Résumé -Introduction : L'adénocarcinome polymorphe de bas grade (APBG) est une tumeur maligne rare qui se trouve presque exclusivement située dans les glandes salivaires accessoires. Il survient fréquemment chez les patients entre 30 et 70 ans, avec un ratio femme-homme de 2/1. Il est généralement situé dans le palais dur ou mou. Observation : Il s'agissait d'une patiente de 38 ans, consultant suite à l'apparition d'une tuméfaction bleutée au niveau du palais mou droit, évoluant depuis un an, indolore et recouverte d'une muqueuse d'aspect normal. Le diagnostic évoqué était un adénome pléomorphe mais l'examen histologique après exérèse chirurgicale révélait un APBG. Une reprise des marges d'exérèse était nécessaire associée à une plastie. Discussion : Le diagnostic de l'APBG est basé principalement sur les éléments cliniques et radiologiques. L'anatomopathologie permet la confirmation et la classification de la lésion. Le traitement reste essentiellement chirurgical. Le pronostic est bon malgré un taux de récidive de l'ordre de 17 % à 24 %. Bien que rares, les métastases dans les ganglions lymphatiques régionaux peuvent se produire dans 9 % des cas. Conclusion : L'APBG est une tumeur non agressive dont le pronostic dépend de plusieurs facteurs dont le délai de consultation, d'où l'importance d'un dépistage précoce.Abstract -Polymorphous low-grade adenocarcinoma of the soft palate: review and case report. Introduction: Polymorphous low-grade adenocarcinoma (PLGA) is a rare malignant tumor that is found almost exclusively in the minor salivary glands. It frequently occurs in patients between 30 and 70 years old, with a female to male ratio of 2/1. It is usually located in the hard or soft palate. Observation: A 38-year-old female patient presented with a bluish swelling in the right side of the soft palate which had evolved for a year, which was painless and covered with a mucosa of normal appearance. The first diagnosis suggested was pleomorphic adenoma, but the histologic exam following the surgical biopsy revealed a PLGA. Surgical revision of the safety margins was necessary together with mucosal plasty. Discussion: The diagnosis of PLGA is based mainly on clinical and radiological aspects, but a histologic exam is absolutely necessary to confirm it, and for classification of the lesion. Treatment is mainly surgical. The prognosis is good, although the recurrence rate is 17% to 24%. Although seldom, metastases in regional lymph nodes may occur in 9% of cases. Conclusion: PLGA is a non-aggressive tumor whose prognosis depends on several factors including the consultation period. This highlights the importance of early detection.
Résumé -Le kyste osseux solitaire, plus communément nommé kyste osseux traumatique, constitue une affection des maxillaires peu connue. Ces multiples dénominations illustrent les propositions théoriques pour expliquer l'étiopathogénie du kyste solitaire. Ne comportant aucune membrane, il est classé parmi les pseudo-kystes intraosseux. Il survient le plus souvent lors de la 2 ème décennie, et atteint de façon préférentielle l'os mandibulaire. Il est généralement asymptomatique et de découverte fortuite lors d'un examen radiologique. L'exploration chirurgicale confirme le diagnostic par la découverte d'une cavité vide ou remplie d'un liquide séro-hématique. Ce travail a pour objectif d'illustrer, à travers un cas clinique, la démarche diagnostique et thérapeutique du kyste osseux solitaire.Abstract -The solitary bone cyst : a case report. The solitary bone cyst, more commonly known as traumatic bone cyst, is a little known pathology of the jaws. These many names illustrate the theoretical propositions to explain the pathogenesis of solitary cyst. Does not include any membran, it is classified as intraosseous pseudocysts. It occurs most often during the second decade, and reached preferentially the mandibular bone. It is usually asymptomatic and discovered incidentally during a radiological examination. Surgical exploration confirmed the diagnosis by the presence of a cavity empty or filled with a sero-hematic liquid. This work aims to illustrate, through a case study, the diagnostic and therapeutic approach of solitary bone cyst.C'est Dupuytren en 1832 qui rédige le premier article relatif aux kystes solitaires des os longs. La première localisation maxillaire a été rapportée en 1929 par Lucas [in 14].L'utilisation courante de la radiographie panoramique et la prise en considération de l'existence de ces lésions ont contribué à une détection plus fréquente. Dans la plupart des études, le kyste solitaire survient surtout au cours de la deuxième décennie. Les hommes semblent plus touchés que les femmes. Le traumatisme est retenu comme cause principale [2]. Les kystes solitaires peuvent intéresser tout le squelette. Dans la sphère cranio-faciale, la mandibule représente le siège le plus fréquent [17].
Résumé -L'améloblastome est une tumeur odontogène bénigne développée à partir des débris épithéliaux de Malassez après régression de l'organe de l'émail. Il représente 30% des tumeurs bénignes de la mandibule. Cette tumeur est dite « bénigne à malignité locale » à cause de son potentiel évolutif important et de sa tendance aux récidives après traitement. Le médecin dentiste représente un maillon incontournable dans la prise en charge de ces patients. En effet, il peut intervenir à différents niveaux : d'abord pour le diagnostic de la lésion qui peut être fait de façon précoce, mais aussi pour le traitement des tumeurs de petite taille, sans omettre son rôle primordial dans la réhabilitation prothétique. La présentation de 3 cas traités récemment au Centre de consultations et de traitements dentaires de la Faculté de Médecine dentaire de Casablanca ou traités en collaboration avec le Service de Chirurgie maxillo-faciale du CHU Ibn Rochd de Casablanca, permet de préciser le rôle du médecin dentiste.Abstract -Ameloblatoma : role of the dentist in the management. The ameloblastoma is a benign odontogenic tumor. It represents 30% of benign tumors of the mandible. This tumor is "local malignant" because of its potential for expansion, and its tendency to relapse after treatment. The dentist is an essential link in the management of these patients. He acts on different levels: first for the diagnosis of the lesion wich can be done early, but also for the treatment of tumors of small size, and his role in the prosthetic rehabilitation of these patients. We report through this article the roles of the dentist and a series of clinical cases treated at the Center of consultation and dental treatment of the Faculty of Dentistry at Casablanca, or treated in collaboration with the Service
