Background As the process and nature of developing sensitivity to house dust mites (HDMs) remain not fully studied, our goal was to establish the pattern, nature and timeframe of house dust mite (HDM) sensitization development in patients in Ukraine as well as the period when treatment of such patients would be most effective. Methods The data of the multiplex allergy test Alex2 was collected from 20,033 patients. To determine age specifics of sensitization, descriptive statistics were used. Bayesian Network analysis was used to build probabilistic patterns of individual sensitization. Results Patients from Ukraine were most often sensitized to HDM allergens of group 1 (Der p 1, Der f 1) and group 2 (Der p 2, Der f 2) as well as to Der p 23 (55%). A considerable sensitivity to Der p 5, Der p 7 and Der p 21 allergens was also observed. The overall nature of sensitization to HDM allergens among the population of Ukraine is formed within the first year of life. By this time, there is a pronounced sensitization to HDM allergens of groups 1 and 2 as well as to Der p 23. Significance of sensitization to Der p 5, Der p 7 and Der p 21 allergens grows starting from the age of 3–6. Bayesian Network data analysis indicated the leading role of sensitization to Der p 1 and Der f 2. While developing the sensitivity to group 5 allergens, the leading role may belong to Der p 21 allergen. Conclusion The results obtained indicate the importance of determining the sensitization profile using the multi-component approach. A more detailed study of the optimal age for AIT prescription is required as the pattern of sensitization to HDMs is formed during the first year of life.
Background. This study determined the efficacy of sublingual allergen-specific immunotherapy (SLIT) in Ukrainian children younger than 5 years of age with allergic rhinitis and bronchial asthma sensitized to house dust mite allergens. Methods. A cohort prospective study was conducted over a two years period with assessment of the sensitization towards inhalant allergens, measured invivo by wheal size and number of positive reactions on the standard skin prick test (SPT). SPSS was used to analyse any statistical correlation. Results. A total of 125 atopic asthma or/and allergic rhinitis with the mean age of 4.11 ±0.83 who were sensitized to house dust mites Dermatophagoides pteronyssinus and/or Dermatophagoides farina were included. 47 children received two-years SLIT to treat house dust mite allergies using standardized extract of sublingual allergens containing a mixture of house dust mites (Der. pteronyssinus and Der. farinae) in the correlation of 1(0,175 НЕР):1(0,175 НЕР) (Diater, Spain). These patients were mono-sensitized with Der. pteronyssinus and Der. farinae or were SPT poly-sensitized, but these patients only had house dust mite allergy symptoms. In this group the assessment of clinical efficacy of SLIT showed the significant differences in VAS before treatment and 6 months after its receiving. The symptom "nasal obstruction" was assessed as 2.3±-1.6 points at the beginning of the therapy, and the data reduced almost twice (1.3±-1.1); Р <0,05 after 6 months of SLIT; then (12 and 24 months) it decreased to 0.91±0.9 and 0.34±0.5; Р <0,01, correspondingly. Conclusion. This study has shown that using of SLIT in atopic asthma or/and allergic rhinitis children under 5 year sensitized to house dust mites is an effective and safe treatment method and allows to quickly (over the first 6 months of SLIT) control the symptoms. The data of comparative analysis in the group of patients who didn't receive SLIT has pointed to high frequency of disease symptom occurrence after the end of baseline therapy.
BACKGROUND In pediatric ages, sublingual immunotherapy (SLIT) allows allergic children to approach the inhaling desensitizing therapy. However, SLIT causes quite a few compliance problems and sometimes it is difficult to carry it out for the required long term. The alternative is subcutaneous immunotherapy (SCIT), commonly performed with a syringe. This technique hasn't changed much in time and it is not without risks or discomfort. This has led to the search for alternative ways of vaccine administration, in order to reduce discomfort to children by improving compliance and diminishing the potential risks of adverse reactions. OBJECTIVE The study aimed to assess the patient's perceived pain and the safety of a new way of administration of SCIT, with a needleless device. METHODS Children with grass or mite-induced allergic rhinoconjunctivitis and/ or bronchial asthma were prescribed a glutaraldehyde-polymerized allergenic extract (Allergovac Polymeryzed®, Bial Aristegui, Italy). Each dose was divided in two parts: half injected with the traditional syringe (dose A), half in the other arm with a needleless device (Injex, Greytree, Ross on Wye, UK; dose B). Patients were blindfolded. The perceived pain and the difficulties of the procedure were registered on a dedicated VAS scale immediately (time 1) and 20 minutes after the first injection (time 20). Also, we evaluated the occurrence of occasional adverse events during such procedure and we assessed the perceived difficulty by the doctor administering with such method. RESULTS 39 patients, aged 5-18 years, were recruited and assessed. All patients completed the study, which involved 468 grass pollen AIT SCIT doses, of which 234 with needleless device. At time 1, the use of the needleless device led to an 88% reduction of the pain perception average rate (from 16,8 to 1,97). The difficulty for the technician to make the inoculum was also analyzed (expressed by an increasing rate, from 0 to 4). In all 6 doses, for each child, the difficulty rate 2 relates to an almost constant number of patients, while the highest difficulty rates (grade 3 and grade 4) gradually decrease (until they disappear). CONCLUSIONS The vaccine administration with needleless device has various advantages and it ensures a better acceptance in comparison with administration by traditional SCIT. This is important in order to start the immunotherapy at an early stage, as it is recommended by the most recent studies, so that the natural evolution of the allergic disease can be immediately modified. The new technique ensures a higher safety both for the vaccinator and for the patient, with a better acceptance of the procedure by the patient and therefore a better and wider compliance to the vaccination schedules.
The review article discusses the current data on the mechanisms of hypersensitivity drugs reactions (HDRs). The relevance of the problem is associated with a significant prevalence of adverse drug reactions, in the structure of which immune allergic reactions occupy a significant proportion. Adverse drug reactions, according to the literature, occur in 10-20% of hospitalized patients and about 25% of outpatients. A serious problem of HDRs is their overdiagnosis, which affects the results of individual treatment and can have serious consequences in case of misdiagnosis, because it causes appropriate restrictions in the use of necessary drugs. Purpose - to analyze current views and recommendations on the problem of drug allergy; to highlight the issues of the 2022 meeting, which highlighted the problems of drug hypersensitivity. In particular, to show the relationship between viral infections and severe skin reactions, such as skin lesion syndrome with eosinophilia and systemic disorders. The article reviews and proposes algorithms that can be used to prevent the development of complications caused by HDRs. The materials of the article are based on the results of the European Meeting organized by the European Association of Allergists and Immunologists in 2022 in Verona, Italy. In the conclusions it is noted that despite the interest of the health care system and the large number of studies and publications on the problem of drug allergy, today there is an urgent need for accurate diagnostic approaches that will ensure the correct identification of the suspected drug by a combination of necessary tests and determine the cause for the possibility of prescribing appropriate treatment. The new mechanisms, phenotypes and endotypes of HDRs to drug responses discussed in the review are the future keys to preventing further exposure to harmful drugs. Preliminary pharmacogenomic screening (identification of specific human leukocyte antigen genotypes associated with severe skin adverse reactions) for patients of certain groups before prescribing high-risk drugs may become the future for Ukraine. EVALUATION OF 3-YEARS COURSE OF SUBLINGUAL IMMUNOTHERAPY WITH EXTRACTS OF CAT EPITHELIUM IN PRE-SCHOOL CHILDREN. Sharikadze O. The article presents the results of a 3-year course of sublingual allergen immunotherapy with a lyophilized extract of cat epidermal allergens in preschool children with allergic rhinitis, rhino-conjunctivitis and bronchial asthma. New possibilities of component diagnostics were found, and in particular, the definition of the major Fel d 1 molecule as a direct indication for the start of therapy and use to assess the achievement of tolerance to the causative allergen. It was found that a 3-year course of sublingual allergen immunotherapy with lyophilized epidermal cat allergens for children with allergic rhinitis and/or bronchial asthma was characterized by a significant improvement in the clinical symptoms of the disease and a decrease in the level of Fel d1. A comparative analysis of the results of observing children with SLIT and without therapy proved that SLIT in children with allergic pathology associated with sensitization to cat epidermal allergens reduces the number of exacerbations and prevents the development of symptoms of bronchial asthma in preschool children. In addition, once again, high safety allergen immunotherapy efficacy in children has been proven. Виявлено нові можливості компонентної діагностики і зокрема, визначення мажорній молекули Fel d 1 як прямого показання до початку терапії і використання для оцінки досягнення толерантності до причинному алергену. Встановлено, що проведення 3-х річного курсу сублингвальной алерген-імунотерапії ліофілізованими епідермальними алергенами кота дітям з алергічним ринітом та / або бронхіальною астмою характеризувалося достовірним поліпшенням клінічних симптомів захворювання і зменшенням рівня Fel d1. Порівняльний аналіз результатів спостереження дітей на тлі злити і без терапії довів, що злитися у дітей з алергічною патологією, пов'язаною з сенсибілізацією до епідермальних алергенів кота зменшує кількість загострень і попереджає розвиток симптомів бронхіальної астми у дітей дошкільного віку. Крім того, в черговий раз була доведена висока безпека алерген-імунотерапія ефективність у дітей. Обнаружены новые возможности компонентной диагностики и в частности, определения мажорной молекулы Fel d 1 как прямого показания к началу терапии и использования для оценки достижения толерантности к причинному аллергену. Кроме того, в очередной раз была доказана высокая безопасность аллерген-иммунотерапия эффективность у детей. Prevention is better than cure. Firstly, many regimes spend an enormous amount of money in order to treat their people who are suffering from different types of serious diseases. This article is an attempt to analyze of the existing international sources, which focuses on the primary prevention allergy. It is clear that the basis of such prevention is prophylaxes of the disease before it showed up. This is a very difficult question, because there are different points of view on the possibility of the prevention allergy today. The main purpose of this review is to summarize evidence base scientific data and researching results for the possibility of providing recommendations on primary prevention of childhood allergies. We would like to increase knowledge about the development of the allergic process, the ability to recognize and evaluate risk factors and all the factors of the next sensitization will enable us to better understand the tactics of prophylaxis allergic diseases.
Клінічний протокол медичної допомоги дітям із коронавірусною інфекцією (COVID 19), яка перебігає на фоні хронічних соматичних захворювань УДК 616.98 053.2 07 08:616.892 036 У рамках протоколу узагальнено клінічні рекомендації щодо діагностики і лікування дітей з коронавірусною інфекцією COVID 19, яка перебігає на тлі різних хронічних соматичних захворювань. В основу протоколу покладено матеріали багатьох іноземних рекомендаційних документів, а також останнього вітчизняного протоколу щодо виявлення терапії коронавірусної інфекції, яка викликана COVID 19 (наказ МОЗ України від 20.05.2020 № 1227). Відображено аспекти класифікації ступеня тяжкості і факторів ризику тяжкого перебігу хвороби, до яких належать і фонові хронічні соматичні захворювання, що потребує індивідуалізованого підходу до лікування. Охарактеризовано діагностичні критерії цієї інфекції, а також відмінності клінічної симптоматики COVID 19 у дітей від клінічних проявів у дорослих. Окремо наведено дані про ускладнення дитячої інфекції COVID 19-мультисистемний запальний синдром. Особливість протоколу-рекомендації щодо ведення дітей з інфекцією COVID 19, яка перебігає на фоні хвороб серцево судинної системи (вроджених вад серця, порушень серцевого ритму і провідності, легеневої гіпертензії тощо), захворювань органів дихання (бронхіальної астми і бронхолегеневої дисплазії), ревматичних (ювенільних артритів, системних хвороб сполучної тканини і системних васкулітів з ураженням легень, варіантів хвороб, які потребують постійної терапії глюкокортикоїдами або перебігають на тлі цукрового діабету) та інших аутоімунних захворювань (аутоімунного гепатиту, гломерулонефриту, особливо з хронічною недостатністю функції печінки або нирок). Також наведено рекомендації щодо ведення дітей, в яких COVID 19 розвинулася на фоні хвороб нирок і сечовидільної системи, ендокринних захворювань (цукрового діабету І типу, патології щитоподібної залози та наднирників), хвороб онкологічного і гематологічного генезу, у тому числі в стані імуносупресії.
