The relationship between various factors predisposing to the formation of spondylolisthesis, including degenerative spondylolisthesis, has been analyzed by many authors. However, not all observations are consistent. In this review, we identified factors whose impact on the prevalence of spondylolisthesis was most often mentioned in the literature. These included gender, age, bone mineral density, ethnic origin, and oophorectomy. The results were inclusive in terms of physical activity, pregnancy status, and use of hormone replacement therapy. Associations between diabetes and smoking were very poorly marked. The literature so far has identified a number of factors significantly affecting the incidence of degenerative spondylolisthesis. These include age, gender, body weight, ethnic origin, bone mineral density, and hormonal balance. Radiological parameters, which include iliac crest, pelvic tilt, pelvic incidence, sacral slope, and lumbar lordosis, may also be of great importance for assessing changes in the occurrence and progression. However, the authors do not agree on the real significance of individual factors. The aim of this review was to identify the factors predisposing to the formation of degenerative spondylolisthesis, the importance of which has been suggested in the current literature. The systematization of knowledge in this field can allow a more accurate adjustment of the treatment plan for each patient affected by this condition.

Lumbar disc herniation is caused by degenerative changes in the intervertebral disc, and is one of the most common causes of disability for people under 45 years old. The most commonly affected regions are the L4/L5 and L5/S1 levels. When conservative treatment does not bring the expected result, surgical treatment is necessary. Fast recovery, early mobilisation, and short hospitalisation is what patients expect nowadays. There are a wide range of surgical techniques available, from open surgeries, such as laminectomy, hemilaminectomy, and microdiscectomy, to minimally invasive surgeries, such as percutaneous endoscopic lumbar discectomy, which are becoming more and more popular. Microdiscectomy is a gold standard in spine surgery for lumbar disc herniation. StreszczeniePrzepuklina międzykręgowa, która jest efektem zmian degeneracyjnych w dysku międzykręgowym, stanowi jedną z najczęstszych przyczyn ograniczenia sprawności u osób poniżej 45. roku życia. Zwykle dotyczy poziomu L4/L5 i L5/S1. W przypadku niepowodzenia leczenia zachowawczego konieczne jest leczenie operacyjne. Ze względu na tempo życia współcześni pacjenci oczekują krótkiego okresu rehabilitacji, szybkiego powrotu do sprawności fizycznej, a także krótkiego czasu hospitalizacji. Obecnie istnieje wiele metod operacyjnych leczenia przepukliny międzykręgowej -tzw. techniki otwarte, np. laminektomia, hemilaminektomia i mikrodiscektomia, oraz techniki małoinwazyjne, cieszące się coraz większą popularnością, takie jak endoskopowe usunięcie przepukliny międzykręgowej. Złotym standardem leczenia operacyjnego przepukliny międzykręgowej jest mikrodiscektomia.
Myelopathy is a neurological deficit related to the spinal cord, and cervical spondylosis is its leading cause. The majority of middle-aged people have radiological evidence of significant cervical spine degeneration. The most characteristic symptoms are: neck pain with reduced range of motion, pain and paraesthesias in shoulders, gait disturbance, weakness or instability of lower extremities, loss of manual dexterity, and glove-distribution sensory loss in hands. The pathophysiology of cervical spondylotic myelopathy involves static, dynamic, and ischaemic factors. There are two surgical approaches: anterior and posterior. The posterior approach includes laminectomy and laminoplasty. The anterior approach includes anterior discectomy and fusion, and anterior corpectomy and fusion. This paper reviews the pathophysiology and clinical manifestation of cervical spondylotic myelopathy, and surgical approaches to treat patients with this disease. Streszczenie Mielopatia to choroba neurologiczna związana z patologiami rdzenia kręgowego, jej najczęstszą przyczyną jest spondyloza kanału kręgowego w odcinku szyjnym. Radiologiczne cechy znacznej stenozy w tym odcinku ma większość osób w średnim wieku. Najczęściej występującymi objawami są: ból szyi, ograniczony zakres ruchu w odcinku szyjnym kręgosłupa, ból i parestezje ramion, zaburzenia chodu, osłabienie siły i niestabilność kończyn dolnych, zaburzenia wykonywania czynności precyzyjnych dłońmi oraz zaburzenia czucia w okolicy rąk. Czynniki patofizjologiczne wywołujące mielopatię szyjną dzieli się na: statyczne, mechaniczne i niedokrwienne. Wyróżnia się dwa dostępy chirurgiczne-przedni i tylny. Z dostępu tylnego wykonywane są laminektomia i laminoplastyka. Z dostępu przedniego przeprowadza się discektomię ze stabilizacją oraz korpektomię i stabilizację. W artykule przedstawiono patofizjologię, obraz kliniczny oraz metody leczenia operacyjnego spondylozy szyjnej.
Introduction:Anterior cervical fixation is one of the most effective methods of treating cervical discopathy. The following parameters are distinguished to assess cervical sagittal balance: cervical lordosis angle, T1 slope, and sagittal vertical axis. Aim of the research: To assess the impact of the increase in the cervical lordosis angle on the other parameters of sagittal balance and on the effects of surgical treatment. Material and methods: Thirty patients were qualified for surgical treatment with the method of a single-level ACDF. An X-ray examination was taken before and 3 months after surgery. The following parameters were analysed: T1 slope, sagittal vertical axis, and cervical lordosis angle. The perceived pain was assessed by analysing the results of the NDI questionnaire and the VAS scale. Results: After surgical treatment, the Cobb angle changed statistically significantly (p = 0.001), on average 11.79°. The SVA C2-7 distance did not change significantly (p = 0.697), with a mean of 22.82 mm after surgery. The value of the T1 slope angle increased to 27.38° (p < 0.001). There was a significant reduction in the NDI scale (p < 0.001) and in the VAS scale (p < 0.001), but no correlation with changes in the sagittal balance parameters was demonstrated. Conclusions: The change in cervical sagittal balance seems to be an important prognostic factor in the assessment of the effectiveness of surgical treatment. The individual parameters showed mutual dependencies. The correction of balance parameters after single-level ACDF stabilization does not significantly affect the result of surgical treatment (NDI, VAS). However, this issue requires further research. StreszczenieWprowadzenie: Stabilizacja przednia szyjna jest jedną z najskuteczniejszych metod leczenia dyskopatii szyjnej. Wyróżnia się następujące parametry służące ocenie szyjnego balansu strzałkowego: kąt lordozy szyjnej, kąt nachylenia trzonu Th1, strzałkowa oś pionowa. Zaburzenia spowodowane zmianą parametrów balansu mogą powodować dolegliwości bólowe i zmiany zwyrodnieniowe tego odcinka. Cel pracy: Ocena wpływu wzrostu kąta lordozy szyjnej na pozostałe parametry balansu strzałkowego oraz na efekty leczenia operacyjnego. Materiał i metody: Przeprowadzono prospektywne badanie, do którego włączono 30 pacjentów zakwalifikowanych do operacyjnego leczenia choroby zwyrodnieniowej kręgosłupa szyjnego metodą jednopoziomowej przedniej stabilizacji. Każdy pacjent miał wykonane badanie rentgenowskie przed leczeniem i 3 miesiące po leczeniu operacyjnym. Przeanalizowano następujące parametry: kąt nachylenia Th1, strzałkowa oś pionowa, kąt lordozy szyjnej. Oceny odczuwanych dolegliwości bólowych dokonano, analizując wyniki kwestionariusza NDI i skali VAS. Wyniki: Po leczeniu operacyjnym kąt lordozy szyjnej uległ statystycznie istotnej zmianie (p = 0,001), średnio 11,79°.
