Background: Multiple studies have described increased risk of severe coronavirus disease among pregnant women compared to nonpregnant women. The risk in middle-income countries where the distributions of age groups and preexisting conditions may differ is less known. Objectives:To determine whether pregnant women with SARS-CoV-2 infection are at increased risk for severe COVID-19 compared to nonpregnant women in Colombia. Methods: We analysed national surveillance data from Colombia, of women aged 15-44 years with laboratory-confirmed infection with SARS-CoV-2 by molecular or antigen testing, from 6 March 2020 to 12 December 2020. An enhanced follow-up of pregnant women with COVID-19 was established to monitor pregnancy and birth outcomes.Results: Of 371,363 women aged 15-44 years with laboratory-confirmed SARS-CoV-2 infection, 1.5% (n = 5614) were reported as pregnant; among those, 2610 (46.5%) were considered a complete pregnancy for reporting purposes at the time of analysis. Hospitalisation (23.9%) and death (1.3%) occurred more frequently among pregnant symptomatic women compared to nonpregnant symptomatic women (2.9% and 0.3%, respectively). Compared to nonpregnant symptomatic women, pregnant symptomatic women were at increased risk of hospitalisation (adjusted risk ratio [RR] 2.19, 95% confidence interval [CI] 2.07, 2.32) and death (RR 1.82, 95% CI 1.60, 2.07), after adjusting for age, type of health insurance and presence of certain underlying medical conditions. Among complete pregnancies, 55 (2.1%) were pregnancy losses, 72 (2.8%) resulted in term low birthweight infants and 375 (14.4%) were preterm deliveries.Conclusions: Although pregnant women were infrequently reported with laboratoryconfirmed SARS-CoV-2 infection, pregnant symptomatic women with COVID-19 were at increased risk for hospitalisation and death compared to nonpregnant symptomatic
Los derechos humanos, que incluyen los derechos de los niños, son concebidos como el contenido sustancial del sistema democrático y enmarcan al Estado y a la construcción social de las familias. El objetivo fue analizar el desarrollo y puesta en marcha de la política pública de la infancia y adolescencia 2011-2021 para Bogotá. Se realizó un análisis cualitativo de base documental fundamentado en la metodología propuesta por Ordoñez-Matamoros. Los resultados evidenciaron que las necesidades definidas fueron interpretadas a conveniencia del tomador de decisión y de acuerdo con la estructura de gobierno de la política. El proceso de identificación de tipologías de la política pública distrital permitió comprender la complejidad del ejercicio y entender que en muchos casos es válida la existencia implícita de múltiples tipos, debido a la diversidad de los contenidos e intencionalidades enmarcados en ella; sin embargo, es necesario que sea incluyente hacia la población en general para que así se transforme en una ventana de oportunidades para el desarrollo humano en todas sus dimensiones.
Introducción: la morbilidad materna extrema (MME) desde la perspectiva de la vigilancia y de la salud pública otorga la posibilidad de la identificación temprana de la complicación severa o grave y que pone en riesgo la vida del binomio madre-hijo, para garantizar la atención adecuada y oportuna que conlleve a la reducción de la probabilidad de desenlaces fatales. Materiales y métodos: se realizó un análisis descriptivo retrospectivo de corte transversal, se calculó la razón de MME, el índice de letalidad y de MPNT y la oportunidad de la notificación para el 2019, junto con las tendencias de los últimos cinco años. Para el análisis de la información se establecieron frecuencias absolutas y relativas. El software utilizado para el análisis de los datos fue Microsoft Excel. Para la estimación de las tendencias se utilizó Joinpoint versión 4.7, teniendo en cuenta que las diferencias significativas se establecieron con p<0,05, para un nivel de significación del 95 %. Resultados: la razón de MME para el 2019 fue de 37,1 casos por cada 1 000 nacidos vivos; 14 entidades territoriales se encontraron por encima del comportamiento nacional. La tendencia de la razón de morbilidad materna extrema para Colombia 2015 a 2019 muestra un comportamiento hacia el aumento. Conclusiones: el comportamiento de la MME en Colombia pone de manifiesto el fortalecimiento de la vigilancia del evento en el país y la aceptabilidad de los actores del sistema para participar de forma activa en la vigilancia.
