Dans le cadre des études qui concernent la réponse des matériaux soumis à de fortes sollicitations dynamiques, la prise en compte de leur comportement élastoplastique s'est avérée nécessaire pour améliorer les restitutions numériques d'expériences, au regard d'un traitement hydrodynamique pur. Cependant, il apparaît que la prise en compte de l'endommagement du matériau améliore encore les simulations par rapport à l'expérience. Pour étudier le comportement des matériaux dans ce domaine, nous développons l'exploitation des chocs de plaques à partir des méthodes d'analyse lagrangienne.
Introduction Le SARS-CoV-2, nouveau virus respiratoire, semble prédisposer à la survenue de maladie thromboembolique veineuse (MTEV). Cela s’explique par sa pathogénie particulière qui reste encore mal comprise. Des études sont nécessaires pour caractériser ces patients. Objectif L’objectif principal est de décrire les patients hospitalisés au CHU de Saint-Étienne pour un Covid-19 et présentant une MTEV, ainsi que leurs évolutions au cours de l’hospitalisation. Les objectifs secondaires sont de suivre les patients à 3 mois puis à 6 mois afin de décrire leur évolution. Il s’agit d’une étude observationnelle rétrospective, monocentrique, entre 03/2020 et 05/2020. Résultats Parmi les 583 patients hospitalisés, 38 (6,52 %, IC 95 % [4,79 ; 8,82]) ont reçu un diagnostic de MTEV, majoritairement une embolie pulmonaire. 26 % sont décédés durant l’hospitalisation et plus d’un tiers ont été hospitalisés en réanimation dans les 24 h suivant leur admission. Il existait une majorité d’hommes (74 %) avec des facteurs de risque cardiovasculaires (surpoids, obésité, hypertension). Durant l’hospitalisation, 18 % des 38 patients ont présenté une hémorragie majeure. À la sortie d’hospitalisation, 87 % des patients avaient un traitement anticoagulant. À trois mois, chez les 20 patients ayant présenté une embolie pulmonaire, 2 (10 %) patients sont décédés, le traitement a été poursuivi chez 9 (45 %) patients. Il n’y a pas eu d’hémorragie majeure ou de récidive de MTEV. À six mois, il n’y a pas eu de nouveau décès ou d’évènement intercurrent notamment hémorragique ou récidive de MTEV. Chez les 9 patients encore sous anticoagulant, le traitement a été arrêté à 6 mois chez 80 % des patients. La dyspnée persistait chez 6 (33 %) patients. Une scintigraphie ventilation-perfusion a pu être réalisée chez 8 (44 %) des patients, seuls 2 (25 %) patients gardaient des séquelles. Conclusion Dans notre CHU de Saint-Étienne, 6,5 % des patients hospitalisés pour un Covid-19 ont présenté une MTEV, majoritairement une EP. Ces patients étaient souvent graves. À six mois, 45 % des patients ayant présenté une EP étaient encore anticoagulés, il n’y a pas eu d’évènement hémorragique. D’autres études sont nécessaires du fait du mécanisme particulier des complications thrombotiques dans le Covid-19.
