We investigated prevalence of and knowledge about tobacco use among physicians, and their counselling of patients in the Odessa region (Ukraine). Paediatricians (40), family doctors (40) and interns (70) were selected from the physician population of the Odessa region. The proportion of smokers was unacceptably high for health care professionals: paediatricians, 32.5%; family doctors, 37.5%; and interns, 50%. Majority of smokers were men. Less than half of smokers had considered quitting or seriously attempted to quit. Interns least frequently asked their patients about smoking (52.5 vs. 80% paediatricians and 72.5% family doctors). Ukrainian universities need to better educate medical students on tobacco control measures.
BackgroundAcute respiratory infections (ARIs), caused by the high level of immaturity of the immune system, are a major cause of morbidity in preterm newborns. The probiotic Escherichia coli strain Nissle 1917 (EcN) is well known for its immuno-modulatory properties and may therefore enhance the immune competence. Thus, EcN administration may provide a promising possibility to decrease the risk of ARIs in this vulnerable group of children. However, clinical data supporting or refuting this hypothesis are, to our knowledge, not available. Therefore, the aim of the presented pilot trial was to collect first data on the efficacy and safety of EcN treatment to prevent ARIs in late preterm newborns.MethodsRight after birth, 62 late preterm newborns were included into an open-labeled, controlled 4-week trial with two parallel groups and a follow-up phase until the age of 1 year. All children of the treatment group received an EcN suspension orally for 3 weeks, whereas the control group was only observed. Primary efficacy variable was the number of participants with at least one ARI during the first 28 days of life. Secondary efficacy variables were the number of ARIs and the number and duration of hospitalizations caused by ARIs during the first year of life.ResultsThe number of participants with at least one ARI during the first 28 days of life was significantly lower in the group treated with EcN compared to that in the control group. Although only of exploratory nature, analyses of secondary efficacy variables suggest that EcN treatment may also reduce the average number of ARIs, the average number of hospitalizations caused by ARIs, and the mean duration of such hospitalizations. There is also some evidence that early EcN treatment may have long-term benefits on newborns’ health status.ConclusionThe present pilot trial provides first evidence that EcN is able to reduce the incidence of ARIs in the neonatal period of late preterm newborns. Additionally, EcN is characterized by an excellent individual biocompatibility in the absence of adverse drug reactions. Limitations of the current trial are discussed and recommendations for future confirmatory studies are made.Trial registrationClinicalTrials.gov identifier: NCT01540162; retrospectively registered on 16 February 2012
Objective: To identify psychosocial problems and self-esteem in children with growth hormone deficiency (GHD) and define the role of some clinical and sociodemographic determinants in the conceptualization of internalizing and externalizing problems as criteria for psychosocial functioning.Materials and Methods: A GHD sample (46 prepubescent children) was selected and compared to a matched control group (80 healthy children). Psychosocial functioning in children with GHD was investigated using Goodman's “Strengths and Difficulties Questionnaire (SDQ).” The study of children's self-esteem was carried out by the Dembo–Rubinstein method.Results: This study reveals that the GHD sample has more internalizing problems and lower self-esteem. Higher score and frequency of assessment in the abnormal score for “total difficulties,” “emotional problem,” and “peer problem” were found in children with GHD. The SDQ score and the frequency of assessment in the abnormal score for all SDQ scales in children with more pronounced growth deficit (height SDS < −3) did not exceed the same indicators in children with less growth retardation (−3 < height SDS < −2). A comparison of psychosocial features in children with isolated growth hormone deficiency and multiple pituitary hormones deficiency did not reveal differences in SDQ score and the frequency of assessment in the abnormal score for all SDQ scales. It was found that children with GHD have a reduced level of assertions, low self-esteem, and a weak discrepancy between the level of assertions and self-esteem. Some sociodemographic determinants (male gender, age < 9 years, and low family income) and clinical determinants (low compliance and suboptimal growth response after 1 year of rGHh therapy) have an impact on the overall assessment of psychological problems in children with GHD. The internalizing difficulties are associated with certain clinical determinants (growth status and treatment status) and sociodemographic determinants (female gender, age < 9 years).Conclusions: The identification of low self-esteem and the high SDQ score for scales “total difficulties,” “emotional problems,” and “peer problems” indicates psychosocial maladjustment and conceptualization of internalizing problems in children with GHD.
Researchers have known for over 20years that the concentration of toxic substances in the auxiliary jet of a tobacco cigarette (i.e. the jet that is emitted at the front of the lit cigarette) is much more harmful than in the smoke that is inhaled directly. This auxiliary jet is what causes the opportunity for passive smoking among individuals in the same environment. Furthermore mothers with new born infants who hold their child close, breathe out air that has toxic substances in the baby's face for the child to breathe in. This study investigated the prevalence of passive smoking exposure among infants in the Odessa region and to determine the frequency of infants-passive smokers with pneumonia of different severity among hospitalized infants to the Odessa Regional Children's Clinical Hospital. The questionnaire covered information on prevalence of smoking at home, mother nicotine dependence, frequency and severity of diseases in children. Two hundred infants were recruited from infants who visited the outpatient clinic № 6 of the Odessa region in "The Day of healthy child". The survey revealed an unexpectedly high prevalence of second hand smoke exposure among infants of the Odessa region (24.5%). The prevalence of IUGR, HIE, recurrent respiratory diseases among infants -passive smokers is significantly higher than among infants which are free from second hand smoke exposure.
