Резюме. Клінічна настанова є результатом узгодженого рішення експертів, прийнятого на підставі ретельного аналізу даних літератури, а також рекомендацій зарубіжного керівництва GINA: Global Strategy For Asthma Management And Prevention. Updated 2019. У документі представлено оновлене визначення бронхіальної астми (БА), дані нові підходи до класифікації та діагностиці захворювання, запропоновані сучасні схеми лікування, які засновані на результатах багатоцентрових міжнародних клінічних досліджень, що обґрунтовує високу доказовість такої терапевтичної тактики. Викладено нові принципи розподілу хворих на фенотипи, які слід враховувати при виборі схем лікування, представлені алгоритми початкової і підтримуючої терапії, нові підходи до лікування загострень захворювання. У першій частині публікації керівництва представлено оновлене визначення та патогенетичні механізми БА, викладені нові підходи до класифікації та діагностиці захворювання, дані принципи комплексної оцінки контролю БА з урахуванням контролю за симптомами астми, інтерпретації змін показників функції зовнішнього дихання і ризику загострень. Настанова адресована лікарям всіх ланок надання медичної допомоги -сімейним лікарям, терапевтам, пульмонологам, алергологам, кардіологам, реабілітологам.
C. Clinical guideline is the result of an agreed decision of experts, adopted on the basis of a thorough analysis of literature data, as well as recommendations of foreign guideline GINA: Global Strategy For Asthma Management And Prevention. Updated 2019. The document provides an updated definition of bronchial asthma (BA), gives new approaches to the classification and diagnosis of the disease, proposes modern treatment regimens based on the results of multicenter international clinical trials, which justifies the high evidence for such therapeutic tactics. New principles for the distribution of patients to phenotypes, which should be taken into account when choosing treatment regimens, are presented, algorithms for initial and maintenance therapy, and new approaches to the treatment of exacerbations of the disease are presented. In previous publications of the manual (Asthma and Allergy, 2020, № 2, № 3), an updated definition and pathogenetic mechanisms of BA were presented, new approaches to the classification and diagnosis of the disease were outlined, and the definition and assessment of asthma control were given. The principles of assessing the severity of asthma, as well as the difference between severe and uncontrolled asthma are considered. Standards for the treatment of asthma patients according symptoms control and minimize risk are presented. The basic principles of asthma management and alternative correction strategies of treatment are considered. The third part of the manual presents a stepwise approach to the correction of therapy, immunotherapy with allergens, vaccination, indications for referral to a specialist consultation. The guideline is addressed to doctors at all levels of medical care – family doctors, general practitioners, pulmonologists, allergists, cardiologists, rehabilitologists. оценить клиническую эффективность и безопасность применения формотерола (препарат Зафирон фармацевтическая компания «Адамед Фарма С.А.») и флутиказона пропионата (препарат Флутиксон, фармацевтическая компания «Адамед Фарма С.А.») с учетом клинической симптоматики БА и функциональных параметров сердца у пациентов с коморбидными БА и ИБС. Материалы и методы. В исследовании принимали участие 72 пациента: 34 мужчины и 38 женщин в возрасте от 39 до 72 лет: 13 человек (18,0 %) не принимали ИКС в течение минимум 3 месяца, 14 человек (19,4 %) принимали ИКС нерегулярно, 45 человек (62,5 %) принимали ИКС в монотерапии в связи с наличием противопоказаний со стороны сердечно-сосудистой системы. Все пациенты были разделены на две группы в зависимости от дозы ИКС: группа А, 25 человек, получавших низкую дозу ИКС-флутиказона пропионат 125 мкг 2 раза в сутки, группа В, 57 человек, которые получали высокую дозу ИКС-флутиказона пропионат 250 мкг 2 раза в сутки. Все пациенты получали формотерол по 12 мкг 2 раза в сутки и при необходимости (дополнительно не более 2 капсул в сутки). Длительность терапии составила 3 месяца. Для оценки состояния пациентов проводили: Астма контроль тест (Asthma Control Test-АСТ), оценку качества жизни по опроснику госпиталя Святого Георгия (SGRQ), спирометрия, оценку показателей электрокардиографии (ЭКГ), включая суточное мониторирование, допплерэхокардиографию. Результаты и их обсуждение. Через 3 месяца терапии у большинства больных БА отмечалось достоверное уменьшение ночных, утренних и дневных симптомы, уменьшилась потребность в скоропомощной терапии, улучшилось качество жизни, в обеих группах отмечено достоверное улучшение бронхиальной проходимости. Анализ данных ЭКГ у пациентов обеих групп не показал достоверного изменения ЧСС, интервалов QT и QTс. Результаты исследования показали отсутствие влияния формотерола на сократительную способность миокарда, инотропное функцию сердца, фракцию выброса левого желудочка у пациентов с коморбидность БА и ИБС. Выводы. Применение флутиказона пропионата (Флутиксон, «Адамед Фарма С.А.») в дозе 125 мкг / 250 мкг и формотерола (Зафирона «Адамед Фарма С.А.») 12 мкг 2 раза в сутки с эпизодическим применением формотерола по необходимости у пациентов с коморбидность БА и ИБС способствовало достоверному улучшению контроля за симптомами астмы, улучшению бронхиальной проходимости, повышению качества жизни и отсутствия развития проаритмогенное эффекта и негативного влияет на структурно-функциональные параметры правых и львах отделов сердца.
