Conservative treatment of spinal disorders, especially scoliosis has been accomplished with plastic or plastic-metal corsets for decades. However, corsets are tailored to the individual's body, the exact distances and action between the patient and the corset are not known. The raw piece is fabricated on the body's exact model, which is made in at least three steps. First, the upper body of a patient set in a predefined posture is plastered, then after drying the plaster is removed. The second step is the preparation of the torso's model that is made of plastic foam covered with plaster. After that the corset is formed according to the desired objectives, namely the orthopaedic technician forms the corset on the torso model and makes the necessary adjustments to achieve the desired spine compensation. The corset is then fitted on the patient and eventually minor corrections are made. New methods have been developed to replace plastering, namely 3D body scanners working with different measurement principles. Traditional corset forming has also been changed to utilize the 3D body model and to create a virtual corset, which can be sent directly to the manufacturing machine. As a last step it might also be necessary to make minor adjustments. As it is said before, there is no measureable data on the interaction of the patient's body and the corset, not even in a static standing position, not to mention during movement, as simple as walking. It is also impossible to determine, whether the patient wears the corset or not, because there are cases when the patients uses the device according to prescription, but there are no improvement in his/her condition or it even gets worse. No one can tell that the changes are because of wearing the tool that is made with the intention to be the best possible or by not using it at all. This paper is about the development of an intelligent battery powered measurement and data collection system for corsets to determine forces between patient and corset at selected points, to detect the corset's position, to measure temperature, and to save the collected data to internal memory. The equipment can be connected to PCs, in order to download and analyze collected data, such as time of wearing, force changes in a timeframe, extreme forces in different life situations. These data help make the corset more appropriate for the intended correction. Further investigations are made with magnetically attached sensors in dynamic motion tests, such as walking, to determine the movement of selected anatomical body points that are not detectable with the motion capture system through the plastic corset. This paper's main contribution is to show the novel data acquisition and processing system to help making corsets used in the treatment of spinal disorders, especially scoliosis, more effective.
Database of scoliosis examination stores a lot of data of patients. The exact numbers and the written opinion of doctors are useable in decision about the resolution of screening process. The moiré method and results of depth scanning are pictures. The conclusions are subjective. The paper is about a data mining based method for qualification of case to help of diagnosis.
Bevezetés: A Caregiver Priorities and Child Health Index of Life with Disabilities (CPCHILD-) kérdőív a sérült gyermekek egészségi állapotának és jóllétének felmérésére alkalmazott eszköz, amely a gondozók szemszögéből értékeli a gyermekek életminőségét. Célkitűzés: Tanulmányunk célja a Gondozói prioritások és fogyatékossággal élő gyermekek egészségindexének magyar nyelvre történő fordítása, kulturális adaptálása és a magyarországi cerebralis paresises betegcsoporton való alkalmazhatóságának vizsgálata (validálása), továbbá annak megítélése, hogy alkalmas-e a kérdőív a sérült gyermekeket nevelő szülők gondozói attitűdjeinek gyors felmérésére is. Módszer: A kérdőívet a Beaton-féle irányelvek szerint fordítottuk. A magyar nyelvű kérdőív megbízhatóságát az ismételhetőséggel, a megítélők közötti megbízhatósággal és a belső konzisztencia tesztelésével vizsgáltuk, korrelációs együtthatót (ICC) és Cronbach-alfa mutatót számoltunk. A kérdőív érvényességét a látszatérvényességgel és a kritériumcsoport-érvényességgel mértük. A szülői attitűdök vizsgálata során a betegeket járóképes és nem járóképes csoportokba osztottuk, és összefüggéseket kerestünk a kérdőív 7. részében szereplő 36 életminőségi kérdésre adott válaszokkal. Eredmények: Az ismételhetőség vizsgálatakor az ICC 0,96 (95% CI: 0,88–0,98) értékű volt, a Cronbach-alfa az 5. domén kivételével (0,67) meghaladta a 0,70-es minimális küszöböt (0,74–0,97). A megítélői megbízhatóság vizsgálatakor az ICC 0,87 (95% CI 0,70–0,94) eredményt adott. A látszatérvényesség minden kérdésre megvizsgálva meghaladta a 2,0 küszöbértéket (átlag: 3,4 ± 1,34; tartomány: 2,6–4,5). A kritériumcsoport-validitásnál szignifikáns különbséget találtunk a járóképes és a nem járóképes csoportok CPCHILD-pontszámai között. A gondozói attitűdök vizsgálata során különbözött a járóképes és a nem járóképes ICP-s gyermekek szüleinek véleménye az ülő helyzet életminőségben betöltött szerepének fontosságáról (2,89 ± 1,28 vs. 3,51 ± 0,82; p<0,01). Következtetés: Bemutatott munkánk kapcsán általánosan elérhetővé vált magyar nyelven is a CPCHILD-kérdőív, amelynek validitását is igazoltuk. Az az eredményünk, hogy az ülésre és az ülő helyzetben végzett tevékenységekre vonatkozó kérdésekre adott válaszok különbséget mutattak a járóképes és a nem járóképes betegcsoportok között, azt mutatja, hogy a nagymotoros funkciók alapján a IV., V. GMFCS-csoportba tartozó betegek szülei fontosabbnak vélik az ülés képességének meglétét, mint az I., II., III. GMFCS-kategóriába tartozókéi. Eredményeink továbbá arra is felhívják a figyelmet, hogy a CPCHILD-kérdőív használatával nemcsak a gyermekek jólléte, általános egészsége, hanem a szülői attitűdök is megismerhetők, ami tovább segítheti a gyermekek rehabilitációs folyamata során az elvárások és a lehetőségek közötti összhang kialakítását.
