Introduction : Les schwannomes sont des tumeurs bénignes neurogènes localisées dans la région sacrée dans 1 à 5 % des cas. Les auteurs rapportent un cas de neurinome présacré. Observation : Une patiente de 55 ans a consulté pour des douleurs pelviennes évoluant depuis un an sans signes de compression digestive ou urinaire. Les touchers pelviens avaient objectivé une masse rétrorectale avec une muqueuse rectale saine. L'imagerie avait évoqué le diagnostic de neurinome. L'exploration chirurgicale avait permis d'objectiver une masse tissulaire rétrosacrée qui a bénéficié d'une exérèse en monobloc. L'histologie a conclu à un neurinome bénin. Discussion : Les schwannomes présacrés géants sont des tumeurs rares. Du fait de leur symptomatologie pauvre, leur diagnostic est souvent retardé. L'imagerie, TDM mais surtout IRM, est intéressante dans le diagnostic des neurinomes ainsi que leurs rapports avec les organes de voisinage ; elle permet de déceler également des signes d'agressivité de la tumeur mais ne peut affirmer de façon formelle la nature bénigne ou maligne de la tumeur. Le traitement repose sur l'exérèse ou l'énucléation de la tumeur. Le pronostic est bon, mais entaché par la survenue de récidives. Conclusion : Devant des douleurs pelviennes, le diagnostic de neurinome présacré est rarement posé. L'imagerie (IRM) permet le diagnostic et guide la stratégie thérapeutique. We report a case diagnosed by imaging modalities (CT-MRI) and treated by complete surgical resection. Case report: A 55-year-old female has had a history of abdominal pain since one year without compressive signs. A pelvic retrorectal mass has been discovered clinically. CT and MRI were very suggestive of a presacral schwannoma. Histological examination after complete surgical removal of the mass confirmed the diagnosis of a benign schwannoma. Discussion: Giant presacral schwannomas are rare pelvic tumors. Often asymptomatic, they are diagnosed lately. CT and MRI are very useful in assessing the diagnosis and evaluating the extent of the tumor. Surgery is the therapeutic modality of first choice. Prognosis is good if the tumor is benign but recurrence is frequent. Conclusion: Presacral schwannoma can cause pelvic pain. Imaging modalities (MRI++) often guide surgical resection.
Duodenal tuberculosis is a rare clinical entity. The authors report and emphasize the lack of special clinical, radiological and endoscopic signs of duodenal tuberculosis. The diagnosis is affirmed, at laparotomy, out of the findings of peritoneal granulations or histology of lymphatic nodes. We report our experience of two cases of duodenal tuberculosis presenting with proximal intestinal obstruction and review the available literature.
La tumeur pseudopapillaire et solide du pancréas (TPPSP) est une tumeur rare. Elle touche le plus souvent la femme jeune. Décrite la première fois par Frantz en 1959, sa pathogénie demeure peu claire. C'est une tumeur de bon pronostic qui nécessite une chirurgie radicale. Les auteurs rapportent un cas de TPPSP chez un homme de 20 ans. La symptomatologie était aiguë, faite de syndrome de compression et d'épigastralgies. L'examen trouvait une masse solide de l'hypochondre gauche. Le scanner montrait une masse de structure mixte de la queue du pancréas. Une spléno-pancréatectomie gauche était réalisée. Le diagnostic était confirmé par l'examen histologique avec immunohistochimie. Le suivi à long terme ne montrait pas de récidive. Le recul est de deux ans et demi. À travers cette observation et une revue de la littérature, les auteurs discutent la contribution de la radiologie dans le diagnostic et insistent sur une chirurgie radicale dans le traitement de ces tumeurs de faible degré de malignité.Mots clés Tumeur pseudopapillaire · Pancréas · Homme · Tumeur kystique · Compression · Scanner · Chirurgie Abstract Solid pseudopapillary neoplasm (SPT) of the pancreas is a rare exocrine tumor. Frantz described the first case in 1959. Despite the increasing recognition of the tumor in this last year, its pathogenesis remains unclear. It occurs predominantly in young woman and has a good prognosis. That's why the surgery must be radical. The authors report a case of 20 years old man with abdominal acute pain and compression sign. The abdominal palpation finds a solid mass in left quadrant. The CT scan showed a cystic and solid mass in pancreas. Spleen pancreatectomy was performed; the follow-up is two years and a half. The diagnosis was confirmed by histological and immunohistochemistry examination. With data review, the authors discuss the contribution of the radiology in diagnosis and insist on the radical surgery in the treatment of these tumors with low malignancy degree.
L'angiosarcome primitif de la rate est une tumeur maligne rare avec un pronostic fâcheux. Les hémorragies digestives constituent un mode de révélation inhabituelle. On rapporte le cas d'un patient de 71 ans présentant un angiosarcome primitif de la rate révélé par des hématé-mèses. Le diagnostic était fait en préopératoire grâce à l'imagerie et à la biopsie. La chirurgie et la chimiothérapie constituent les principaux traitements. Avec une revue de la littérature, quelques concepts cliniques radiologiques et thérapeutiques de ces rares tumeurs seront discutés. Surgery and chemotherapy are the principal treatments. The literature on primary splenic angiosarcoma is reviewed. Clinical, radiological and therapeutic aspects of this rare tumor are discussed.
Le liposarcome est une tumeur maligne des tissus mous dont la localisation intra-abdominale est rare. Nous rapportons l'observation d'un patient de 60 ans ayant présenté un liposarcome multiple intéressant le mésentère et le mésocôlon. Le scanner préopératoire était en faveur de la nature adipeuse des masses. La biopsie scanoguidée préopératoire avait confirmé la composante adipeuse sans affirmer la malignité. La laparotomie a trouvé des tumeurs multiples du mésentère et du mésocôlon. Une résection des masses était réalisée. L'examen histopathologique confirmait qu'il s'agissait d'un liposarcome bien différencié. La chimiothérapie adjuvante était donc prescrite. L'évolution était favorable avec un recul de 18 mois. À la lumière de cette observation, nous voudrions insister sur la chirurgie qui représente le traitement curatif de choix pour les liposarcomes. Toutefois, la chimiothérapie semble prometteuse. À travers la littérature, certains problèmes diagnostiques et thérapeutiques liés à ce type de néoplasme méritent d'être signalés.Mots clés Liposarcome · Mésentère · Mésocôlon · Tomodensitométrie · Chirurgie · Chimiothérapie Abstract Liposarcoma represent the single most common type of soft tissue sarcoma. Its abdominal localization is rare, occurring in only 5% of cases. A 60-year-old male was found to have a case of primary multiple liposarcoma of the mesentery and mesocolon. Computed tomography revealed the presence of a peritoneal mass presenting a homogeneous density with an adipose component. Preoperative biopsies were falsely reassuring. A laparotomy confirmed the presence of multiple tumors in the mesentery and the mesocolon. The masses were removed and the histopathological report noted a well-differentiated liposarcoma. Chemotherapy was prescribed. The patient was well at 18 months follow-up. Surgery currently represents the best curative therapy for this type of tumor. Chemotherapy seems promising in liposarcoma. Diagnostic and therapeutic problems related to this type of neoplasm as well as a literature reviews are reported.
