correlates of NPAFP to discern explanations for the increase. The incidence of polio AFP in India has decreased. However, the nonpolio AFP rate has increased since 2000. Follow-up of these cases of nonpolio AFP is not done routinely. However, one-fifth of these cases of nonpolio AFP in the state of Uttar Pradesh (UP) were followed up after 60 days in 2005; 35.2% of patients were found to have residual paralysis, and 8.5% had died. This suggests that the pathology in children being registered as having nonpolio AFP cannot be considered trivial. Therefore, there is a compelling reason to try to determine the underlying causes for the surge in nonpolio paralysis numbers. METHODS:The data on AFP, polio and nonpolio AFP, and number of polio rounds were examined in each state in each year from 2000 to 2013. Multiple linear regression analysis adjusting for region or state, total and female literacy rate, population density, and per-capita gross domestic product was performed. Differences between states and changes over time were analyzed.RESULTS: NPAFP increased with the number of oral polio vaccine (OPV) doses used (R2 5 25.02%; P , .001). When effect of cumulative doses over the previous years was examined, the NPAFP rate in 2013 best correlated with the cumulative doses received in the previous 7 years (R2 5 57.16%), with 2012 excluded because data for this year were incomplete. This correlation was highly significant (P , .001). On multiple regression analysis, the number of OPV doses was the only factor that showed a positive correlation with the NPAFP rate. The average increase in the NPAFP rate was 1.31 per 100 000 population (P , .001; 95% confidence interval, 1.11-1.52) with each dose of OPV. The NPAFP rate in UP and Bihar, which had consistently increased each year until 2011, decreased in the 2 states in 2012, coinciding with a reduction in doses of OPV administered.CONCLUSIONS: The incidence of NPAFP was strongly associated with the number of OPV doses delivered to the area. A dose-response relationship with cumulative doses over the years was also observed, which strengthens the hypothetical relationship between polio vaccine and NPAFP. The fall in the NPAFP rate in Bihar and UP for the first time in 2012, with a decrease in the number of OPV doses delivered, is evidence of a causative association between OPV doses and the NPAFP rate. Congenital neural tube defects (NTDs) are the most common central nervous system congenital defect and among the most common congenital malformations of all organ systems. NTDs are the leading cause of neonatal and infant mortality and childhood disability. The objective was to develop risk groups for having children with NTDs among reproductive-age women based on the detection of endogenous and exogenous risk factors and to offer a differentiated approach to fetal NTD prevention.

Педиатры и семейные врачи всегда должны действовать в интересах ребенка. Если родители не обеспечивают необходимую ребенку медицинскую помощь, то медицинские работники должны предпринять ряд действий, направленных на выявление и устранение причин медицинской запущенности. Медицинские работники всегда должны выбирать наиболее оптимальный вариант вмешательства для устранения медицинской запущенности, который обеспечивает здоровье и безопасность жизни ребенка.
Актуальність. Симуляційне навчання — відносно новий напрямок медичної освіти, що дуже швидко розвивається. Цей метод навчання особливо важливий для оволодіння навичками екстреної та невідкладної допомоги дітям. Мета статті — обговорити питання впровадження в програму до- та післядипломної підготовки лікарів підходів навчального курсу Американської асоціації кардіологів і Американської академії педіатрів PEARS® із застосуванням симуляційних методів навчання. Матеріали та методи. Авторами наведено досвід впровадження підходів до оцінки, виявлення і стабілізації невідкладних станів у дітей з використанням імітаційних методів навчання. Результати. У симуляційному класі кафедри пропедевтики педіатрії Одеського національного медичного університету 786 студентів і 158 лікарів навчалися основам невідкладної допомоги дітям з використанням підходів навчального курсу «Педіатрична оцінка, розпізнавання і стабілізація невідкладних станів». Студенти і лікарі визначали високу ефективність одночасного засвоєння теоретичного матеріалу та набуття практичних навичок на симуляторах-манекенах. Висновки. При навчанні студентів і лікарів навичкам невідкладної допомоги дітям необхідно використовувати як клінічні протоколи, побудовані на принципах доказовості, так і освітні програми професійних асоціацій розвинених країн.
Одеський національний медичний університет (м. Одеса, Україна) ЯК ОПТИМІЗУВАТИ ПРАКТИКУ ВИГОДОВУВАННЯ НЕМОВЛЯТ Резюме Вступ. Ожиріння -одна з найважливіших неінфекційних епідемій ХХІ сторіччя. Швидке збільшення ваги на першому році життя розглядається як фактор ризику ожиріння в дитячому та підлітковому віці. Педіатри відіграють центральну роль у формуванні здорового харчування та боротьбі з епідемією дитячого ожиріння. Необхідно оптимізувати викладання питань харчування дітей на до дипломному і післядипломному етапах, в першу чергу, це стосується вигодовування малюків. Мета дослідження: оптимізація практики вигодовування немовлят шляхом оновлення змісту безперервної медичної освіти лікарів для покращення їх навичок консультування батьків. Матеріали і методи дослідження. Методом випадкової вибірки проведено добровільне, анонімне анкетування 40 лікарів, які надають первинну допомогу дітям, щодо їх рекомендацій стосовно вигодовування немовлят. За допомогою сервісу Google Forms лікарів просили відповісти на 20 запитань анкети on-line. Розповсюдженість рекомендацій лікарів (self-reported) порівнювали як із сучасними рекомендаціями Guidelines for Infants and Young Toddler (2017), так і з відповідями батьків (self-reported) про існуючу практику годування немовлят. Результати дослідження та їх обговорення. Встановлено, що під час прийому дитини першого року життя лікарі витрачають у середньому 15-20 хвилин на консультування з питань грудного вигодовування, введення прикорму або оцінки та корекції вигодовування у другому півріччі. Не зазнавали труднощів при консультуванні батьків тільки 27,5% лікарів. На думку лікарів, покращенню практики консультування батьків могло б сприяти наявність чітких алгоритмів консультування (42,5%) і друкованого матеріалу для батьків (55%). Основні рекомендації лікарів щодо грудного вигодовування, часу введення прикорму і загального переліку продуктів додаткового харчування є сучасними і достовірно впливають на практику годування малюків. При цьому засновані на доказах рекомендації, що представлені Feeding Guidelines for Infants and Young Toddler (2017), стосовно продуктів прикорму, збагачених залізом, потенційних високо алергенних продуктів, не рекомендованих продуктів та респонсивного годування, виявилися для частини лікарів новими. Висновки. Для оптимізації практики годування малюків необхідно вдосконалити підготовку педіатрів і сімейних лікарів на додипломному етапі та під час безперервної післядипломної освіти. В програмі додипломного навчання необхідно приділяти більшу увагу формуванню таких компетентностей, як ефективне спілкування лікарів з батьками, а елементи консультування щодо грудного вигодовування та респонсивного годування малюків можуть бути включені в перелік навичок об'єктивного структурованого клінічного іспиту. Безперервна післядипломна освіта лікарів передбачає постійне оновлення знань з урахуванням появи оновлених рекомендацій, що базуються на доказах.
