Introdução: O leite materno é considerado o alimento mais completo e adequado aos bebês nos primeiros seis meses de vida. É a forma mais natural de vínculo entre mãe e filho, o que torna o aleitamento materno uma estratégia sábia para nutrição e proteção da criança, constituindo a mais econômica e eficaz intervenção para redução da morbimortalidade infantil. Contudo, a adesão a essa prática, ainda é insatisfatória. Objetivo: Analisar os fatores determinantes que influenciam a adesão do aleitamento materno exclusivo por mães da região nordeste e os fatores associados ao desmame precoce. Material e métodos: Foi realizada uma revisão narrativa baseada na investigação de publicações científicas nas bases de dados BVS (Biblioteca Virtual de Saúde), SCIELO (Scientific Eletronic Library Online) e PubMed, com seleção de artigos originais e dados de pesquisas nacionais. Resultados: Foram encontrados artigos que evidenciaram os fatores socioeconômicos (escolaridade materna, trabalho, renda familiar, licença maternidade, apoio familiar e saneamento básico), biológicos (idade materna, tipo de parto, fatores psicológicos, traumas mamilares), políticos (assistência no pré-natal, parto e pós-parto por meio de serviços de saúde qualificados; o desmame precoce é fortemente influenciado pela região geográfica na qual as famílias residem) e culturais (crenças sobre o leite materno e a amamentação, introdução precoce de alimentos sólidos, uso de chupetas e mamadeiras) que influenciam na adesão ao aleitamento materno exclusivo nessa região. Conclusão: Mesmo de acordo com as recomendações da OMS, a prevalência do aleitamento materno exclusivo na região nordeste está abaixo das recomendações, sendo necessário considerar os fatores determinantes que fragilizam e potencializam a prática, e influenciam na adesão, bem como o aprimoramento das políticas públicas e assistência qualificada.
