The problem of timely diagnosis and treatment of orphan diseases is today one of the leading in many countries of the world. The article considers the epidemiology of acromegaly, a rare disease that, in the absence of timely medical care, significantly reduces the life expectancy of patients. The review presents global data on the prevalence of disease and the establishment of registers of such patients and suggest ways to address the issue of timely diagnosis of acromegaly in Ukraine.

Харьков zenrost@mail.ruБолезнь Иценко-Кушинга (БИК) тяжелое многосимптомное заболевание гипоталамо-гипофизарного генеза, обуслов-ленное наличием опухоли гипофиза (корти-котропиномы) или гиперплазией аденома-тозных клеток, секретирующих повышенное количество адренокортикотропного гормо-на (АКТГ), и, как следствие, увеличением продукции гормонов корой надпочечнико-вых желез [1].В 1924 году советский невропатолог Н. М. Иценко описал двух больных с гиперт-рихозом, высыпаниями и полосами растяже-ния на коже, резкой слабостью, диспласти-ческим ожирением. У этих больных при па-тоанатомическом исследовании были обна-ружены дегенеративно-дистрофические из-менения в гипоталамусе, в области серого бугра, супраоптических и паравентрикуляр-ных ядер, а в гипофизе увеличенное ко-личество хромофобных клеток. В 1932 году подобное заболевание было описано нейро-хирургом Х. В. Кушингом, который в ги-пофизе больных обнаружил небольшие аде-номы из базофильных клеток и предполо-жил, что именно это было причиной ха-рактерной клинической картины заболева-ния. В дальнейшем заболевание было раз-делено на две формы: болезнь Иценко-Ку-шинга диэнцефально-гипофизарно-надпо-чечениковая патология и синдром Ицен-ко-Кушинга, в который вошли аденоматоз, аденома, карцинома коры надпочечников, и эктопированный АКТГ-синдром при раз-личных опухолях, секретирующих АКТГ или АКТГ-подобную субстанцию, вызываю-щую гиперстимуляцию коры надпочечени-ков с развитием гиперкортицизма.Болезнь Иценко-Кушинга самая частая причина эндогенного синдрома ги-перкортицизма (68 %) [2]. Данная патология относится к классу редких заболеваний, возникает в молодом возрасте (20-40 лет), соотношение женщин и мужчин состав-ляет 3,5/1,0 [3]. Основными причинами преждевременной смерти больных с БИК являются кардиоваскулярные (29 %), це-реброваскулярные (11 %) и инфекционные (12 %) осложнения [4]. Показано, что предик-торами преждевременной смерти больных с БИК выступают длительность экспозиции высокого уровня кортизола в крови, воз-раст на момент диагностики заболевания и уровень АКТГ перед хирургическим лечением [5].При БИК в нейронах паравентрикуляр-ных и супраоптических ядер гипоталаму-са нередко обнаруживаются дегенератив-ные изменения; в аденогипофизе уве-личение количества базофильных клеток и повышения их функцинальной активно-сти. В статье представлены данные исследований, подтверждающие важность обучения самоконтролю для достижения и поддержания долговременной компенсации у больных сахарным диабетом. Приведены анализ литературы и собственные данные о работе школы самоконтроля на базе клиники ГУ «Институт проблем эндокринной патологии им. В.Я. Данилевского НАМН Украины» (г. Харьков). Оценена эффективность и определены наиболее важные проблемные аспекты системы обучения. Предлагается применение структурированной программы, включающей основные вопросы самоконтроля; проведение занятий в виде диалога с обсуждением ситуационных задач и активным участием слушателей в процессе рассмотрения проблемных вопросов. Отмечено, что пациенты, которые ранее прошли обучение в школах самоконтроля, более привержены повторному обучению и лечению. Для больных с впервые выявленным сахарным диабетом целесообразно посещение школы самоконтроля вместе с близкими.
