Objective: In this study we aimed to define clinical, radiologic and pathological specialties of patients who applied to General Surgery Department of Atatürk University Medical Faculty with granulomatous mastitis and show medical and surgical treatment results. With the help of this study we will be able to make our own clinical algorithm for diagnosis and treatment. Materials and Methods:We searched retrospectively addresses, phone numbers and clinical files of 93 patients whom diagnosed granulomatous mastitis between a decade of January 2001 -December 2010. We noted demographic specialties, ages, gender, medical family history, main complaints, physical findings, radiological and laboratory findings, medical treatments, postoperative complications and surgical procedures if they were operated; morbidity, recurrence and success ratios, complications after treatment for patients discussed above. Results:In this study we evaluated 93 patients, 91 females and 2 males, with granulomatous mastitis retrospectively who applied to General Surgery Department of Atatürk University Medical Faculty between January 2001 and December 2010. Mean age was 34.4 years. The diagnosis was confirmed by histopathologic examination of the lesions. Seventy three patients had idiopathic granulomatous lobular mastitis and 20 patients had specific granulomatous mastitis IGM (18 tuberculosis mastitis, 1 alveolar echinococcosis and 1 silk reaction). All the patients had surgical debridement or antibiotic, and anti-inflammatory treatment with results bad clinical response before applied our clinic. Conclusion:Empiric antibiotic therapy and drainage of the breast lesions are not enough for complete remission of idiopathic granulomatous mastitis. The lesion must be excised completely. In selected patients, corticosteroid therapy can be useful. In the patients with tuberculous mastitis, abscess drainage and antituberculous therapy can be useful, but wide excision must be chosen for the patients with recurrent disease. IntroductionHemangioma is a relatively common benign proliferation of blood vessels that primarily develops during childhood. Two main forms of hemangioma recognized: capillary and cavernous. The capillary form presents as a flat area consisting of numerous small capillaries. Cavernous hemangioma appears as an elevated lesion of a deep red color, and consists of large dilated sinuses filled with blood. The purpose of the study was to report the case of a capillary hemangioma in a patient and to describe the successful treatment of this case.Case presentationThe patient was a 19-year-old female who presented herself to the Atatürk University, Faculty of Dentistry, Department of Periodontology, with the complaint of bleeding and slowly enlarging mass on the upper right molar region. The lesion was diagnosed as capillary hemangioma after clinical examination and biopsy. Treatment consisted of scaling, root planning and surgical excision. Four months after surgery healing was occurred and two years later area of the lesion appeared completely normal as clinically.ConclusionsThe surface is highly keratinized and no further growth was evidenced during the two year of follow-up. Early detection and biopsy is necessary to determine the clinical behavior of the tumor and potential dentoalveolar complications.

Idiopathic pulmonary hemosiderosis in children with celiac disease We read with interest the recent article by Malhotra et al. 1 regarding celiac disease (CD) as a cause of unusual severe anemia in a young man with idiopathic pulmonary hemosiderosis (IPH). According to their case report, CD should be specifically looked for in patients with IPH, especially those in whom the severity of anemia is disproportionate to radiological findings even in the absence of gastrointestinal symptoms. Here we present a 15 1 2-year-old boy, with oneyear history of recurrent hemoptysis. Although he has had failure to thrive, pallor, and fatigue for years, he had not been taken to any hospital so far. He was admitted to our hospital with dyspnea and hemoptysis. He had no family history of tuberculosis. On physical examination axillary temperature, blood pressure, heart rate, and respiratory frequency were 37 1C, 120/60 mmHg, 105/min, and 56/min, respectively. His weight was 41 kg (3-10 percentiles) and height was 154 cm (3-10 percentile). He was pale and had clubbing of fingers. His liver was 3.5 cm palpable and spleen was unpalpable. On the auscultation, fine end inspiratory crackles were heard from both left and right hemithorax. Laboratory evaluation was as follows: Hb 4.8 mg/ dl, Htc 16.5%, leukocyte count 12,900/mm 3 , platelet count 515,000/mm 3 , reticulocyte count 4%. His mean corpuscular volume (MCV) and red cell distribution (RDW) were 61 fl, and 19, respectively. Urinalysis, serum urea, creatinine, glucose, creatine kinase (CK), and electrolytes (Na, K, Ca,) were normal. Direct Coombs' test was negative, and hemoglobin electrophoresis was normal. Antigliadin IgA, IgG and antiendomysial antibodies were positive. Skin test for tuberculosis was negative. Chest X-ray revealed bilateral reticulonodular infiltration (Fig. 1). Female sex steroids may play a role in the proliferation of meningiomas, which usually have a high level of progesterone receptors (PgRs). We aimed to investigate the presence of PgRs and the Ki-67 labeling index (Ki-67 LI) in meningioma cases (N = 110) in relation to the severity of the disease. We studied PgR immunoreactivities and the Ki-67 LI in paraffin-embedded sections from meningiomas. Immunodetection of PgRs was conducted with peroxidase-antiperoxidase complex. Immunodetection of Ki-67 antigen was achieved by streptavidin-biotinperoxidase complex. Of 110 meningiomas, in 57 (52%) the immunostaining for PgRs was moderately to strongly positive (Group 1), in 23 (20%) weakly positive (Group 2), and in 30 (28%) negative (Group 3). The positive immunostaining rate for the PgR in the benign meningiomas (76%) was significantly higher than that in nonbenign tumors. The positive immunostaining rate for the PgR was significantly higher in women (81%) than men (55%). The results suggested that progesterone may play a role in the growth of meningiomas. Our results confirmed that the immunodetection of the PgR and Ki-67 antigens on the paraffin sections of meningiomas provides a valuable tool for estimating the biological behavior of meningiomas.
