Mục tiêu: Đánh giá kết quả điều trị bước 1 ở bệnh nhân ung thư phổi không tế bào nhỏ giai đoạn tiến xa có đột biến gen EGFR dương tính bằng afatinib. Đối tượng và phương pháp: Nghiên cứu tiến cứu, theo dõi dọc 41 bệnh nhân ung thư phổi không tế bào nhỏ giai đoạn tiến xa, có đột biến gen EGFR dương tính được điều trị bước 1 bằng thuốc afatinib, theo dõi và đánh giá đáp ứng mỗi 3 tháng hoặc khi có triệu chứng của bệnh tiến triển. Tiêu chuẩn chính là thời gian sống thêm bệnh không tiến triển (PFS), tỷ lệ đáp ứng khách quan (ORR), tiêu chuẩn phụ là thời gian sống thêm toàn bộ (OS), tác dụng không mong muốn. Kết quả: Tỷ lệ đáp ứng khách quan là 92,6%, trung vị PFS 14 tháng, trung vị OS 20 tháng, tỷ lệ OS ước tính 1 năm và 2 năm lần lượt là 93,3% và 39,1%. Trung vị PFS ở bệnh nhân không có di căn não là 14 tháng, dài hơn ở bệnh nhân di căn não (12 tháng), p>0,05. Trung vị PFS ở bệnh nhân mang đột biến gen nhạy thuốc là 14 tháng, dài hơn bệnh nhân mang đột biến hiếm (11 tháng), p>0,05; trung vị OS ở bệnh nhân mang đột biến xóa đoạn exon 19 là 32 tháng, dài hơn trung vị OS ở bệnh nhân mang đột biến điểm L858R và đột biến hiếm lần lượt là 20 tháng và 17 tháng, p>0,05. Tác dụng không mong muốn gặp ở 82,9% bệnh nhân, chủ yếu là nổi ban (80,5%) đều là độ 1, độ 2. Có 5% bệnh nhân bị tiêu chảy độ 3. Kết luận: Afatinib có hiệu quả tốt trong điều trị bước 1 ở bệnh nhân ung thư phổi không tế bào nhỏ giai đoạn tiến xa có đột biến gen EGFR, đặc biệt, thời gian sống thêm toàn bộ có xu hướng dài hơn ở nhóm bệnh nhân mang đột biến xóa đoạn ở exon 19 so với đột biến L858R và đột biến hiếm.
Mục tiêu: Đánh giá mối liên quan giữa đột biến gen EGFR với nguy cơ di căn não ở bệnh nhân ung thư phổi không tế bào nhỏ (UTPKTBN). Đối tượng và phương pháp: Nghiên cứu tiến cứu, theo dõi dọc 213 bệnh nhân UTPKTBN giai đoạn I - IIIB được xét nghiệm đột biến gen EGFR từ năm 2014 đến 2020. Yếu tố nguy cơ được phân tích đa biến bằng phương trình hồi quy tuyến tính. Kết quả: Có 41,3% bệnh nhân xuất hiện di căn xa, trong đó di căn não 22,7%. Tỉ lệ đột biến gen EGFR dương tính là 27,2%, có 37,5% bệnh nhân di căn não mang đột biến gen EGFR, OR 2,89 (khoảng tin cậy 95%: 1,011 - 8,270), p=0,048. Bệnh nhân mang đột biến điểm L858R ở exon 21 có tỉ lệ di căn não 55,6% xu hướng cao hơn bệnh nhân mang đột biến xóa đoạn ở exon 19 (28,6%), nhưng sự khác biệt không có ý nghĩa thống kê (p>0,05). Không thấy yếu tố khác liên quan đến nguy cơ di căn não. Kết luận: Đột biến gen EGFR là một yếu tố làm tăng nguy cơ di căn não ở bệnh nhân ung thư phổi không tế bào nhỏ.
Mục tiêu: So sánh hình ảnh tổn thương màng phổi ở bệnh nhân tràn dịch màng phổi do lao và ung thư phổi di căn. Đối tượng và phương pháp: Nghiên cứu mô tả, cắt ngang đặc điểm tổn thương màng phổi ở 186 bệnh nhân tràn dịch màng phổi, được nội soi màng phổi bằng ống nội soi màng phổi bán cứng dưới gây tê cục bộ tại chỗ. Kết quả: 59,6% bệnh nhân tràn dịch màng phổi do ung thư phổi di căn, 40,4% bệnh nhân tràn dịch màng phổi do lao. Dịch màng phổi màu đỏ máu xuất hiện ở 59,5% bệnh nhân do ung thư phổi di căn, cao hơn so với căn nguyên do lao 42,75%, dịch màng phổi màu vàng có ở 57,3% bệnh nhân tràn dịch màng phổi do lao so với 40,5% bệnh nhân tràn dịch màng phổi do ung thư phổi di căn, sự khác biệt có ý nghĩa thống kê với p<0,05. Týp ung thư biểu mô tuyến ở phổi chiếm tỉ lệ cao nhất với 67,57%. Hình ảnh tổn thương màng phổi dạng u sùi tụ thành đám và nốt tổn thương có kích thước to nhỏ không đều gặp chủ yếu ở bệnh nhân tràn dịch màng phổi do ung thư phổi di căn với tỷ lệ lần lượt là 32,4% và 45%. Tổn thương dày dính màng phổi và nhiều nốt nhỏ trắng xám đều nhau là tổn thương thường gặp trong tràn dịch màng phổi do lao với 44% và 72% theo lần lượt, sự khác biệt có ý nghĩa thống kê với p<0,05. Kết luận: Tràn dịch màng phổi do ung thư phổi di căn thường gặp dịch màu đỏ máu, tổn thương màng phổi dạng u sùi tụ thành đám và nốt kích thước to nhỏ không đều, còn hình ảnh dày dính màng phổi, nhiều nốt nhỏ màu trắng xám đều nhau và dịch màng phổi màu vàng thường gặp trong lao màng phổi với p<0,05.
