Background: Chronic diseases are well established as the predominant death cause, and obesity, being one of the factors strongly contributive to chronic diseases, has been consistently threatening the global health. Aim of the Work: To compare between laparoscopic mini gastric bypass and laparoscopic sleeve gastrectomy on morbid obese patients in Egypt as regard weight loss, outcome of associated co-morbiditis and with 1 year follow up after operation. Patients and Methods: This study included 200 patients, 100 patients in each group. The group age ranged between 22-55 years with a mean ± SD of 37.88 ± 9.52 years. Female patients represented the main population of this study (60.5%). It is a common finding in the literature that women are undergoing bariatric surgery more than men. Background Undescended testis or cryptorchidism is one of the most common congenital abnormalities of the genitourinary system in young boys, approximately 1-2% of boys at the age of 1 year have undescended testis, the disorder being unilateral in about 90% of cases and bilateral in about 10%. Aim of the Work We conducted this prospective study to assess the efficacy and safety of single-incision, transscrotal orchidopexy in children with palpable UDT. Patients and Methods A prospective study was adopted to fulfill the purpose of the study. The study was conducted at Pediatric Surgery Department, Ain Shams University Hospitals in Cairo. The included study population was pediatric patients with palpable, inguinal mal-descended testes who attend to Pediatric Surgery Outpatient Clinic, Ain Shams University Hospital till the fulfillment of the sample size. Results The most common sites of undescended testis were intracanalicular and scrotal neck regions. The most common postoperative complication was scrotal edema. Mean operating time was 21 minutes. There was no significant difference in the testicular size pre and post operative. Conclusion Based on these results, we concluded that the undescended testis represents a common pediatric problem requiring surgical intervention. Trans-scrotal orchidopexy is an effective, less invasive and highly acceptable cosmetically approach for the treatment of palpable undescended testis.
Background the goal of modern breast surgery is to cure patients with preservation of breast tissue as much as possible with satisfactory physical and psychological outcome. This goal has led to introduction of a new scope of surgery into our clinical practice called oncoplastic breast surgery, with the advantage of combining oncological safety and satisfactory cosmetic outcome. Objective it was to focus on the infero-medial pedicle technique as an oncoplastic procedure for management of breast cancer and to assess the technique clinically regarding oncological safety, surgical outcomes and patient satisfaction. Patients and Methods a prospective analytical study included 15 patients, which was conducted at Ain-Shams University Hospitals. An approval of the Ethical Committee and written informed consent from all participants were obtained. Septum based infero-medial pedicle technique was proposed for patients in whom oncoplastic surgery were possible on oncologic grounds for upper outer quadrant of breast. Results the mean age of our study was 39 years, 50% of the cases fall between 38 to 57 years. In our study only 3 cases (33%) have had complications, 2 cases got wound infection; they were diabetics reflecting the immune compromisation with diabetes mellitus. We reported lower results of surgical site infection (15%). The operative duration of our study was long at first (240 minutes) but with progression of our study and as we build up more experience the operating time was reduced to (120 minutes). Conclusion the choice of the oncoplastic technique is mainly based upon the location of the tumor, size of the breast and distance of the tumor from the nipple areola complex.
