Mục tiêu: Đánh giá hiệu quả của bổ sung Lactococcus lactis JCM 5805 đối với tỷ lệ mắc bệnh nhiễm khuẩn hô hấp và tiêu hóa và tình trạng dinh dưỡng ở trẻ 6-9 tuổi” tại 4 xã tỉnh Ninh Bình. Phương pháp nghiên cứu: Nghiên cứu thử nghiệm cộng đồng có đối chứng giả dược nhằm đánh giá hiệu quả cải thiện tình trạng mắc nhiễm khuẩn hô hấp sau 8 tuần sử dụng sản phẩm có bổ sung Lactococcus lactis JCM 5805 (LC-Plasma) trên trẻ tiểu học 6-9 tuổi tại 4 xã của tỉnh Ninh Bình. Chọn 1.109 trẻ 6-9 tuổi thỏa mãn yêu cầu, chia ngẫu nhiên thành 2 nhóm ăn uống bình thường tại gia đình, nhóm can thiệp được sử dụng sản phẩm có bổ sung Lactococcus lactis JCM 5805 (LC-Plasma) mỗi ngày liên tục trong 8 tuần, nhóm chứng sử dụng giả dược. Kết quả: tỷ lệ mắc, tỷ lệ mắc mới, số đợt mắc, số ngày mắc, chỉ số mắc theo thời gian về nhiễm khuẩn hô hấp ở nhóm can thiệp có xu hướng giảm so với nhóm placebo, tuy nhiên chưa có sự khác biệt có ý nghĩa thống kê giữa 2 nhóm ở các thời điểm sau 4 tuần và sau 8 tuần can thiệp. Số ngày mắc tích lũy của triệu chứng sốt, ho, chảy nước mũi trong 8 tuần của nhóm can thiệp thấp hơn so với số ngày mắc ở nhóm placebo (p<0,05). Số ngày nghỉ học do bệnh nhiễm khuẩn hô hấp ở nhóm can thiệp (33 ngày) thấp hơn số ngày nghỉ học ở nhóm placebo (62 ngày) (p<0,01). Kết luận: Sử dụng sản phẩm có chứa 50mg Lactococcus lactis JCM 5805 (LC-Plasma) 7 ngày/tuần trong 8 tuần đã có cải thiện về số ngày mắc tích lũy về nhiễm khuẩn hô hấp, số ngày nghỉ học tích lũy do nhiễm khuẩn hô hấp trên trẻ 6-9 tuổi tại Ninh Bình
Mục tiêu: Tìm hiểu một số yếu tố liên quan đến tình trạng dinh dưỡng của người bệnh tai biến mạch máu não tại Bệnh viện Lão khoa Trung ương năm 2019. Phương pháp: Nghiên cứu mô tả cắt ngang được tiến hành với cỡ mẫu 125 người bệnh tai biến mạch máu não từ tháng 8 năm 2018 đến tháng 3 năm 2019. Kết quả: Trong tổng số 125 người bệnh tai biến mạch máu, BMI bình thường chiếm 80,0%, suy dinh dưỡng chiếm 16,8%, thừa cân - béo phì chiếm 3,2%. Nhóm độ tuổi ≥70 có nguy cơ suy dinh dưỡng cao gấp 2,8 lần so với nhóm <70 tuổi (p < 0,05). Người bệnh nhồi máu não có nguy cơ suy dinh dưỡng cao gấp 1,9 lần so với những người bệnh xuất huyết não (p < 0,05). Người bệnh nuôi ăn qua sonde có nguy cơ suy dinh dưỡng cao gấp 1,6 lần so với nhóm người bệnh tự ăn được qua đường miệng và nuôi dưỡng qua đường tĩnh mạch (p < 0,05). Kết luận: Tỷ lệ suy dinh dưỡng ở người bệnh tai biến mạch máu não cao. Có mối liên quan giữa tuổi, loại tai biến mạch máu não và đường nuôi dưỡng đến tình trạng dinh dưỡng. Người bệnh tai biến mạch máu não cần được đánh giá tình trạng dinh dưỡng ngay sau khi nhập viện. Từ đó có định hướng, mục tiêu chăm sóc và cải thiện dinh dưỡng cho người bệnh.
Tai biến mạch máu não (TBMMN) là bệnh thường gặp ở người cao tuổi. Tình trạng bệnh lý ảnh hưởng tới tình trạng dinh dưỡng (TTDD) và nuôi dưỡng. Mục tiêu: Đánh giá TTDD và mô tả thực trạng nuôi dưỡng bệnh nhân TBMMN tại Bệnh viện Lão khoa Trung ương từ tháng 8 năm 2018 đến tháng 3 năm 2019. Phương pháp: Nghiên cứu mô tả cắt ngang. Kết quả: Trong số 125 Bệnh nhân TBMMN có tuổi trung bình 76,0 ± 10,4. Mức thay đổi lúc nhập viện và sau 1tuần đối với cân nặng là 50,9 ± 7,6 kg so với 50,7 ± 7,5, đối với BMI là 21,6 ± 2,2 kg/m2 so với 21,5 ± 2,1. Tỷ lệ bệnh nhân có nguy cơ suy dinh dưỡng (SDD) lúc nhập viện theo công cụ NRS 2002 là 77,6%. Tỷ lệ SDD theo BMI trong ngày đầu là 16,8%, sau 7 ngày tăng lên 22,4%. Tỷ lệ bệnh nhân được nuôi ăn sau <24 giờ đầu là 94,4%, từ 24 đến 48 giờ là 5,6%. Bệnh nhân được nuôi ăn qua sonde dạ dày chiếm tỷ lệ cao nhất với 68%. Tỷ lệ bệnh nhân đạt mức năng lượng từ 25 - 30 kcal/kg/ngày trong ngày đầu chiếm 22,4%, ngày thứ 7 tăng lên 48,8%. Kết luận: Tỷ lệ bệnh nhân TBMMN có nguy cơ SDD cao và tăng lên sau 1 tuần nằm viện. Bệnh nhân được nuôi dưỡng phần lớn qua đường ruột trong vòng 24 giờ đầu nhập viện với mức năng lượng và protein tăng lên sau 1 tuần nằm viện nhưng chưa đủ mức nhu cầu khuyến nghị.
