Quiste epidermoide; Quiste renal
ResumenIntroducción: El quiste epidermoide renal es una condición rara, con muy pocos reportes en la literatura 1---3 . Se realiza la revisión de la literatura disponible y la descripción de un caso sobre un quiste epidermoide renal. Materiales y métodos: Se presenta el caso de una mujer en la quinta década de la vida, con dolor lumbar derecho y hematuria macroscópica intermitente. La urotomografía muestra una lesión en la pelvis renal derecha. La pielografía evidencia un defecto de llenado y la ureteropielorrenoscopia evidencia un gran cálculo en cáliz superior. Con estos hallazgos se realiza una nefrolitotomía percutánea (NLP). El reporte histopatológico confirma la presencia de un quiste epidermoide renal. Conclusiones: El quiste epidermoide renal es una condición rara, con muy pocos reportes en la literatura. Su etiología no es clara. Hasta el momento, en los pocos casos reportados, la mayoría de los quistes renales están localizados en el parénquima renal y tan solo en algunos reportes están localizados en el sistema colector. El manejo en todos los casos ha sido la nefrectomía parcial o radical.
El embarazo esplénico primario es considerado la forma más rara de embarazo extrauterino, con sólo pocos casos bien documentados reportados. En el presente reporte se informa un caso de embarazo esplénico en una mujer de 32 años con historia de aborto recurrente, que se presentó con cuadro de abdomen agudo y hemoperitoneo. El embarazo esplénico requiere esplenectomía de urgencia y debe ser considerado en el diagnóstico diferencial en mujeres en edad reproductiva con aborto recurrente y abdomen agudo.
El Síndrome Mielodisplásico (SMD) es un grupo de neoplasias hematológicas cuyo diagnóstico se basa en la presencia de citopenia inexplicada, displasia de alguna de las líneas mieloides y anormalidades citogenéticas típicas. Los estudios moleculares han permitido avanzar en el estudio de la fisiopatología y se han incorporado también al abordaje diagnóstico, permitiendo la clasificación en subtipos específicos, así como la predicción de respuesta a terapias específicas y el pronóstico. A pesar de esto, el diagnóstico aún recae en una adecuada historia clínica, en el análisis morfológico, en la exclusión de otras entidades que cursan con displasia y en los estudios de citogenética. La citometría de flujo no hace parte de los criterios diagnósticos, pero puede ser de utilidad en casos dudosos. El pronóstico es variable en función del subtipo pero, en general, es pobre y además del IPSS-R, se han identificado otras variables que inciden en el pronóstico, sobre todo en los casos asociados a terapia.
