Non-erosive esophagitis is a chronic inflammatory condition of the esophagus and is a form of gastroesophageal reflux disease. There are limited treatment options for non-erosive esophagitis, and it often progresses to Barrett’s esophagus and esophageal carcinoma. Hydrogen sulfide has been demonstrated to be a critical mediator of gastric and intestinal mucosal protection and repair. However, roles for H2S in esophageal mucosal defence, inflammation and responses to injury have not been reported. We therefore examined the effects of endogenous and exogenous H2S in rat models of non-erosive esophagitis. Mild- and moderate-severity non-erosive esophagitis was induced in rats through supplementation of drinking water with fructose, plus or minus exposure to water-immersion stress. The effects of inhibitors of H2S synthesis or of an H2S donor on severity of esophagitis was then examined, along with changes in serum levels of a pro- and an anti-inflammatory cytokine (IL-17 and IL-10, respectively). Exposure to water-immersion stress after consumption of the fructose-supplemented water for 28 days resulted in submucosal esophageal edema and neutrophil infiltration and the development of lesions in the muscular lamina and basal cell hyperplasia. Inhibition of H2S synthesis resulted in significant exacerbation of inflammation and injury. Serum levels of IL-17 were significantly elevated, while serum IL-10 levels were reduced. Treatment with an H2S donor significantly reduced the severity of esophageal injury and inflammation and normalized the serum cytokine levels. The rat models used in this study provide novel tools for studying non-erosive esophagitis with a range of severity. H2S contributes significantly to mucosal defence in the esophagus, and H2S donors may have therapeutic value in treating esophageal inflammation and injury.
Inhibition of endogenous H2S synthesis provides a novel experimental model that can be useful in preclinical studies NSAID-related non-reflux oesophagitis. H2S contributes significantly to mucosal defence in the oesophagus.
Recent data of study H 2 S in gastrointestinal tract has proven its potent cytoprotection on mucosal defense among acid-related diseases in the gut. The aim was to evaluate the effects of H 2 S-releasing aspirin derivative (ATB-340) on esophageal and gastric mucosa compromised by stress injury. Rats were treated with vehicle (control), aspirin (10 mg/kg), ATB-340 (17.5 mg/kg) single or 9 days duration, with or without induction of stress injury. Esophageal mucosa, gastric mucosa were estimated by histopathological damage scoring. Serological levels of VCAM-1, IL-6 by ELISA. ATB-340 treatment resulted in protective effect and lower grade of damage score in esophageal mucosa and gastric mucosa lesions vs effect of aspirin in single or 9 days applications. The serum levels of VCAM, IL-6 in rats who were aspirin-treated and subjected to stressinjury were higher than those in control animals. Treatment with ATB-340 produced an anti-inflammatory effect by decreasing VCAM and IL-6 vs aspirin. Cytoprotective effect of ATB-340 on esophageal mucosa and gastric mucosa was modulated by inhibi ting inflammation and improving endothelial functions. K e y w o r d s: H 2
Metaflammation (metabolic inflammation) is low‐grade chronic inflammatory status linking to nutrient excess. Visceral adipocytes in mesenterium (AM) related to high fructose diet (HFD) are sources for metaflammation process, seriously threatening human health. Novel pathogenesis of AM is still unclear and there are no specific effective drugs for treatment of AM. Recently hydrogen sulfide (H2S) demonstrated potent vasodilatation actions which is promised for AM that have reduced vascularization Therefore, the aim of this study was designed to investigate the effects and underlying mechanisms of H2S on the prevention and treatment of experimental rats with AM during HFD. Methods The study was done on rats with standard diet (SD, control group) and 28 days HFD, and acute water immersion restraint stress (WRS) was induced for evaluation AM adaptive response and testing therapeutic potential of exogenous H2S donor NaHS (100 μmol/kg/day) and the hybrid H2S‐Aspirin (ATB 340, H2S‐ASA, 17,5 mg/kg/day) vs conventional aspirin (ASA, 10 mg/kg/day) on AM were evaluated by damage index by electronic microscopic study, and glucose level, activity of Cystathionine gamma‐lyase (CSE), Cystathionine beta‐synthase (CBS), Thiosulfate‐dithiol sulfurtransferase (TST) and Carbonyl group (CG) level in blood were examined biochemically (Institutional Ethical Committee Approval: 23/04/2018 № 4). Results In HSD group with NaHS treatment protected the endothelial and sub‐endothelial structures in mesenterium destructions were absent, the nucleus of endothelial cells it was observed an uneven condensation of chromatin. Effect of hybrid H2S‐aspirin characterized as a protective on AM against HFD and WRS‐induced injury, exhibited a downward trend in the inflammatory infiltration, lowering the level CG, which may indicate a reduction in oxidative stress under the influence of hybrid H2S‐aspirin. ATB‐340 showed an increase CSE, CBS and TST activities vs ASA pretreatment (p<0.05). The use of aspirin and NaHS has shown an increase on 67% CSE and CBS activities in HSD rats vs saline group. Conclusion These findings suggested that exogenous H2S prevented and ameliorated AM metflammation via reduction of oxidative damage, and provided a theoretical basis for effective tool for prevention of HFD‐related AM against long‐term postprandial hyperglycemia. За останні десятиліття значно зросла кількість малоінвазивних хірургічних втручань як в плановій хірургії так і в ургентній ендоскопічній хірургії. З розвитком сучасних відеоендоскопічних систем активно розвиваються ендоскопічні інструменти та аксесуари для виконання ендоскопічних операцій. Сучасною розробкою у вигляді сухої форми гемостатичного клею-неорганічного походження, що з допомогою аерозолю доставляється ендоскопічним методом є Hemosprey, фірми Cook, препарат сертифікований та широко використовується в країнах Європейського Союзу, Канаді та США з 2011р. Коли порошок контактує з кров'ю, він поглинає воду і утворює гель, який діє когезивно і адгезивно, створюючи стабільний механічний бар›єр, який охоплює місце кровотечі. Дана методика ще не поширена в Україні, тому нами зроблена спроба вивчити ефективність даного препарату в умовах регіональної лікарні. Актуальність проблеми підкреслює висока захворюваністьсереднє число госпіталізацій з приводу кровотеч складає 1 на 1000 населення. В залежності від етіологічного фактора, летальність досягає від 3 до 50%. Загальна летальність при шлунково-кишкових кровотечах у місті Києві складає 3,1%, по Україні в цілому-8,5%. Серед пацієнтів переважають люди працездатного віку. Мета роботи. Вивчити ефективність методу зупинки кровотечі за допомоги Hemospray в умовах ендоскопічного втручання. Дослідити ефективність використанням гемостатичного клею як методу моногемостазу, так і в комплексі з іншими ендоскопічними методами ендогемостазу кровотеч ШКТ. Матеріали та методи. До когорти спостереження ввійшло 7 пацієнтів (чоловіків 5, жінок 2), віком 28-79 років (середній вік 52±7,1 років), які на першому етапі лікувались у Львівській обласній клінічній лікарні, хірургічному відділенні №3, хірургічному відділенні №2 а на другому-у гастроентерологічному відділенні ЛОКЛ в період з 2018 до 2019 рр. Використовували відеоендоскоп Fujinone-250, кліпатори Hemoclips виробник Olympus, засіб порошкового гемостазу Hemosprey виробник Cook При зупинці кровотечі за рішенням хірурга-ендоскопіста порошковий гемостаз комбінувався з кліпуванням джерела кровотечі. Результати. Результатом наведеного ендоскопічного гемостазу з допомогою засобу Hemospray як при монометоді, так і в комплексі з ендоскопічним кліпування з подальшою консервативною терапією врятувало від ургентних хірургічних втручань та зберегло життя 7 хворих. В жодного із пацієнтів не було рецидиву кровотечі. Пацієнти були із різною етіологією та причиною кровотеч ШКТ. 100% хворих яким був проведений ендоскопічний гемостаз засобом Hemospray дав позитивний ефект.
