Aortic valve lesion is a common and may have diverse causes, from degenerative, congenital and infectious diseases to autoimmune conditions. We present a rare case of Takayasu arteritis and severe heart lesion due to the myxomatous degeneration of the aortic and mitral valves associated with development of infective endocarditis (IE) complicated by abscess, fistula, valve perforation and recurrent acute decompensated heart failure in a young female patient. A combined use of histopathological and PCR analyses of valve tissues was critically important for differential diagnosis of the valve lesions, as it made it possible to identify the true cause of the disease. The presence of Takayasu arteritis has played an indirect role by creating conditions for the development of immunosuppression and determining the disease severity and its progression.
П о гистогенезу амелобластому относят к одон-тогенным опухолям [1], однако ее отличитель-ная особенность от других доброкачественных опухолей -местнодеструирующий характер роста и склонность к рецидивированию [2,3]. На сегодняшний день хирургический метод -основа лечения амелобла-стомы [4]. Можно выделить две основные группы опера-тивных вмешательств -это обработка костной полости фрезой и радикальная резекция челюсти (гемимандибу-лоэктомия или мандибулоэктомия), на которую пациент не всегда согласен и настаивает на вмешательстве в меньшем объеме. Проведение ограниченной резекции, по сравнению с радикальной операцией, требует заметно меньшее количество времени и является более щадящей для пациента. Метод имеет и негативные черты -в лите-ратуре приводится немало сведений о рецидиве после первичной операции. Частота возникновения рецидивов зависит от вида оперативного вмешательства. Цистотомия, которую обычно назначают для удаления образования небольших размеров, приводит к рецидиву в 90-100% случаев, в связи с прорастанием опухолью прилежащих тканей до 1,0 см [5]. В зарубежных литературных источ-никах приводятся данные, что в 6,1% случаев даже при проведении радикальной операции отмечалось
