La leucemia linfoblastica aguda B (LLA-B) representa una malignidad hematológica con pobre desenlace clínico y bajas tasas de supervivencia en paciente adultos. En Colombia, solo el 61% de los pacientes adultos con LLA-B logran remisión completa con una mediana de supervivencia global de 11.3 meses y una supervivencia libre de evento de 7.34 meses. La identificación de factores de valor pronóstico es crucial para la aplicación correcta de estrategias terapéuticas y subsecuentemente para el desenlace de los pacientes. En este estudio a través de análisis de microarreglos, se identificó una firma de expresión génica que puede distinguir entre el grupo de pacientes con buena y con mala respuesta al tratamiento quimioterapéutico. Adicionalmente, se analizó la relevancia pronostica de los marcadores inmunofenotípicos incluidos en el protocolo EuroFlow y se encontró por primera vez que la evaluación de la combinación de la firma de expresión génica de mal pronóstico y el perfil inmunofenotípico en la cohorte de pacientes Colombianos con diagnóstico de novo de LLA-B puede mejorar la clasificación en grupos de riesgo. Por último, se usó un modelo celular in vitro de LLA-B con modulación de la expresión de los genes de la firma de expresión génica ID1, ID3 y IGJ en el cual se identificó que estos genes podrían ser moduladores de diferentes eventos moleculares que pueden promover la resistencia a la terapia y el desenlace desfavorable de los pacientes adultos con LLA-B.
