ТОДОСЬЕВ КЛАССИФИКАЦИОННЫЕ СТРАТЕГИЧЕСКИЕ ПОДХОДЫ В ЛЕЧЕНИИ ВРОЖДЕННЫХ СОСУДИСТЫХ МАЛЬФОРМАЦИЙ:ВЗГЛЯД СОСУДИСТОГО ХИРУРГА ГУ «Национальный институт хирургии и трансплантологии им. А.А. Шалимова НАМНУ», г. Киев, УкраинаСтатья посвящена вопросам терминологии, классификации разных форм врожденных сосудистых мальформаций (ВСМ) или ангиодисплазий, рассматриваются основные принципы диагностики и лечения.Целью данной работы является усовершенствование классификации врожденных сосудистых маль-формаций и создание диагностических и лечебных алгоритмов, что позволит оптимизировать тактику и улучшить результаты лечения больных. Подробно анализируются достоинства и недостатки основных предшествующих отечественных и зарубежных классификаций, отмечается необходимость принятия уни-версальной терминологии, классификации с целью обеспечения унифицированной диагностической и ле-чебной тактики.На основе многолетнего клинического опыта авторы предлагают для обсуждения собственную классификацию врожденных сосудистых аномалий "vasc+t" и лечебно-диагностические алгоритмы для наиболее распространенных форм ВСМ. Предложенная классификация включает разделы, позволяющие определить форму ВСМ, распространенность заболевания, наличие осложнений с указанием показаний к оперативному лечению: I. cосудистый дефект ВСМ; II. анатомический дефект ВСМ; III. локализация де-фекта; IV. осложнения, в том числе являющиеся абсолютными и относительными показаниями к лечению; V врожденные сосудистые опухоли. Учтена возможность дифференциальной диагностики с врожденными сосудистыми опухолями, что позволяет избежать фатальных ошибок на этапе диагностики. The aim of this paper is to improve the classification of congenital vascular malformations and the establishment of diagnostic and therapeutic algorithms to optimize tactics and treatment outcomes.The advantages and disadvantages of basic preceding domestic and foreign classifications are analysed in details, the necessity of acceptance of uniform terminology, classification with the purpose of providing of universal diagnostic and treatment tactics is marked. On the basis of long-term clinical experience the authors offer their own classification of congenital vascular abnormalities "vasc+t" and algorithms of diagnostics and treatment of the most widespread forms of CVM.The offered classification includes points, allowing to define the form of CVM, prevalence of disease, presence of complications with indications to operative treatment: I. Vascular defect of CVM; II. Anatomic defect of CVM; III. Localisation of defect; IV. Complications being absolute and relative indications to treatment; ...
Objective. The structure of combined osseous–vascular work injury of the lower extremities was studied, and the results of its operative treatment analyzed retrospectively. Materials and methods. The data concerning 147 patients with combined osseous–vascular work trauma of the lower extremities were analyzed. All the patients managed were from 19 to 48 yrs old, and their average age constituted 30.2 yrs old. The mine trauma with injuries of femoral and/or the shin bones and a. poplitea prevailed – in 129 (87.8%) and 116 (78.9%) observations, accordingly. There were performed 92 two–staged interventions in a common operative team with traumatologists. Results. Positive result of operative treatment was noted in 73 (79.3%) patients. The time span between the trauma moment and the operation beginning in this group of the patients have constituted from 2 to 5 h, while the degree of an acute arterial ischemia in accordance to V. S. Savelyev (1973) were signed as IIa–IIb. The operation duration have constituted from 3 to 4 h. All the patients have had a combined shock Degree II. The preoperative blood loss volume has been ranged from 1 to 2.5 l. The a. poplitea injury have had prevailed and occurred in 43 (58.9%) patients. In 6 (6.5%) patients after reconstructive intervention the extremity amputation was performed. Conclusion. Mostly favorable result was obtained in the injured persons, who were delivered early (upto 2 h) to specialized stationaries with conduction of preoperative antishock therapy.
Purpose of the study. Improvement of the results of treatment of patients with arteriovenous forms (AVF) of congenital vascular malformations (CVM) of the extremities based on the development of the classification scheme of CVM and the study of proliferative activity of the pathology.Materials and methods. The clinical data of 155 patients with AVF of CVM of extremities were analyzed in terms of observation from 1 month to 10 years. Patients of sex of men there were 65 (42%), women – 90 (58%), their correlation – 1 : 1,4; the average age was about 25,1 ± 10,4, children's age group – 53 (34%). The division into clinical-anatomical forms (11 groups) was carried out on the basis of the «working» classification scheme «VASC + T». Expression proliferation markers VEGF and KI-67 woos study were performed by immunoperoxidase method with additional hematoxylin staining.Results and discussion. Pathomorphological (66; 45,8%) and immunohistochemical studies (10; 7%) revealed the presence of proliferative activity of angiomatous tissues and degenerative changes in the walls of the vessels both due to developmental defects and disorders of hemodynamics, while the source of progression of the AVF of CVM there are vessels of the microvasculature.Conclusions. The application of the CVM classification scheme and the step-by-step complex treatment approach, depending on the clinical and anatomical AVF of CVM, led to the absence of major postoperative complications. This tactic made it possible to achieve satisfactory long-term results in 136 (94,4%) cases. Мета. Поліпшення результатів лікування хворих з артеріовенозними формами вроджених судинних мальформацій (АВФ ВСМ) кінцівок на основі розроблення диференційованих алгоритмів діагностики та лікування. Матеріали і методи. Робота базується на даних комплексного клінічного дослідження та результатах хірургічного лікування 155 пацієнтів з АВФ ВСМ кінцівок. Строки спостереження становили від 1 міс до 10 років. Чоловіків було 65 (42%), жінок - 90 (58%), число чоловіків і число жінок співвідносилися як 1: 1,4. Середній вік пацієнтів становив (25,1 ± 10,4) року. У віці до 18 років (дитяча вікова група) було 53 (34%) хворих. Клінічний матеріал аналізували в залежності від клініко-анатомічних форм захворювання, які були розподілені на 11 груп на основі робочої класифікаційної схеми «VASC + Т». Результати. Розроблення класифікаційної схеми вроджених судинних мальформацій уможливило поліпшення формування плану діагностики та етапного лікування пацієнтів у вигляді алгоритмів у залежності від клініко-анатомічної форми захворювання. Застосування етапного комплексного підходу з використанням емболізаційних, ендовенозних та хірургічних методик дало змогу уникнути розвитку великих післяопераційних ускладнень. Висновки. Впровадженням розробленої тактики досягнутo задовільних віддалених результатів лікування у 136 (94,4%) пацієнтів.
