Background: Mandibular fractures with sagittal splitting and splaying of both the lingual and buccal cortices poses a challenge in their fixation as screws may push the lingual cortex during screw tightening resulting in widening of the mandible. Methods: Screw lagging inside the lower mandibular plate technique may help approximation of the near and far cortices during screw tightening, this was applied for eight cases with mandibular fractures. Results: The splaying between the two cortices was prevented regaining its normal medullary space width. Conclusion: Using the screw lagging inside the plate technique may be a safe technique used in mandibular fractures that have cortical splaying.
Introduction: Gynecomastia is a common condition encountered nowadays. Many surgical treatment options are available yet the best modality is still controversial. ND: YAG laser assisted and ultrasound (VASER) assisted liposuction are well defined and respected tools for liposuction yet no one proved to be superior over the other. This study's proposal is to compare the post-operative results for patients treated with both modalities concerning the complications and patient satisfaction.Patients and Methods: Patients with gynecomastia (grade 2) were randomly divided to ND: YAG laser liposuction (group 1) and ultrasound assisted (VASER) liposuction (group 2), where liposuction zones were marked in all patients. Complications and satisfaction scores were recorded and statistically analyzed.Results: Twenty patients were in this study; 10 in each group. The median satisfaction score for laser assisted group was 4.5 while for ultrasound group was 3.4. Conclusion:Both ND: YAG laser and VASER are powerful tools in management of gynecomastia (grade 2) with more satisfaction and less complications in the laser group.
Background: Vertical scar mammaplasty is a surgical approach that successfully unites both theories of diminished scar formation and an aesthetically appealing breast. However, unfavorable results may follow, as decreased upper pole fullness. Many techniques were proposed to overcome this complication.Aim: Proposal of usage of long lateral pillar flap to enhance the aesthetics of the breast. Patients and Methods:This study is a prospective clinical observational study, that was performed at Ain-Shams University Hospitals from December 2018 till June 2020, with an average follow-up time of one year. Eighteen (18) female patients with ptotic breast had reduction mammaplasty by superomedial pedicle technique with usage of long lateral pillar flap to reinforce suspension of superomedial pedicle. Standard photographs were obtained pre and post operative. All participant women had a routine pre and postoperative mammography and Strasser's cosmetic satisfaction score were done. Results:The age of the 18 women included in this study, ranged from (25-50 years), with an average of (34±4.723). Long-term results were satisfactory both objectively and subjectively, with good upper pole fullness with enhancement of the lateral breast crease. Conclusion:The use of lateral pillar flap adds on satisfaction in the breast's final outcome by enhancing the lateral rounded contour and by addition of upper pole fullness.
Background the goal of modern breast surgery is to cure patients with preservation of breast tissue as much as possible with satisfactory physical and psychological outcome. This goal has led to introduction of a new scope of surgery into our clinical practice called oncoplastic breast surgery, with the advantage of combining oncological safety and satisfactory cosmetic outcome. Objective it was to focus on the infero-medial pedicle technique as an oncoplastic procedure for management of breast cancer and to assess the technique clinically regarding oncological safety, surgical outcomes and patient satisfaction. Patients and Methods a prospective analytical study included 15 patients, which was conducted at Ain-Shams University Hospitals. An approval of the Ethical Committee and written informed consent from all participants were obtained. Septum based infero-medial pedicle technique was proposed for patients in whom oncoplastic surgery were possible on oncologic grounds for upper outer quadrant of breast. Results the mean age of our study was 39 years, 50% of the cases fall between 38 to 57 years. In our study only 3 cases (33%) have had complications, 2 cases got wound infection; they were diabetics reflecting the immune compromisation with diabetes mellitus. We reported lower results of surgical site infection (15%). The operative duration of our study was long at first (240 minutes) but with progression of our study and as we build up more experience the operating time was reduced to (120 minutes). Conclusion the choice of the oncoplastic technique is mainly based upon the location of the tumor, size of the breast and distance of the tumor from the nipple areola complex.
