos ao LPO (7546) apenas 8 casos receberam diagnóstico final de inflamação esclerosante idiopática da órbita. Em todos os casos houve piora ou deterioração da acuidade visual à despeito do tratamento, com exceção de um, que permaneceu com a mesma visão (percepção luminosa). Em nosso estudo, observamos envolvimento orbitário predominantemente apical. Todos os nossos pacientes foram submetidos à biópsia incisional com finalidade diagnóstica. Também submeteram-se à corticoterapia e em um caso foi associada à radioterapia anti-inflamatória; em dois destes casos, foi utilizado também ciclofosfamida. Dois pacientes com acuidade final sem percepção luminosa tiveram que ser submetidos à exenteração parcial da órbita para tratamento de dor incontrolável. Conclusão: A inflamação esclerosante orbitária idiopática permanece como uma doença pouco conhecida, de péssimo prognóstico visual.Descritores: Inflamação/patologia; Doenças orbitárias/patologia; Órbita/patologia; Prognóstico; Diagnóstico diferencial
Avaliação da camada de fibras nervosas na retinopatia diabética tratada por panfotocoagulação com laser de argônio INTRODUÇÃO O Early Treatment Diabetic Retinopathy Study (ETDRS) definiu as estratégias do tratamento da retinopatia diabética (RD) de acordo com a sua classificação (1) , bem como a técnica de aplicação recomendada (2)(3) . Segundo este estudo, a panfotocoagulação da retina é recomendada a partir da RD não proliferativa grave ou muito grave e para RD proliferativa, preconizando a técnica "full scatter photocoagulation" com laser de argônio (4) . A energia liberada pelo laser é primeiramente absorvida pelo epitélio pigmentado da retina e, mesmo utilizando baixa intensidade e intervalo de tempo reduzido, a lesão não é limitada a esta estrutura. Foi demonstrado que micropulsos de laser com baixa energia lesaram também os fotorreceptores (5) . Outro estudo evidenciou lesões em todas as camadas da retina, Resultados: A amostra foi composta por 27 pacientes (27 olhos) portadores de diabetes mellitus tipo 2. A idade variou entre 41 e 64 anos (média de 53,7 ± 6,2 anos), sendo 10 (37%) pacientes do sexo masculino e 17 (63%) do feminino. Quanto ao tipo de retinopatia, 22,2% apresentavam RD proliferativa e 77,8%, RD não proliferativa muito grave. Houve aumento significante nas medidas da espessura da camada de fibras nervosas, permanecendo nos setores temporal, 3 e 4 horas após seis meses de seguimento. Não foi observada qualquer redução na espessura em todos parâmetros analisados. Conclusão: Não foi evidenciada, a curto e médio prazo, redução na espessura da camada de fibras nervosas em portadores de retinopatia diabética tratada por panfotocoagulação que possa ser identificável por meio da tomografia de coerência óptica. Por outro lado, alguns setores mostraram aumento na espessura durante o seguimento. RESUMO
