Mục tiêu nghiên cứu: Nghiên cứu đánh giá kết quả điều trị sỏi thận tái phát bằng phương pháp phẫu thuật tán sỏi qua da. Phương pháp nghiên cứu: Mô tả cắt ngang có phân tích trên 89 bệnh nhân sỏi thận tái phát được điều trị bằng tán sỏi nội soi qua da. Kết quả: Người bệnh thuộc nhóm 41 - 60 tuổi chiếm 66,3%. Số lượng bệnh nhân nam chiếm 60,7%; người bệnh có chỉ số BMI bình thường chiếm 77,5%. Đặc điểm lâm sàng người bệnh: 97,8% bệnh nhân có triệu chứng đau thắt lưng. Đái máu chiếm 17,3%; cơn đau quặn thận chiếm 6,7%. Đặc điểm cận lâm sàng: Sỏi thận phức hợp chiếm 30,3%, bệnh nhân sỏi bể thận đơn thuần chiếm 23,6%. Nhóm sỏi có kích thước từ 2-3cm chiếm tỷ lệ nhiều nhất với 52,8%; số lượng người bệnh có từ 3 viên sỏi trở lên chiếm nhiều nhất với 56,2%; diện tích bề mặt sỏi từ 300 mm2 và dưới 200mm2 tương đương nhau; Mức độ giãn đài bể thận độ I và độ II chiếm nhiều nhất với 59,6%. Kết quả tán sỏi qua da: Có 65,2% người bệnh chỉ cần 1 lần chọc dò vào bể thận. Bệnh nhân sử dụng 1 đường hầm trong quá trình tán sỏi chiếm 88,8%. Bệnh nhân chọc dò vào đài thận từ vị trí đài giữa chủ yếu chiếm 82,0%. Kêt quả ghi nhận 5 trường hợp tai biến trong phẫu thuật và 13 biến chứng trong phẫu thuật. Kết luận: Đường chọc dưới sườn vào nhóm đài dưới sau được đa số các phẫu thuật viên lựa chọn Đường vào đài trên hoặc đài giữa thuận lợi tiếp cận sỏi tán sỏi dưới da.
Mục tiêu: Đánh giá kết điều trị sỏi thận san hô bằng phương pháp tán sỏi thận qua da đường hầm nhỏ tại Bệnh viện Đa khoa tỉnh Hải Dương năm 2020. Phương pháp: Nghiên cứu mô tả, cắt ngang 52 bệnh nhân sỏi thận san hô được điều trị bằng tán sỏi nội soi qua da đường hầm nhỏ tại bệnh viện đa khoa tỉnh Hải Dương. Kết quả: Tuổi trung bình: 54,9 ± 11,9 tuổi; Kích thước sỏi trung bình trên cắt lớp vi tính là 28,1 ± 5,6 mm; Số lượng sỏi: 69,2% có 1 viên, 7,7% có 2 viên và 23,1% có từ 3 viên trở lên; Phân loại sỏi: Sỏi san hô toàn phần 2/52 bệnh nhân, sỏi san hô bán phần 50/52 bệnh nhân (chiếm 96,2%); Số lần chọc dò: 84,6% 1 lần chọc, 11,5% 2 lần chọc và 3,8% 3 lần chọc dò; Số đường hầm: 1 đường hầm 37/52 bệnh nhân (chiếm 71,2%), 2 đường hầm 15/52 bệnh nhân (chiếm 28,8%); Thời gian chọc dò: 13,31 ± 6,14 phút; Tỷ lệ biến chứng sau phẫu thuật là 13/52 bệnh nhân (chiếm 25,0%); Tỷ lệ sạch sỏi sau 3 ngày là: 67,3%, sau 1 tháng là 80,8%; Kết quả chung sau phẫu thuật: Tốt 67,3%, trung bình 30,8%, xấu 1,9%. Kết luận: Điều trị sỏi thận san hô bằng phương pháp tán sỏi nội soi qua da tại bệnh viện Đa khoa tỉnh Hải Dương là phương pháp an toàn, hiệu quả với tỷ lệ sạch sỏi cao và tai biến biến chứng thấp.
Mục tiêu: Nghiên cứu đặc điểm lâm sàng, cận lâm sàng của bệnh nhân vỡ bàng qua do chấn thương tại bệnh viện Việt Đức từ tháng 1/2014 – 9/2020. Phương pháp nghiên cứu: nghiên cứu mô tả cắt ngang kết hợp hồi cứu và tiến cứu trên 67 bệnh nhân vỡ bàng quang được điều trị tại bệnh viện Việt Đức. Kết quả: Tuổi trung bình của bệnh nhân là 34,5 ± 12,8 tuổi; Bệnh nhân nam chiếm 82,1%, nữ chiếm 17,9%. Nguyên nhân do tai nạn giao thông chiếm 86,6%, tai nạn lao động 13,4%; Thời gian trung bình từ khi bệnh nhân bị tai nạn đến khi vào viện là 4,08 ± 1,34 giờ; Thời gian trung bình từ khi bệnh nhân nhập viện đến khi đươc chẩn đoán là 3,02 ± 1,14 giờ; Sốc chấn thương chiếm 37,3%; Kết quả chụp X quang: Vỡ xương chậu 11,9%, có hơi trong ổ bụng 11,9%, có dịch ổ bụng 4,5%; Kết quả chụp cắt lớp vi tính: Tổn thương thành bàng quang 85,1%, thoát thuốc vào ổ bụng 43,3%, hình ảnh vỡ xương chậu 40,3%;Tạng tổn thương phối hợp: tốn thương gan chiếm 6%, thận 1,5%, lách 1,5%, đại tràng 9,0%, Xương chậu 40,3%. Kết luận: Chấn thương vỡ bàng quang thường gặp ở độ tuổi lao động, nguyên nhân do tai nạn giao thông chiếm đa số với tỷ lệ sốc chấn thương là 37,3% và vỡ xương chậu phối hợp là 40,3%
