Background: Infections with Gram-positive bacteria, including acute bacterial skin and skin structure infections (ABSSSIs), are common in children. We describe a single-dose pharmacokinetics and safety study of tedizolid phosphate, a new oxazolidinone under investigation for the treatment of ABSSSIs in children, in hospitalized participants 2 to <12 years of age.Methods: This open-label, multicenter, phase 1 trial (NCT02750761) enrolled hospitalized children 2 to <12 years of age receiving treatment for a confirmed/suspected Gram-positive bacterial infection. Participants were stratified by age (2 to <6 years and 6 to <12 years) to receive a single oral or intravenous dose of tedizolid phosphate. Evaluations included safety and pharmacokinetics of tedizolid phosphate and its active metabolite, tedizolid. Palatability of the oral suspension was also evaluated. Results: Thirty-two participants were enrolled and received 3-6 mg/kg of study medication. For both routes of administration, tedizolid phosphate was rapidly converted to tedizolid; median time to maximum tedizolid plasma concentration was 1-2 hours after initiation of the 1-hour intravenous infusion and 2-3 hours after oral dosing. The tedizolid mean terminal half-life was 5-6 hours and 6-7 hours for the intravenous and oral administration groups, respectively. The oral tedizolid phosphate suspension demonstrated high bioavailability comparable to that of the parenteral administration. A single dose of intravenous or oral tedizolid phosphate was well tolerated; no unexpected safety findings were observed.Conclusions: Pharmacokinetic and safety observations provide the information necessary for the continued development of tedizolid phosphate for the treatment of Gram-positive infections in children, particularly ABSSSIs.
Adhesive intestinal obstruction is one of the most urgent problems in pediatric abdominal surgery due to its high rate resulting in a lot of complications, recurrences and relaparotomies.The objective of the research was to examine the clinical efficacy of using intraoperative antiadhesive drugs in children to prevent recurrent adhesive intestinal obstruction.Materials and methods. The examination and analysis of medical records of 86 children with adhesive intestinal obstruction treated in the clinic of pediatric surgery of the Ivano-Frankivsk National Medical University over the past 5 years was carried out. We identified two groups of patients: the comparison group included 40 children who received traditional treatment; the main group included 14 children who underwent the application of antiadhesive gel intraoperatively. The assessment of treatment effectiveness was carried out on the base of the postoperative course (recovery time of intestinal peristalsis, onset of enteral feeding, duration of hospital stay, incidence of early postoperative complications) and long-term outcomes (severity of adhesive disease symptoms, presence of recurrent adhesive intestinal obstruction and relaparotomies).Results and discussion. In comparative evaluation of the postoperative course in two groups of patients we noticed a more favorable course in children of the main group that was shown by faster recovery of peristalsis, reduced duration of gastric stasis, quicker occurrence of self-defecation and shortened term of hospital treatment. In the comparison group recurrent adhesive intestinal obstruction occurred in 8 (20.0%) patients, in 6 (15.0%) cases relaparotomy was performed. In the main group of patients, recurrent adhesive intestinal obstruction was observed in one (7.1%) child; however, relaparotomy was not performed.Conclusions. The intraoperative application of antiadhesive gel is a highly effective and safe way to prevent the recurrence of adhesive intestinal obstruction in children.
ВступМеконієвий перитоніт (МП) розвивається у плода чи у новонародженої дитини у перші дні життя при потраплянні меконію у черевну порож-нину. Вперше МП як окремий синдром описав у 1761 р. Morgagni. Частота МП в середньому скла-дає 1 : 30000 новонароджених. За даними бага-тьох авторів, у 46-50 % новонароджених МП по-єднується з вродженою кишковою непрохідністю. За останнє десятиріччя частота МП збільшується, що пов'язано з покращенням антенатальної діа-гностики та лікування новонароджених в перші дні життя [1, 2, 3].Причиною МП є перфорації травного тракту. В 50 % випадків це зумовлено перфораціями тонкої кишки. Причини внутрішньоутробних перфора-цій досить різноманітні: атрезії і стенози кишок, хвороба Гіршпрунга, меконієвий ілеус, внутрішні грижі, заворот кишечника, внутрішньоутробна ін-вагінація, гіпоксія плода, внутрішньоутробні ві-русні інфекції (особливо -цитомегаловірусна та парвовірусна інфекція). Найбільш частою причи-ною МП більшість авторів вважають меконієвий ілеус (15-40 %). Атрезії кишок у 25 % випадків супроводжуються судинними порушеннями киш-кової стінки, що й зумовлює перфорацію. Деякі автори вказують на ідіопатичну перфорацію ки-шечника, як причину МП [1, 4, 5, 6].Розрізняють 3 форми МП: 1) кистозна (з утво-ренням псевдокист); 2) дифузна; 3) фіброадге-зивна. Найбільш часто зустрічається фіброадге-зивна форма МП. При цьому утворюються щільні фіброзні зрощення між петлями кишок та паріє-тальною очеревиною, що зумовлено стерильним «хімічним» запаленням очеревини. У багатьох випадках під час хірургічного втручання перфо-рацію кишки не виявляють, оскільки вона може бути прикрита зрощеннями чи фіброзними наша-руваннями. При кистозній формі МП фіброплас-тичний процес виражений менше. Утворюється псевдокиста, стінками якої є частково некроти-зовані петлі кишок, кальцифікована оболонка, заповнена вона рідким меконієм. При дифузній формі меконій та фібринозні нашарування розпо-всюджені по всій очеревині, фіброзні зрощення не виражені. Остання форма МП спостерігається при виникненні перфорації кишки незадовго перед народженням дитини. У випадках, коли перфо-рація настала до облітерації вагінального відрос-тка очеревини, спостерігають також кальцифікати мошонки у хлопчиків [1, 3, 4, 7, 8].Антенатальна діагностика МП базується на ультразвуковому дослідженні плода.
