Samenvatting Toepassing van een hypothesentoetsend model in de praktijk kan tot procedurele kwaliteitsverbetering leiden. Na de verschillende stappen van zo'n model te hebben besproken, worden drie redenen voor de toepassing ervan gegeven. Allereerst zou dit tot een verhoging van de wetenschappelijke kwaliteit van de diagnostiek leiden, omdat het proces systematischer verloopt en het beslissingsproces inzichtelijker wordt. Voorts krijgen foutenbronnen minder de kans om invloed uit te oefenen tijdens het complexe diagnostische proces. Zo zouden diagnosten de waarschijnlijkheden van hun oordelen juister inschatten en hun eerste indruk, indien noodzakelijk, bijstellen. Ten derde bevordert het model een behandelingsgerichte diagnostiek door de verklaringsgerichte werkwijze en door de fase van indicatiestelling. Keywords diagnosis Á hypothesis testing InleidingEen van de belangrijkste ontwikkelingen van de laatste tijd in de hulpverleningspraktijk aan kinderen is de sterke opleving van de belangstelling voor diagnostiek. In het bijzonder is men geı¨nteresseerd in het proces dat tot diagnostische beslissingen leidt: wat is de aard van dat proces en wat is de rol daarin van de diagnost als beoordelaar en beslisser? Werd diagnostiek in het verleden vaak gelijkgesteld aan het afnemen van diagnostische instrumenten, tegenwoordig beschouwt men haar als een complex proces van aanmelding tot en met advisering. Het onderkennen van de noodzaak tot een meer systematisch en doelgericht diagnostisch denken gaat hiermee gepaard. Doel is een diagnostisch proces dat op wetenschappelijk verantwoorde wijze verloopt.Een manier om de wetenschappelijke kwaliteit van de diagnostiek te verhogen, is het hanteren van een diagnostisch model waarin bepaalde wetenschappelijke regels toepasbaar zijn gemaakt. Een aantal diagnostische modellen, de zogenaamde hypothesentoetsende modellen, is gebaseerd op de grondfiguur van het wetenschappelijk probleemoplossend denken: de empirische cyclus van De Groot (1961). Deze modellen beschrijven de wijze waarop een diagnost vermoedens of hypothesen omtrent de aard en de verklaring van problemen dient te formuleren en te toetsen met het oog op het oplossen van die problemen.Wetenschappers en docenten zijn het erover eens dat diagnostiek op een verantwoorde wijze dient te geschieden en dat hypothesentoetsende modellen hieraan een bijdrage kunnen leveren (De Bruyn, 1984, 1985Rispens, Carlier & Schoorl, 1984, 1990Stevens, 1982 en Van Strien, 1984. Vooralsnog heeft deze opvatting weinig effect gehad op de diagnostische praktijk. Het diagnostische proces in de jeugdhulpverlening verloopt zelden systematisch en volgens methodologische regels. Dit is een ongewenste situatie omdat beslissingen gebaseerd op diagnostisch onderzoek verregaande gevolgen voor de clie¨nt en andere betrokkenen kunnen hebben.Hypothesentoetsende modellen zijn didactisch waardevol gebleken vanwege hun verhelderende en systematiserende werking. Op 17 november jongstleden organiseerden het NIP (sector Jeugd) en de Universiteit van Amsterdam (vakgroep Ontwikkelingspsychologie) het congres 'Sociaal vaardig: wel zo aardig!' Dit congres beoogde inzicht te geven in het aanbod van sociale vaardigheidstrainingen (Sovatrainingen) en lesprogramma's die de afgelopen jaren in Nederland zijn ontwikkeld.De belangstelling voor het congres bleek enorm: slechts 400 van de 800 geı¨nteresseerden konden deze dag bijwonen. Terecht sprak de dagvoorzitter Pijnenburg (SPI, Nijmegen) van een 'Sova-rage'.Uit de openingstoespraak van Pennekamp, directeurgeneraal van Welzijn, bleek dat het thema ook leeft bij beleidmakers binnen het ministerie van VWS.Van Lieshout (Katholieke Universiteit Nijmegen) constateerde vervolgens dat de bestaande programma's te weinig aansluiten bij de verschillende ontwikkelingstaken. Het belang van een gedegen diagnostiek en een daarop afgestemde Sova-training werd door Van Lieshout benadrukt. Praktische implicaties werden helaas niet altijd even duidelijk door de vele theoretische nuances die in kort tijdsbestek de revue passeerden.Eppink (Psychologisch Buro Eppink) illustreerde aan de hand van persoonlijke anekdotes uit enkele culturen hoe sterk 'sociale codes' per cultuur verschillen en hoe belangrijk het is om hier bij het ontwikkelen van een Sova-training rekening mee te houden. Alhoewel zijn verhaal een duidelijke boodschap bevatte, was het jammer dat een concrete uitwer-king ervan naar Sovatrainingen ontbrak.Als laatste spreker van de ochtend stelde Prins (Universiteit van Amsterdam) enkele opmerkelijke resultaten betreffende de effectiviteit van Sova-trainingen aan de orde. Zo blijken de behandelingseffecten meestal in maar heel beperkte mate te generaliseren naar de schoolklas of leefgroep. Bovendien zijn de korte-termijneffecten die met de meeste programma's worden verkregen na drie maanden al niet meer aantoonbaar. Prins bood echter ook enkele perspectieven. Door ouders en leerkrachten daadwerkelijk te trainen in het ondersteunen van nieuw verworven vaardigheden, kan generalisatie bevorderd worden. Ook het gegeven dat kortere programma's effectiever blijken te zijn dan langdurige programma's was opmerkelijk. Prins besloot zijn betoog onder meer met de stelling dat niet de toevallige beschikbaarheid, maar de afstemming van een programma op de aard van de sociale problemen de keuze voor een specifiek Sovaprogramma dient te bepalen.Opvallend was dat bij alle drie inleiders een paar thema's terugkeerden: Sova-trainingen moeten meer dan nu het geval is aansluiten bij het dagelijkse leven van kinderen, en ze moeten meer 'op maat' zijn en aansluiten bij de specifieke sociale problemen. Deze noodzaak bleek eens te meer uit het middagprogramma (waarvan ik het deelgebied basisonderwijs volgde), dat voornamelijk bestond uit presentaties van curricula en standaardprogramma's, met soms onwerkelijk aandoende situaties of opdrachten.Dekkers gaf een heldere toelichting op het preventieve programma 'Zeg nou zelf', dat de sociale weerbaarheid in de bovenbouw p...
Samenvatting Discussies over de functie van diagnostiek zijn onverminderd actueel. Te meer nu er belangrijke ontwikkelingen gaande zijn in de diagnostiek binnen de jeugdzorg. Twee recente ontwikkelingen zijn: verwetenschappelijking van het diagnostisch proces met een hypothesentoetsend model, en een clie¨ntgerichte diagnostiek met een contractmodel. In de praktijk heerst verwarring over de relatie tussen deze twee modellen. In dit artikel beschrijven we de praktische mogelijkheden en knelpunten van beide modellen, en we vergelijken ze aan de hand van drie aspecten van diagnostiek: het methodologische, vakinhoudelijke en relationele aspect. We bevelen ten slotte aan om zowel hypothesentoetsend als clie¨ntgericht te werken.
