RESUMENIntroducción: la búsqueda de eficiencia en la gestión hospitalaria motiva el empleo de indicadores de gestión que detecten y evalúen la presencia de estancias hospitalarias inadecuadas. Objetivo: diseñar una clasificación de las causas que determinan una estadía hospitalaria inadecuada adaptada al contexto del sistema de salud argentino. Métodos: se revisaron 25 artículos publicados entre los años 1990 y 2009 en revistas argentinas y de otras nacionalidades, en idioma inglés y español. Para la exploración se incluyeron buscadores genéricos de Internet y las bases de datos bibliográficos: Medline, Cochrane y Lilacs. Las palabras clave empleadas fueron: inappropriateness, inappropriate hospital days, the appropriateness evaluation protocol, factors associated with inappropriate hospitalization days, uso inadecuado de la hospitalización, lenght of stay. Sintesis de los datos: es necesario distinguir entre ingresos y estadías inadecuadas. Las causas de estas últimas, según la clasificación diseñada, pueden deberse a la responsabilidad de: 1. La planificación y gestión hospitalaria. 2. El médico. 3. La familia del paciente. 4. El sistema de salud. Conclusiones: el diseño de esta clasificación es una herramienta útil para la implementación de indicadores de gestión hospitalaria que involucren los días de estadía.Palabras clave: gestión hospitalaria, indicadores, estadía inadecuada. Difficulties in hospital planning and management, 2. The physician's responsibility, 3.The responsibility of the patient's family, 4. The responsibility of the health care system. Conclusions: the design of this classification is a useful tool to implement hospital management indicators involving the length of hospital stay.
Para avanzar en el conocimiento y estimación de la carga económica de una enfermedad surgen los estudios de costos de enfermedades. Estos identifican, cuantifican y valoran todos los recursos económicos asociados a una enfermedad, clasificándolos en costos directos, indirectos e intangibles. El presente trabajo tiene como objetivo describir críticamente los diferentes métodos de estimación de cada uno de ellos. La valoración de los costos de una enfermedad generalmente involucra un vector de precios y cantidades. Las cantidades surgen de los recursos consumidos o perdidos. Los precios se estiman a partir del valor de mercado del bien o servicio, o bien, cuando no existe mercado, a partir de variables proxy o métodos específicos de estimación de preferencias individuales. Entre las principales debilidades metodológicas y teóricas encontradas se destacan problemas en la estimación del recurso “tiempo”, presencia de externalidades e imperfecciones de mercado y violación algunos postulados de teoría económica.
Introducción: el crecimiento sostenido del gasto en salud observado a nivel mundial ha dado lugar a la necesidad de asignar e cientemente los recursos. Para ello, las evaluaciones económicas constituyen una valiosa herramienta para la toma de decisiones en el sector salud. Para su implementación se deben estimar los costos de las alternativas a ser evaluadas. Entre los estudios de costos se encuentran los que estiman los costos de enfermedades, los cuales permiten identicar, cuanticar y valorar todos los recursos económicos comprendidos en una decisión relativa al proceso de salud-enfermedad-atención. Objetivo: describir cómo las diferentes perspectivas de análisis pueden modificar la estimación de costos de una condición de salud. Materiales y métodos: se realizó un estudio descriptivo con base en una revisión no sistemática de la literatura acerca de la perspectiva de análisis en los estudios de costos de enfermedades en el periodo comprendido entre 1960 y 2015. Resultados: se observa que las estimaciones de los costos asociados a una enfermedad consideran, en la mayoría de las perspectivas, solo los costos directos sanitarios derivados de las prestaciones necesarias para la prevención y tratamiento de enfermedades. Salvo desde la perspectiva del paciente y su familia, los costos directos no sanitarios no suelen incluirse en las valoraciones económicas de una enfermedad. Los costos indirectos e intangibles solo están presentes en forma indirecta cuando desde la perspectiva del profesional, el Estado o el tercer pagador se toman decisiones en función de estudios del tipo costo-utilidad en los que subyace la calidad de vida de los pacientes. Discusión: no reconocer todos los costos que intervienen en una enfermedad puede subestimar el impacto económico de la misma dando lugar a políticas sanitarias incorrectas, ine ciente asignación de recursos y evaluaciones parciales de los costos asociados a tratamientos o alternativas disponibles, lo cual podría sesgar los resultados obtenidos en estudios del tipo costo efectividad utilizados para guiar la práctica médica y la cobertura de prestaciones.
En Argentina, la provisión de fármacos para pacientes con diabetes mellitus tipo 2 (DM2) sin cobertura de salud se efectúa mediante programas públicos. En la provincia de Buenos Aires coexisten el programa Remediar, de nivel nacional, y el Programa de Prevención, Diagnóstico y Tratamiento del Paciente Diabético (PRODIABA), de nivel provincial. El presente trabajo estima el porcentaje de población adulta con DM2 sin cobertura del municipio de Bahía Blanca (provincia de Buenos Aires) con necesidad de tratamientos de antidiabéticos orales, que satisface su demanda en el sector público. Es un estudio cuantitativo que evalúa la demanda y la necesidad de provisión pública. Los resultados indican que: 1) el mayor porcentaje de la demanda se satisface en el primer nivel de atención; 2) la provincia de Buenos Aires financia el mayor porcentaje de comprimidos, seguida por el nivel municipal y el nacional; 3) el nivel local también interviene para satisfacer la demanda y 4) la provisión pública total solo daría cobertura a aproximadamente el 25% de las necesidades en el escenario correspondiente al consumo medio. Esto muestra que, incluso con diferentes programas públicos, la provisión no es suficiente y se requiere de la intervención local aun cuando la descentralización en la adquisición de fármacos no es recomendada por la teoría económica.
In this paper we analyze the degree of equity in access to the public health care system in the Province of Buenos Aires (Argentina). Through a quantitative retrospective study, we analyze the inequalities in the distribution of the total public health expenditure per capita. This variable is used as a proxy for the ability of the inhabitants of each jurisdiction to access health care services. The results indicate the existence of large disparities in the levels of expenditure devoted to the population without health coverage. Moreover, the existence of greater health care needs (estimated using infant mortality rates and percentage of homes with basic needs unmet) does not translate into higher levels of public expenditure. Finally, we detect a positive association between the relative wealth of municipalities (measured by the gross geographic product per capita) and the public health expenditure per capita.
