We describe a patient who developed progressive weakness in all limbs without sensory symptoms 4 weeks after upper respiratory system infection. Electrophysiological findings suggested a new variant of Guillain-Barré syndrome named "acute motor conduction block neuropathy". Electrophysiological studies were performed at admission, 12 and 28 weeks. At the 28 week, the clinical examination and electrophysiological findings showed complete recovery.
Objective: The most frequent electrodiagnostic symptoms in carpal tunnel syndrome (CTS) are: decrease of sensory conduction velocities at the palm-wrist and 1-4 finger-wrist segments, reduced compound nerve action potential amplitudes, and retardation of distal motor latency observed through the abductor pollicis brevis (ABP) muscle. Material and Methods: In this study, we examined 90 CTS patients with forearm median MCV within the normal limits and 100 controls. Results: Compared to controls a significant decrease in median motor conduction velocity (MCV) and amplitude of forearm in patients with carpal tunnel syndrome. In order to explain this decrease, we compare forearm median mixed nerve amplitudes of carpal tunnel syndrome and control groups and found that there was no significant differences between the median mixed nerve conduction velocities among groups. However there were significant differences in calculated forearm median mixed velocity and forearm median mixed nerve action potential amplitudes between CTS cases and controls, which has never been reported in the literature (p<0.001). Conclusion:In CTS, the reduction in stimuli speeds at the distal carpal tunnel and delay in motor distal latency observed through the ABP are both anticipated symptoms. However, the loss of amplitude and slowing of conduction at the proximal carpal tunnel compared to non-CTS cases imply that Wallerian degeneration occurred at the proximal segment due to CTS. Key Words: Carpal tunnel syndrome, axonal degeneration, median nerve Özet Amaç: Karpal tünel sendromunda (KTS) en sık karşılaşılan elektrodiagnostik bulgular; avuç içi-bilek ve parmak-bilek segmentlerinde duyu iletim hızlarında yavaşlama, amplitüd küçülmesi ve abduktor pollisis brevis kasından kayıtla bakılan distal motor latansta gecikmedir. Bazı hastalarda abduktor pollisis brevis kasından kayıtla elde edilen bilek-dirsek motor iletim hızında yavaşlama da izlenebilmektedir. Gereç ve Yöntemler: Bu çalışmada ön kol median motor iletim hızı normal sınırlarda olan 90 KTS'li hasta ile 100 kontrol olgu karşılaştırılarak, hızlar ve amplitüdler arası fark olup olmadığı araştırıldı. Bulgular: Karpal tünel sendromu olanlarda kontrollere göre ön kol median motor hızlarında ve amplitüdlerinde anlamlı bir azalma tespit edildi. Çalışmanın ikinci aşamasında bulunan bu ön kol median motor iletim hızındaki yavaşlamanın mekanizmasını açıklayabilmek için KTS'liler ile kontrol grubunun ön kol median karışık sinir amplitüdleri (B-D BSAP amp) ile direkt ve hesaplanan ön kol median karışık sinir iletim hızları karşılaştırıldı. Karpal tünel sendromlu olgular ile kontroller arasında direkt ön kol median karışık sinir iletim hızları arasında anlamlı bir fark tespit edilmedi. Ancak hesaplanan ön kol median karışık sinir hızında ve ön kol median karışık sinir aksiyon potansiyeli amplitüdünde KTS'li olgular ve kontroller arasında anlamlı bir farklılık mevcuttu (p<0,001). ÖZETGuillain-Barre sendromu (GBS) akut başlangıçlı, asendan ekstremite güçsüzlüğü ve arefleksi ile karakterizedir. GBS'nun birçok nadir görülen varyantı tanımlanmıştır. Faringoservikobrakiyal (FSB) varyant bunlardan biri olup, orofarinks, boyun ve üst ekstremite kaslarında güçsüzlük ile gider. Miller Fisher sendromu (MFS) ise ataksi, arefleksi ve eksternal oftalmoparezi ile seyreden bir diğer subtipidir. Olgumuz bulber kas güçsüzlüğü ve hafif pitoz ile prezente olmuştu. Nörolojik muayenesinde faringeal ve palatal kaslarda zafiyet ve jeneralize bir arefleksi tespit edilmiş olup, ekstremite güçsüzlüğü, oftalmoparezi veya ataksi saptanmamıştı. Lomber ponksiyonda BOS proteini yüksek bulundu. EMG'sinde demyelinizan özellik gösteren yaygın sensorimotor periferik nöropati saptandı. Hasta intravenöz immunglobulin (IVIg) tedavisi sonrasında tamamen normale döndü.
