Prednisolone is a corticosteroid, it works on immune system to relieve swelling, redness, and inflammation.it is a choice of drug for arthritis, cancer, asthma, allergic condition, bell's palsy, Nephrotic syndrome and disorders of endocrine system. Prednisolone helps decreasing inflammation by suppression of leukocytes and decrease expression of pro-inflammatory markers. Nephrotic syndrome is a condition in which damage kidney glomeruli which leaks protein into urine from blood. In pediatric population the choice of drug selected is prednisolone which resolves the symptoms and reduces infection. Prednisolone is the drug considered for the frequently relapsing Nephrotic syndrome. It should be given on alternate basis. for patient with infrequent relapses, therapy should be not prolonged. Adverse effects of prednisolone are hypertension, adrenal suppression, osteoporosis, diabetes mellitus, poor growth and cushingoid syndrome. Systemic toxicity due to prednisolone in pediatric patient increases the risk of infection and pre-renal failure. In this case report, it is summarized that despite of everyday prednisolone intake, alternate day therapy is more effective and causes less side effects. Proper monitoring is necessary and timely laboratory findings should be obtained for less adverse events.
Intractable hiccups (singultus) are continuous involuntary spasmodic contraction of diaphragm resulting in inspiration followed by closure of glottis. However, it is naturally occurring common phenomenon but intractable hiccups are a continuous form of hiccups which persist for more than 2 months. This can be tough task for physician to found out the organic cause of hiccups. We report a case of 40-year-old female who had hiccups since more than 2 months and she is known case of anxiety neurosis, for which ongoing treatment with anti-depressants and anti-anxiety is going on. Evaluating a psychogenic origin hiccup is the main consequence here.
NPK Biofertilizer are used for agricultural purpose to increase the crop yield. If swallowed in small amount they are poisonous and harmful to human health. Containing certain macronutrients such nitrogen, phosphorous and potassium can cause severely ill effects in the body and lead to life threating conditions. A 22-year-old female patient came to emergency ward with the complaints of abdominal pain with burning sensation, nausea, dizziness and giddiness. poisonous intoxication was removed out by gastric lavage. Patient was treated with includes inj. Ceftriaxone (1g) IV 12 hourly used as infection prophylaxis, Inj NS. 0.9%+ 3 ampule optineuron IV 12 hourly for rehydration, Inj. Pantoprazole (40) IV 12 hourly as antacid, Inj Emset 4cc IV 12 hourly as anti-emetic and Inj. Atropine 1 ampule as an antidote. Here there is case report of NPK Biofertilizer poisoning describing the severity and timely related treatment intoxication with poisonous substance.
