Nghiên cứu nhằm đánh giá tác động của đại dịch COVID-19 đối với sức khỏe tâm thần của nhân viên y tế dự phòng. Nghiên cứu cắt ngang sử dụng bộ câu hỏi phỏng vấn trực tiếp và thang đo tác động quy mô sự kiện(IES-R) trên 455 nhân viên y tế từ các trung tâm kiểm soát bệnh tật (CDC) ở 4 tỉnh trong đại dịch COVID-19 vào năm 2021. Kết quả nghiên cứu cho thấy 45,93% đối tượng có vấn đề sức khỏe tâm thần cần được quan tâm, 8,8% có ảnh hưởng sức khỏe tâm thần lâu dài trong nhiều năm, và 2,8% được chẩn đoán mắc trạng thái căng thẳng. Điểm trung bình của “Ám ảnh” về COVID 19 là cao nhất (11,34 ± 6,67), tiếp theo là “Tránh” (7,35 ± 5,79) và “Phản ứng thái quá” (6,86 ± 5,20). Cán bộ, nhân viên y tế làm việc tại khoa phòng chống dịch bệnh truyền nhiễm thường tránh mắc bệnh hơn các nhân viên làm công tác phòng chống dịch bệnh truyền nhiễm. Nhân viên nữ có nguy cơ bị ám ảnh, né tránh và mắc bệnh thái quá cao hơn nam giới.
Đặt vấn đề: Rối loạn nhịp nhanh bộ nối tăng tính kích thích (Junction Ectopic Tachycardia - JET) đặc trưng bởi nhịp tim nhanh với phức bộ QRS hẹp và phân ly nhĩ thất, xuất hiện sớm trong 24 - 48 giờ sau phẫu thuật, gây ảnh hưởng đến diễn biến điều trị và kết quả hồi sức bệnh nhân sau phẫu thuật. Các yếu tố nguy cơ trước - trong - sau phẫu thuật liên quan tới rối loạn nhịp JET là gì? Các phương pháp điều trị hiện nay chưa có cách lựa chọn nào thực sự tốt nhất.Mục tiêu: Xác định tỷ lệ rối loạn nhịp JET, các yếu tố liên quan và nhận xét kết quả điều trị sớm rối loạn nhịp JET sau phẫu thuật tim mở tim bẩm sinh.Đối tượng và phương pháp: Mô tả trên 494 bệnh nhân sau phẫu thuật tim mở tim bẩm sinh được hồi sức tại Khoa Hồi sức Ngoại tim mạch – Bệnh viện Nhi Trung ương từ 01/12/2017 đến 30/06/2018.Kết quả: Tỷ lệ JET trong tổng số rối loạn nhịp tim là 26,1%. Tỷ lệ JET cao nhất ở nhóm thông liên thất - hẹp eo/gián đoạn quai động mạch chủ (19,0%), tiếp đó là tứ chứng Fallot (15,4%). Các yếu tố như tuổi phẫu thuật nhỏ, cân nặng thấp, thời gian tuần hoàn ngoài cơ thể kéo dài, thời gian cặp chủ kéo dài và hạ kali máu có nguy cơ gây rối loạn nhịp JET sau phẫu thuật (p<0,001). Tỷ lệ dùng thuốc cordarone chiếm tỷ lệ cao nhất (82,6%), tiếp đến là phương pháp chườm mát (73,9%), đặt máy tạo nhịp vượt tần số (65,2%). Hầu hết bệnh nhân rối loạn nhịp JET đều phải sử dụng các phương pháp điều trị phối hợp. Tất cả bệnh nhân rối loạn nhịpJET đều trở về nhịp xoang và không có bệnh nhân nào tử vong.
Kết luận: Rối loạn nhịp JET thường gặp nhất, xuất hiện sớm trong 24 giờ sau phẫu thuật,đặc biệt sau sửa toàn bộ tứ chứng Fallot và vá thông liên thất. Kiểm soát tốt các yếu tố nguy cơliên quan trước - trong - sau phẫu thuật để hạn chế loại rối loạn nhịp này. Bệnh nhân rối loạnnhịp JET đáp ứng tốt với điều trị thuốc cordarone và liệu pháp hạ thân nhiệt.
Mục tiêu: Mô tả một số đặc điểm vận động thô ở trẻ bại não thể co cứng tại bệnh viện Nhi Trung ương năm 2018.Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang có phân tích, nghiên cứu định lượng trên 112 trẻ bại não thể co cứng, sử dụng thang đo GMFCS.Kết quả: Tình trạng vận động thô theo GMFCS, 54,5% ở mức 5; 21,4% ở mức 4 và 24,1% ở mức 3. Không tìm thấy sự khác biệt tỷ lệ các mức độ tình trạng vận động thô theo GMFCS với các đặc điểm độ tuổi, giới tính, tiền sử cân nặng sơ sinh, tiền sử tuổi thai khi sinh, tiền sử kiểu sinh, tiền sử tình trạng sinh, thời gian chẩn đoán bại não và loại bại não thể co cứng (p>0,05).Kết luận: Phần lớn trẻ bại não có tình trạng vận động thô theo GMFCS ở mức 5, nghiên cứu cũng cho thấy không sự khác biệt về tỷ lệ các mức độ vận động theo GMFCS với các đặc điểm của trẻ.Từ khóa: đặc điểm vận động thô, bại não thể co cứng, thang đo GMFCs
